Licensure Requirements The State of Alabama requires that anyone operating or advertising as a massage therapist ​ or offering massage therapy services be licensed by this Board. All business or establishments offering massage therapy services must be licensed unless specifically exempt. The Massage Therapist License requirements are a minimum of 650 hours from an accredited massage therapy school. Additionally, an applicant must have taken and passed either the NCBTMB exam or the MBLEx exam. Each Massage Therapist applicant must also produce evidence of a $1,000,000 Personal Liability Insurance coverage along with certified transcripts and verification of passing one of the approved exams. An Application request form is available under the FORMS section of this website, at the cost of $25.00 (Cashier’s Check or Money Order) per application package.

The Massage Therapy Establishment License requirements are completion of the licensure application and producing evidence of professional liability insurance. The Massage Establishment application is under FORMS of this website.

Massage Therapy Licensure in Alaska

With the passage of Bill 328, Alaska joins approximately 45 other states in regulating the massage therapy profession. Massage practitioners in some parts of the state (for example, Anchorage and Fairbanks) already work under licenses. Since July 1, 2015, massage therapy has been a licensed profession throughout the state of Alaska.

The licensing mandate will be delayed for massage practitioners who are already working in the state and who hold credentialing through an approved third party. The Commission of Commerce, Community, and Economic Development reports, though, that even these practitioners must hold state licenses by July 1, 2017.

HB 328 provided the statutory basis and sets basic requirements. The Alaska Chapter of the American Massage Therapy Association (the state’s professional organization) has worked with local governments to establish a State Board, which was appointed February 20, 2015. Alaska massage therapists may be licensed by examination or ​ endorsement. License by Examination

Massage therapists who are new to the profession will be licensed by examination (http://commerce.state.ak.us/dnn/cbpl/ProfessionalLicensing/BoardofMassageTherapists/FAQs.aspx). ​ ​ The license by examination process includes two main components: education and examination.

Education: A prospective massage therapist will need to complete at least 500 hours of education through an approved massage therapy school. State statute defines “approved massage therapy school” as one that has been authorized by the Alaska Commission on Postsecondary Education or comparable out-of-state organization or that holds accreditation through a nationally recognized agency.

The Board discussed accrediting agencies at their first meeting (http://commerce.state.ak.us/dnn/cbpl/ProfessionalLicensing/BoardofMassageTherapists.aspx). A motion was made ​ ​ to approve the following: the American Massage Therapy Association (AMTA), Associated Bodywork and Massage Practitioners (ABMP), the Commission on Massage Therapy Accreditation (COMTA), and the National Certification Board for Therapeutic Massage & Bodywork (NCBTMB).

State statute also allows for the possibility of licensure by examination following completion of an approved program.

Applicants for licensure by examination will also need to have four hours of safety training; this must include coverage of blood borne pathogens/ universal precautions.

Examination: The Board has approved examinations offered by two organizations: the Federation of State Massage Therapy Boards (FSMTB) and the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB).

By agreement between the two credentialing organizations, the FSMTB is now the sole organization that provides an examination designed for initial massage therapy licensure or entry-level practice. The FSMTB examination, the MBLEx, is accepted in most states. It is multiple choice. Candidates may submit applications online or through the mail; registration must be accompanied by a $195 fee. The Authorization to Test (ATT) grants a 90 day eligibility window (https://www.fsmtb.org/). Examinations are scheduled at Pearson Vue testing centers around the nation; ​ ​ there is a testing center located in Anchorage.

The NCBTMB (http://www.ncbtmb.org/) offers the Board Certification Examination for Therapeutic Massage and ​ ​ Bodywork (BCETMB). It is designed as one component of a Board Certification process intended for experienced practitioners. The NCBTMB has offered other examinations in the past. A prospective Alaska massage therapist may want to seek clarification about whether all NCBTMB examinations are accepted for licensure.

License by Endorsement

License by endorsement can be based on state licensure or certification that is acceptable to the Board. The ​ out-of-state license must be in good standing.

Transitioning to Licensing

Current massage therapists may be licensed without meeting all education and examination requirements.

There are multiple requirements for transitional licensing. The first is to provide evidence of having operated, owned, or worked for a massage business prior to the effective date. The licensing agency will accept multiple forms of documentation including municipal licenses, professional association memberships, copies of income tax returns, or sworn statements by employers. A massage therapist who uses income tax records will need to document or ownership during only one of the five years preceding the July 1, 2015 effective date. A massage therapist who uses an employer statement must document that the employment took place during the five year period preceding July 1, 2015. Professional association membership can be accepted if the organization was established before 2000, has a code of ethics, and offers liability insurance as a membership benefit; the member will need to have been active for at least one year. Municipal licenses can be license qualifying if they were held on June 30, 2015.

Additionally, the massage therapist must hold national certification unless he or she is licensed in another state on the basis of requirements at least on a par with those of Alaska.

Additional Requirements: CPR Certification and Criminal Background Checks

The minimum age for licensure is 18.

All prospective massage therapists will need to complete CPR certification and clear criminal background checks; this is the case regardless of the method of licensure.

The fingerprint-based background check will be carried out by the Alaska Department of Public Safety. A criminal history will not preclude licensure in all instances. However, crimes that are considered to involve “moral turpitude” are disqualifying. State statute does allow the Board to use discretion in licensing an individual who has been convicted of such a crime if it is determined that the circumstances are such that the professional is capable of safe, competent practice.

Alaska Commission on Postsecondary Education

To be authorized to operate by the ACPE, an institution must, according to their website: ​ ​

● submit an application ● pay appropriate fees ● provide required surety ● meet minimum standards regarding quality of education, ethical business practices, and fiscal responsibility as described in law (Alaska Statute 14.48 and Section 20 of the Alaska Administrative ​ ​ ​ Code Chapter 17). ​ ● These standards describe in the application packet. ● complete an on-site facility visit ● receive final approval by the Commission

To begin the process, you will have to request an application packet from the commission. You can do this by email at [email protected] or call to request it at 907-465-6741. Once the application is complete and mailed back ​ ​ to the commission. It will review by the commission at one of their quarterly board meetings. You must turn in the application approximately 3 months before the meeting at which it will review.

For instance, in order for it to review at the April meeting, the application must receive by January 15. The cost for initial authorization is $2500 and renewals can range from $500 to $2500, depending on your tuition receipts. Authorization is not transferable, so if you sell your school, the new owner must apply for a new authorization.

National Accreditation Agencies

The Alaska Board listed AMTA, ABMP, NCBTMB, and COMTA as credentialing agencies they would recognize as ​ ​ ​ ​ proof of national accreditation. To become a school member of AMTA, your school must have a minimum of 500 in-class hours and must accreditor approved by the state. You can become a school member by contacting AMTA by telephone at 877-905-0577 and asking to speak to a school account executive. To become a school member of ABMP, you will have to contact them by phone or email.The phone number is 800-458-2267. Their email is [email protected]. Both ABMP and AMTA are membership organizations whose purpose is to provide support ​ and benefits for their members. Both offer no-cost memberships for schools, but they require that school owners sign up their students as student members in their organization. They also offer liability insurance programs for students, educators, professionals and schools. NCBTMB offers an “assigned school” status at no cost for schools that meet ​ ​ certain criteria including a 500-hour program with a minimum number of hours in anatomy and physiology, hands-on instruction, pathology, and business and ethics.You must submit an application that shows that your school is legitimate, operating legally, and meets their minimum curriculum criteria. Students cannot sit for the NCBTMB certification exam unless their school is an assigned school. COMTA accreditation can take 1-2 years to achieve and ​ ​ can only be obtained after your school is already operational. The major requirements are at least 5 students enrolled, approved by the state, offer your program at the post-secondary level and at least 600 hours of training. Therefore, you could only obtain COMTA accreditation after you obtained authorization from the ACPE.

Notice of proposed relating to licensure requirements, education requirements, license renewal, continuing education requirements, and application submissions in the regulations of the Board of Massage Therapists Proposed Regulations - FAQ November 2018 1. What is the purpose of the proposed regulations? What will this regulation do? The purpose of the proposed regulations is to implement the changes to Sections 3 and 4 of House Bill 110 (HB 110) dealing with course of study requirements for qualifications for initial licensure, continuing education requirements, and fingerprint information requirements for license renewal, which will take effect on July 1, 2019, and to also update and clarify current regulations. Proposed changes to 12 AAC 79.100 will update the required course of study hours from 500 to 625 of in-class supervised instruction and clinical work from an approved massage school effective July 1, 2019; additionally, the board will accept apprenticeship programs that meets the ​ requirements and standards of the Commission on Massage Therapy Accreditation (COMTA) and the requirements of the Entry Level Analysis Project (ELAP) standards (there are currently none offered in Alaska. There is one in development by the Dept. of Labor). This will allow persons to be able to apply for a license by evidence of completion of a board-approved apprenticeship program. Also, effective July 1, 2019, the required hours for safety education covering bloodborne pathogens and universal precautions is changing from four to two hours. Proposed changes to 12 AAC 79.110 will require that verifications or credentials such as transcripts, exam scores, and license verifications be sent directly from the appropriate agency to the division office. Proposed new section 12 AAC 79.140 establishes the criteria for massage school ​ education or curriculum requirements. To clarify the breakdown of what is required to be considered a massage therapy school or program. Proposed changes to 12 AAC 79.200 will update the requirements for submitting fingerprints and fingerprint processing fees for license renewal to at least once every six years instead of every two years, i.e., if you submitted fingerprints and fingerprint processing fees with your renewal during the September 2017 renewal period, your next fingerprints and fingerprint processing fees won’t be required until the September 2023 renewal period (while a time frame suggested by the Division is no fingerprints for 2019 renewal and starting in 2021 the 6 year increment will be started, it has not been voted on by the board). MAS FAQ November 2018 Page 2 Proposed changes to 12 AAC 79.210 effective July 1, 2019, will update the continuing education credit from four to two hours for completion of a bloodborne pathogens and universal precautions course once every six years. Proposed new section 12 AAC 79.920 establishes a staleness date period for incomplete application file. To ensure that application can’t remain pending for years and all information provided would remain relevant. 2. What are the costs to comply with the proposed regulations? No additional cost. All application, renewal, and fingerprint processing fees would remain the same. A reduction of fingerprint processing fee of $120 for two biennial renewal cycle. 3. Will the education requirements affect already licensed massage therapists? No, the proposed changes to the education requirements will affect initial applicant trying to get licensed. However, proposed changes to the continuing education requirements for those who want to do bloodborne pathogen training as part of their CE’s, the allowable amount hours will decrease from 4 to 2 hours. 4. When will the regulations be effective? After public comment deadline, comments received are compiled and given to the Board for consideration. The Board may adopt the regulation as written/publicly noticed, may amend and adopt them, or choose to take no action, or may withdraw the proposed regulations in part or in its whole. After Board action, the adopted regulations goes to Department of Law (DOL) for final review/approval. DOL either approves or disapproves regulations. Once approved by DOL, it goes to the Lt. Governor for filing. Regulation takes effect on the 30th day after they have been filed by the Lt. Governor.

California Requirements for CAMTC Certification

The California Massage Therapy Council issues voluntary certifications to massage professionals that meet the requirements in the California State Law. Certification is voluntary. CAMTC Certification is not required ​ ​ ​ by State Law in order for a massage professional to practice their profession in the State. However, some ​ ​ cities and counties locally require CAMTC Certification for massage professionals. To find out if your city or ​ county requires CAMTC Certification, please contact the city or county in which you wish to practice. Local permits may be needed for each city or county where you practice unless you are CAMTC Certified. If you are in an unincorporated area, please check with the county. Massage professionals may find CAMTC Certification useful as it allows certified individuals to practice their profession in all cities and counties in the State without the need to get a local permit to provide massage for compensation.

California Business and Professions Code section 4611 makes it an unfair business practice for anyone not certified by CAMTC to use the titles: Licensed, Certified, Certified Massage Therapist, Certified Massage Practitioner, CMT, or CMP.

In order to obtain CAMTC Certification, an applicant must submit a fully completed application and meet all of the following requirements:

● Be 18 years of age or older; ● Pay the $200 fee; ● Submit fingerprints to an authorized agency in California for a Live Scan using only the CAMTC form at Request for Live Scan Service (the authorized agencies charge you a fee for this service) and ​ ​ pass a criminal background check; ● Have successfully completed, at a CAMTC Approved School, curricula in massage and related ​ ​ subjects totaling a minimum of 500 hours (or the credit unit equivalent), which incorporates appropriate school assessment of student knowledge and skills. Of the 500 hours, a minimum of 100 hours shall be in anatomy and physiology, contraindications, health and hygiene, and business and ethics; and ● Have not violated any of the provisions of the California Massage Therapy Act or any policy, procedure, or regulation of CAMTC. ● Make a good, clean photocopy of the front of your Driver’s License or Official State Identification to include with your Application. You are required to send a clear copy of your driver’s license, passport, military ID, or other government issued photo identification card with your Application for photo matching purposes. - ● Get Your Passport Photo Taken. ○ You’ll need to include a recent (less than 60 days old) passport photo of yourself along with your completed Application. Don’t take this picture yourself; instead, please locate a local vendor that advertises and specializes in passport photos to ensure that your photo is taken properly. Be sure to legibly print your name and Driver’s License or Official State Identification number on the back of the photo that will be taped to your Application. ○ Your head must face the camera directly with full face in view. ○ You must have a neutral facial expression or a natural smile, with both eyes open. ○ Taken in the last 2 months. ○ Use a plain white or off white background. - ○ 2x2inches(51x51mm). ○ Head must be between 1 1 3/8 inches (25 35 mm) from the bottom of the chin to the top of - - the head. ○ Printed on matte or glossy photo quality paper. ○ Printed in color. ○ Your full face must be visible.

Please make sure that you have updated your contact information as required by California State Law, including your phone number, primary email address, home address and each workplace address, within 30 days of any changes. This will help to ensure that you receive a reminder of your need to recertify.

Recertification

CAMTC Certification is for a two year time period. Certificate holders must therefore apply for CAMTC Recertification every two years. Certificate holders must apply for CAMTC Recertification within eighteen (18) months of the date of the expiration of their certificate and pay a late fee. Under no circumstances can this fee be waived.

RECERTIFICATION LATE FEE SCHEDULE

Late Fees to apply for recertification after expiration are in addition to the $200 application processing fee. The Late Fee is based on the date a fully completed application is received by CAMTC and the number of days after the Expiration Date on your CAMTC ID Card:

Up to 10 Days After Expiration: $ 50

Up to 29 Days After Expiration: $ 80

Up to 18 Months After Expiration: $ 180

If expired certificate holders fail to apply for CAMTC Recertification within this time frame, then they must re-apply as a new applicant. This means that they must meet all requirements for CAMTC Certification existing at the time of re-application, including education requirements and possibly exam requirements if required at the time of re-application.

There are no continuing education requirements for CAMTC Certificate Holders.

Closed School Policy

CAMTC will accept education for CAMTC Certification purposes from schools closed on or before June 30, 2016, with either no lawful custodian of records or a lawful custodian of records that is not a CAMTC Approved School as long as all of the following conditions are met:

1) The education occurred at a time when the closed school was approved or accredited by at least one of the organizations listed in California Business and Professions Code section 4601;

2) The closed school is or was not subject to any disciplinary actions or pending investigations by any approval or accrediting agencies, Law Enforcement Agencies (LEA), government agencies, CAMTC or other massage or school related entities;

3) The applicant for CAMTC Certification provides verifiable proof that the education received at the closed school meets minimum standards for training and curriculum and the statutory education requirements for CAMTC Certification; and

4) The individual application for CAMTC CMT Certification is received in the CAMTC office on or before December 31, 2020. Applicants whose applications are received after this date may still use education from closed schools described in the Policies and Procedures for Approval of Schools section 3.A.a. for CAMTC Certification purposes as long as they either provide proof of current and continuous city and/or county permit(s) to provide massage for compensation, or they pass a CAMTC Education Hearing (oral telephonic hearing or consideration of a written statement).

Out-of-State Education

Education and training received outside of California must be, at a minimum, substantially equivalent to the requirements applied to California school programs. One important aspect of educational program equivalence is whether the educational institution attended has been approved by the national, regional, or state authority with responsibility for vocational program approvals. Such status can readily be ascertained by CAMTC for each of the 50 U.S. states, U.S. territories.

California Healing Arts College has been training quality Massage Therapists in Southern California since 1988, and our reputation has grown over time, because of the success of our Massage Therapist graduates.

The Massage Therapist Program thoroughly prepares you for work in a variety of professional massage therapy ​ ​ environments including but not limited to chiropractic and medical clinics, health clubs, beauty salons and spas.

The nine hundred (900) clock hour training program at California Healing Arts College offers extensive practical and theoretical training in: Massage Theories and Foundations; Swedish Massage; Deep Tissue Massage; Integrative Massage; Body Mechanics; Anatomy, Physiology and Pathology; Kinesiology; Business and Ethics, Indications and Contraindications; and Health and Hygiene.

The Massage Therapist training program at California Healing Arts College is designed to prepare graduates for California Massage Therapy Council (‘CAMTC’) Certification or for individual city and county licensing requirements in California. Students are sufficiently trained to sit for the Massage & Bodywork Licensing Examination (“MBLEx”) while completing this program.

License

Massage Therapy licensure requirements differ based on the state in question. In states with Massage Therapy regulations, workers must graduate from a training program and obtain a license prior to practicing a massage. Typically, a Massage Therapist must pass an examination to obtain a license. There are two types of certification tests offered by the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB):

● National Certification Examination for Therapeutic Massage and Bodywork (NCETMB) ● National Certification Examination for Therapeutic Massage (NCETM)

Massage therapy licensure boards decide which certifications and tests to accept on a State-by-State basis. In states that use the National Certification Examination (NCE), if a Massage Therapist has passed the appropriate exam in one state, he or she can apply for licensing in another state without having to re-take the test. In states that do not use the NCE, even if a Massage Therapist in another state, he or she is required to either take that state’s test or apply for reciprocity, which is typically granted on a case-by-case basis. Reciprocity refers to the exchange, recognition or enforcement of licenses, privileges or obligations between states of the US or between nations. Massage Therapy (MT) State Certification Programs:

MT250 Program (250 hours): - Recommended State Certification Curriculum MT250 Program has the curriculum recommended to apply for a State Certificate as a Massage Practitioner through ​ the California Massage Therapy Council (CAMTC). A State Certification would allow you to practice in any city in California. Our MT250 program starts with our MT150 Program (10 weeks) and runs an additional 7 weeks totaling 17 weeks in length. Students attend class four times per week. MT250 is available for both day and evening classes and run concurrently.

MT500 Program (500 hours): - Recommended State Certification Curriculum ​ Our MT500 Program is recommended for applying for State Certification as a Massage Therapist through the ​ California Massage Therapy Council (CAMTC), which allows you to practice in any city in California. Certification for Massage Therapist will require your taking and passing a test. To earn a certificate for the MT500 Program you will need to take our MT250 Program and 250 additional hours of electives. When you have completed the hours and passed any and all applicable tests of the additional 250 hours of electives, you will be issued a MT500 Massage Therapist Certificate of Completion.

● Function as the apprentice’s “on-boarding buddy” and go-to for questions about policies, procedures, and business how-tos, as well as bodywork advice ● Receive a complementary bodywork session from the apprentice every other week and provide feedback on technique and progress ● Oversee the apprentice’s work on at least one case study with a regular client. The apprentice and mentor will meet monthly to discuss the case study progress in addition to other supervision. ○ If needed, this case study client will pay a reduced rate to ensure they can come in for monthly work. The apprentice will receive the same pay for the session regardless of client’s adjusted rate. ○ If possible, this case study client will receive work related to the apprentice’s area of continuing education. ● Meet monthly with the apprentice to provide supervision on bodywork sessions including technique, contraindications, session design, client intake, client identity, and boundaries. ● Be paid for their time. o Apply Applicants must have completed their massage therapy programs and should be licensed LMTs in the District of Columbia. Massage Apprenticeship Program

Under State of Hawaii laws*, and individual who has completed a minimum of 150 total hours of training with 50 hours of classroom work in anatomy, physiology and structural kinesiology and 100 hours massage theory and practice at an approved school, he/she may apply to complete an additional minimum training of 420 hours under the supervision of an approved licensed State of Hawaii massage therapist (MAT) who has held a massage license for a minimum of three years. This practical training must take place at a licensed massage establishment (MAE) under the direct supervision of a Board of Massage approved massage therapist.

After completing a minimum of six months to a maximum of one year of documented supervision of hands on work of 310 hours with requirements in Clinical operations, 70 hours, and Advanced techniques, 40 hours, as well as documenting current CPR training from either the American Red Cross, or the American Heart Association, an individual will submit the “Application for Exam & Licensure-Massage Therapist” at least 45 days before the next scheduled Massage Therapist Licensing Exam. These exams are given four times a year on each island in March, June, October and December. All training will take place at my home in north Kihei at 886 Mahealani Street just off Pi’ilani highway and Kulanihakoi road. My home has a dedicated lanai space for massage which is surrounded by beautiful native plants and is fenced for privacy.

My massage establishment license (MAE 3350 listed under my business, San Corporation) is current until 6/30/20. I will be renewing it along with my massage license (MAT 1617) at that time. I have been licensed to practice massage here in Hawaii since 1985. I bring over 40 years of experience in teaching and practicing massage as well as training and practice as a licensed counselor in California. I started and successfully ran two massage schools: one in Monterey California, the Monterey Institute of Touch, and the other: Maui Academy of healing Arts from 1988 until I sold it in March of 2017.

Each applicant will be considered on a individual basis and will be required to provide written documentation of all previous training at the time of our initial meeting. Information regarding fees charged and the benefits of doing apprenticeship training will be disclosed at that time.

Hawaii Massage Schools - A list of massage schools in Hawaii ​ A massage therapist applying for licensure in Hawaii must first complete the above listed application for a massage license. You must have completed a state certified course that consists of no less than 570 hours of training.

Hawaii offers another level of massage accreditation called "Massage Apprentice." This is not a massage license, but rather a permit to practice massage therapy. There are three main formats for the questions within the exam, Direct Questions, Incomplete Sentences, and All of the Following Except questions. All questions are multiple-choice, with four possible answers. The program takes about 1 year to complete. In accordance with Subsection 58-47b-302(2)(e)(ii), an apprentice supervisor shall: (1) not begin an apprenticeship program until: (a) the apprentice is licensed; and (b) the supervisor is approved by the Division; (2) not begin a new apprenticeship program until: (a) the apprentice being supervised passes the FSMTB MBLEx and becomes licensed as a massage therapist, unless otherwise approved by the Division in collaboration with the Board; and (b) the supervisor complies with subsection (1); (3) if an apprentice being supervised fails the FSMTB MBLEx three times: (a) together with the apprentice being supervised, meet with the Board at the next appropriate Board meeting; (b) explain to the Board why the apprentice is not able to pass the examination; (c) provide to the Board a plan of study in the appropriate subject matter to assist the apprentice in passing the examination; and (d) upon successful completion of the review as provided in Subsection (3)(c), the apprentice shall again be eligible to take the FSMTB MBLEx; (4) supervise not more than two apprentices at one time, unless otherwise approved by the Division in collaboration with the Board; (5) train the massage apprentice in the areas of: (a) anatomy, physiology and kinesiology - 125 hours; (b) pathology - 40 hours; (c) massage theory - 50 hours; (d) massage techniques including the five basic Swedish massage strokes - 120 hours; (e) massage client service - 300 hours; (f) hands on instruction - 310 hours; (g) professional standards, ethics and business practices - 40 hours; and (h) sanitation and universal precautions including CPR and first aid - 15 hours; (6) submit a curriculum content outline with the apprentice application, including a list of the resource materials to be used; (7) display a conspicuous sign near the workstation of the apprentice stating "Apprentice in Training"; (8) keep a daily record which shall include: (a) the number of hours of instruction and training completed; (b) the number of hours of client services performed; and (c) the number of hours of training completed; (9) make available to the Division upon request, the apprentice's training records; (10) verify the completion of the apprenticeship program on forms available from the Division; (11) notify the Division within ten working days if the apprenticeship program is terminated; (12) must not have been disciplined for any unprofessional or unlawful conduct within five years of the start of any apprenticeship program; and (13) ensure that the massage client services required in Subsection (5)(d) only be performed on the public; all other hands on instruction or practice must be performed by the apprentice on an apprentice or supervisor.

R156-47b-302d. Good Moral Character - Disqualifying Convictions. (1) When reviewing an application to determine the good moral character of an applicant as set forth in Subsection 58-47b302(2)(c) and whether the applicant has been involved in unprofessional conduct as set forth in Subsections 58-1-501(2)(c), the Division and the Board shall consider the applicant's criminal record as follows: (a) a criminal conviction for a sex offense as defined in Title 76, Chapter 5, Part 4 and Chapter 5a, and Title 76, Chapter 10, Parts 12 and 13, may disqualify an applicant from becoming licensed; or (b) a criminal conviction for the following crimes may disqualify an applicant for becoming licensed: (i) crimes against a person as defined in Title 76, Chapter 5, Parts 1, 2 and 3; (ii) crimes against property as defined in Title 76, Chapter 6, Parts 1 through 6; (iii) any offense involving controlled dangerous substances; or (iv) conspiracy to commit or any attempt to commit any of the above offenses. (2) An applicant who has a criminal conviction for a felony crime of violence may be considered ineligible for licensure for a period of seven years from the termination of parole, probation, judicial proceeding or date of incident, whichever is later. (3) An applicant who has a criminal conviction for a felony involving a controlled substance may be considered ineligible for licensure for a period of five years from the termination of parole, probation, judicial proceeding or date of incident, whichever is later. (4) An applicant who has a criminal conviction for any misdemeanor crime of violence or the use of a controlled substance may be considered ineligible for licensure for a period of three years from the termination of parole, probation, judicial proceeding or date of incident, whichever is later. (5) Each application for licensure or renewal of licensure shall be considered in accordance with the requirements of Section R156-1-302.

Out-of-state massage therapists can be licensed on the basis of 500 hour educational programs if they have practiced at the professional level for at least 2,000 hours. Washington State has approved examinations offered by both the National Certification Board for ​ ​ Therapeutic Massage and Bodywork (NCBTMB) and the Federation of State Massage Therapy Boards ​ (FSMTB). In order to become a licensed massage therapist in the State of Washington, you must:

● Complete an online Washington state jurisprudence exam ● Supply your social security number ● Provide evidence of at least for hours of AIDS education and training ● Submit a photocopy of your current Red Cross First Aid card and American Heart Association CPR (cardiopulmonary resuscitation) card ● Provide evidence of a passing score on either the MBLEx or NCTBMB exams ● Complete a 500-hour massage therapist training course including: ● 1. 130 hours of anatomy, physiology, and kinesiology including palpation, range of motion,

and physics of joint function; 2. 50 hours of pathology including indications and contraindications consistent with the particular area of practice; 3. 265 hours of theory and practice of massage to include techniques, remedial movements, body mechanics of the practitioner, and the impact of techniques on pathologies; and 4. 55 hours of clinical/business practices at a minimum to include hygiene, record keeping, medical terminology, professional ethics, business management, human behavior, client interaction, and state and local laws.

After you complete your 500-hour training program, you then can fill out an application packet to become a therapist. You must mail in the application packet to the Washington Board of Massage Credentialing, along with a cashier’s check or money order for $106. The form can be found here. ​ In addition, original transcripts from the massage therapy school where training was completed must be sent directly to the Washington Board of Massage Credentialing. Following licensure, note that all massage therapists must complete at least 24 hours of continuing education every two years. Failure to complete those hours may be grounds for revocation of a massage therapist license.
