Morocco Abstract Rhabdomyosarcomas are the most common soft tissue sarcomas developed in children under 15 years of age. The reported fi nding is a vaginal tumour developed in a 30-month-old granddaughter. This was a typical botryoid rhabdomyosarcoma that usually occurs in the hollow organs lined with mucous membrane. Rhabdomyosarcomas have multiple aspects that vary according to the degree of cellular differentiation. The majority of these tumours can be classifi ed into four histological categories: embryonic, botryoid, alveolar or pleomorphic. Treatment involves excisional surgery combined with radiotherapy and chemotherapy. The prognosis remains bleak despite therapeutic advances in recent years. Introduction Anatomopathological examination of the specimen showed that it was a polypoid (grape-like) tissue bordered Rhabdomyosarcomas are the most common soft tissue by a slightly hyperplastic squamous epithelium. Histological tumour type (Required) Reason/Evidentiary Support All ovarian epithelial malignancies and borderline tumours should be typed according to the WHO classification.1 There are 5 major subtypes of primary ovarian carcinoma, high‐grade serous, clear cell, endometrioid, mucinous and low‐ grade serous.2‐5 There are also other uncommon minor subtypes, those listed by the WHO including malignant Brenner tumour, seromucinous carcinoma and undifferentiated carcinoma.1 Carcinosarcoma is a mixed epithelial and mesenchymal malignancy but is included in the category of epithelial malignancies in this dataset since most are of epithelial origin and histogenesis.6 Although management of ovarian carcinoma is, at present, largely dependent on tumour stage and grade, accurate typing will almost certainly become more important in the future with the introduction of targeted therapies and specific treatments for different tumour types. This is in part because, although clinically often considered as one disease, there is an increasing realisation that the different morphological subtypes of ovarian carcinoma have a different pathogenesis, are associated with distinct molecular alterations and have a different natural history, response to traditional chemotherapy and prognosis.2‐5 Tumour typing may also be important in identifying or initiating testing for an underlying genetic predisposition; for example, high‐grade serous carcinoma may be associated with underlying BRCA1/2 mutation while endometrioid and clear cell carcinomas can occur in patients with Lynch syndrome.7 The most common ovarian carcinoma is high‐grade serous carcinoma (approximately 70%) followed by clear cell and endometrioid.8,9 Mucinous and low‐grade serous are less common. Approximately 90% of advanced stage ovarian carcinomas (stage III/IV) are high‐grade serous in type.8,9 Most primary tubal carcinomas are high‐grade serous or endometrioid and most primary peritoneal carcinomas are of high‐grade serous type. -

Ebeling, [email protected] Abstract Desmoid tumors, also known as aggressive fibromatosis, are fibromuscular neoplasms that arise from mesenchymal cell lines. They may occur in almost all soft tissue compartments. Primary desmoids of the small bowel are rare but potentially serious tumors presenting unique challenges to the general surgeon. We present one case of a 59-year-old man presenting with three months of abdominal distension secondary to a small bowel desmoid. Computed tomography of the abdomen showed an 18-cm mass in the mid-abdomen without obvious vital structure encasement. Percutaneous biopsy of the mass indicated a desmoid tumor. The patient underwent a successful elective exploratory laparotomy with resection and primary enteric anastomosis. Final pathology revealed the mass to be a primary desmoid of the small bowel. His post- operative course was uneventful. At two years after surgery, he is symptom free, and there is no evidence of disease recurrence. Due to the rare nature of primary small bowel desmoids, there are few specific care pathways outlined. This is a challenging pathology to treat that often requires a multidisciplinary team of surgical and medical oncologists. Persistent gestational trophoblastic disease following molar pregnancy is a potentially fatal complication that must be recognized early and treated aggressively for both immediate and long-term recovery. We present the case of a 21-year-old woman with abdominal pain and presyncope 1 month after a molar pregnancy with a subsequent uterine rupture due to invasive gestational trophoblastic neoplasm. We will discuss the complications of molar pregnancies including the risks and management of invasive, metastatic gestational trophoblastic neoplasia. -

cancer.org | 1.800.227.2345 Understanding Your Pathology Report: Benign Breast Conditions When your breast was biopsied, the samples taken were studied under the microscope by a specialized doctor with many years of training called a pathologist. The pathologist sends your doctor a report that gives a diagnosis for each sample taken. Information in this report will be used to help manage your care. The questions and answers that follow are meant to help you understand medical language you might find in the pathology report from a breast biopsy1, such as a needle biopsy or an excision biopsy. In a needle biopsy, a hollow needle is used to remove a sample of an abnormal area. An excision biopsy removes the entire abnormal area, often with some of the surrounding normal tissue. An excision biopsy is much like a type of breast-conserving surgery2 called a lumpectomy. What does it mean if my report uses any of the following terms: adenosis, sclerosing adenosis, apocrine metaplasia, cysts, columnar cell change, columnar cell hyperplasia, collagenous spherulosis, duct ectasia, columnar alteration with prominent apical snouts and secretions (CAPSS), papillomatosis, or fibrocystic changes? All of these are terms that describe benign (non-cancerous) changes that the pathologist might see under the microscope. They do not need to be treated. They are of no concern when found along with cancer. More information about many of these can be found in Non-Cancerous Breast Conditions3. What does it mean if my report says fat necrosis? Fat necrosis is a benign condition that is not linked to cancer risk. -

We describe a Center case of intrapulmonary A-V shunt as the etiology of an acute ischemic event. A 63 year old male with a past history of (Dominik supraventricular tachycardia and recurrent deep vein thrombosis; who has been non-compliant on Rivaroxaban, presents with Zampino) pleuritic chest pain and was found to have a right lower lobe pulmonary embolus. The deep vein thrombosis and pulmonary embolus were not significant enough to warrant ultrasound-enhanced thrombolysis by Ekosonic EndoWave Infusion Catheter System, and the patient was subsequently restarted on Rivaroxaban and discharged. The patient presented five days later with left arm tightness and was found to have multiple areas of punctuate infarction of both cerebellar hemispheres, more confluent within the right frontal lobe. Of note he was compliant at this time with Rivaroxaban. The patient was started on unfractionated heparin drip and subsequently admitted. On admission, his vital signs showed a blood pressure of 138/93, heart rate 65 bpm, and respiratory rate 16. Cardiopulmonary examination revealed regular rate and rhythm, without murmurs, rubs or gallops and his lungs were clear to auscultation. Neurologic examination revealed intact cranial nerves, preserved strength in all extremities, mild dysmetria in the left upper extremity and an NIH score of 1. -

HLRCC is characterized by benign leiomyomas of the skin and the uterus, renal cell carcinoma, and uterine leiomyosarcoma. The aim of this study was to identify new families with FH mutations, and to further examine the tumor spectrum associated with FH mutations. FH germline mutations were screened from 89 patients with RCC, skin leiomyomas or ovarian tumors. Subsequently, 13 ovarian and 48 bladder carcinomas were analyzed for somatic FH mutations. Two patients diagnosed with ovarian mucinous cystadenoma (two out of 33, 6%) were found to be FH germline mutation carriers. One of the changes was a novel mutation (Ala231Thr) and the other one (435insAAA) was previously described in FH deficiency families. These results suggest that benign ovarian tumors may be associated with HLRCC. -

Introduction We live in an age when it is increasingly important to have ‘key facts’ and ‘headline messages’. The national registry for bone and soft tissue sarcoma want to be able to produce high level factsheets for the general public with statements such as ‘There are 2000 soft tissue sarcomas annually in England’ or ‘Survival for soft tissue sarcomas is (eg) 75%’ It is not possible to write factsheets and data briefings like this, without a shared understanding from the SSCRG about which sarcomas we wish to include in our headline statistics. The registry accepts that soft tissue sarcomas are a very complex and heterogeneous group of cancers which do not easily reduce to headline figures. We will still strive to collect all data from cancer registries about anything that is ‘like a sarcoma’. We will also produce focussed data briefings on sites such as dermatofibrosarcomas and Kaposi’s sarcomas – the aim is not to forget any sites we exclude! The majority of soft tissue sarcomas have proved fairly uncontroversial in discussions with individual members of the SSCRG, but there were 7 particular issues it was necessary to make a group decision on. This paper records the decisions made and the rationale behind these decisions. 2. Summary of SSCRG’s decisions: Include all tumours with morphology codes as listed in Appendix A for any cancer site except C40 and C41 (bone). -

The subject is now of unquestionable importance and evidently of great interest to Ophthalmologists, whether they are orbital- palpebral specialists or not. Indeed, errors or delays in the diagnosis of tumor pathologies are relatively common and the consequences can be serious in the case of malignant tumors, especially carcinomas. Swift diagnosis and anatomopathological confirmation will lead to a treatment, discussed in multidisciplinary team meetings, ranging from surgery to radiotherapy.
