The LifeWise Connect network will change in the new year. Individuals and families with an Exclusive Provider Organization (EPO) plan from LifeWise Health Plan of Washington have access to the LifeWise Connect network.
Starting Jan. 1, 2017, the following health systems will no longer be in the LifeWise Connect network:
• CHI Franciscan Health, with the exception of the facilities in Kitsap County listed on the next page • Providence Health & Services, which includes Swedish Health Services, Swedish Edmonds, and PacMed systems
These changes do not impact LifeWise grandfathered plans.
Key facts for 2017
• Customers with the LifeWise Connect network will have access to more than 33,000 providers across Washington state. These providers share our commitment to quality care, managed costs, and a better customer experience.
• Except for emergencies, care outside of the individual plan’s network is not covered.
• A customer’s out-of-pocket costs will be lower if they choose a doctor or hospital within their network.
• Customers can find in-network doctors and hospitals by using the Find a Doctor tool on lifewisewa.com. LifeWise Connect network starting Jan. 1, 2017
