Transforming the Way We Care through Technology Hillside is leveraging the power of technology to better serve our youth, families, and strategic partners and keep our staff, supporters, and communities better informed. Technological advances are bringing our 2,151 employees in 40 locations across Western and Central and in Prince George’s County, Maryland closer together. Inside these pages, you will see how state-of-the-art cloud computing, mobile technology, and video conferencing are conquering geographical challenges and expanding our horizons of care.

The plan was motivated, in part, by the paradigm shift occurring in healthcare today. As federal and state governments struggle to balance their budgets, they are looking for ways to reign in rapidly increasing healthcare expenses. In New York State, for example, Medicaid accounted for more than $50 billion last year – the single largest expenditure in the state budget. Governor Andrew Cuomo’s administration is paving the way to implement managed care as a means to control Medicaid expenses. A managed care environment has profound implications for all healthcare providers, including Hillside. New York State will determine the type of treatment required for youth with a particular diagnosis and set the reimbursement rate for that treatment. The burden will be ours to provide effective treatment within that reimbursement budget. Managed care will require robust information systems that allow us to gather, analyze, and report data quickly and easily. We will need to monitor progress and determine when to move youth from one service to another in order to achieve treatment goals most efficiently. At the same time, we will need systems to handle the state’s increasingly complex billing and documentation requirements. In anticipation of these changes, Hillside has implemented a powerful information system to handle the requirements. We are well on our way to full implementation of an Electronic

Management Message Management Medical Record (EMR) system that will centralize data and make it available online to all staff who need it. Instead of moving boxes of paper files when a youth moves from one program to another, the records will be available instantly via computer to his/her new treatment team. Staff will have easy access to data that will facilitate early intervention and improve case management and planning. We will have tools to analyze data in a variety of ways to inform our research efforts. While many organizations leave historical information in their legacy systems when they implement the EMR, we elected to convert all the historical data from all our affiliate systems into the EMR. This wealth of data will help us make the most informed decisions as we move into the new managed care environment. And thanks to mobile technology, staff throughout our geographic service area will have access to it. Although preparing for managed care is a compelling driver for technology, our highest priority is caring for youth and families. We are pioneering the use of technology to transform the way we deliver services, communicate with families, and educate youth. Technology is helping us to reconnect families and keep youth in residential treatment in closer touch with family members through video conference comfort calls and virtual treatment conferences. It is also helping to engage our board members more fully and keep them involved when they travel. As we move forward in this brave new world, we are ever mindful of the fact that technology is only as good as the people who use it. We are fortunate to have talented, dedicated staff who are passionately committed to the youth and families we serve. Empowered with the robust technology tools we now provide, they are transforming the way we provide care at Hillside.

l Crestwood Children’s Center (CCC) supports children ages birth to 21 and their families with an array of special education, child welfare, mental health, behavioral/health, and family development services. Crestwood Children’s Foundation (CCF) raises funds to support the programs of Crestwood Children’s Center.

u Hillside Children’s Center (HCC) is a provider of care for youth and families with a wide range of emotional, behavioral, or life-circumstance challenges. HCC offers mental health, child welfare, juvenile justice, adoption, special education, developmental disabilities, health, and safety net services to youth and families. Its services are customized based on the strengths and needs of individual youth, families, and communities. Hillside Children’s Foundation (HCF) raises funds to support all of the service affiliates of Hillside Family of Agencies.

p Hillside Work-Scholarship Connection (HW-SC) is a model proven to help youth graduate from high school and become college and career ready. The program serves youth at risk of dropping out of school in Rochester, NY, Syracuse, NY, Buffalo, NY, and Prince George’s County, MD.

The rolling hills and rural farm setting of Snell Farm Children’s Center (SFCC) provide a wonderful environment for youth. Thanks to technology, we’re also able to provide top-quality education and healthcare to youth, despite the rural setting. Our Distance Learning Program or virtual classroom, allows youth to participate via computer in classes offered at any Hillside school. “We can provide classes such as Spanish and math that otherwise would not be possible,” says Executive Director Karen Sylvester. “Families want the best educational opportunities for their kids. Hillside is on the leading edge.” Hillside’s TeleHealth initiative also helps Senior Psychiatrist Stuart Loeb, M.D., and his colleagues to provide better care for youth and families in rural areas via high-definition video conferencing. Between his weekly visits to Snell Farm Children’s Center, Stuart can meet with youth using TeleHealth. The system also allows him to conference with families at their convenience or to consult with parent advocates or psychologists. “It allows us to get more opinions and provide better service,” Stuart says, “We can also accommodate families and keep them involved in their children’s care. We can see facial expressions and body posture to get a more complete picture of how a youth is doing. It’s more personal than a phone call so we can be more supportive. And the kids love it because they love technology!”

7 Monroe Region

Hillside Adoption Services uses technology to generate more matches between adoptive parents and waiting youth. Video Adoption Chronicles* allow youth to tell their stories in their own words—their likes and dislikes, hopes, and dreams. According to Lisa Maynard, Manager of Adoption Services, “Something as simple as a child talking about Batman on the video might resonate with someone who loved comics as a kid. So many of the kids say, ‘I just want somebody to love me,’ and families react very strongly to that.” Family Chronicles also help prospective adoptive parents present themselves to pregnant women who are creating adoption plans. “When we meet with birthmothers we show them the videos,” Lisa says. “The videos present the families in such a beautiful way. Birthmothers can make a better connection with them and get a sense of the kind of life their baby would have with each family.” A new Adoption Services mini-website provides access to the Chronicles, as well as complete information about all the services we offer for adoptive parents and birthmothers. “The site includes lots of resources, plus all the forms and the fees,” Lisa says. “Many agencies don’t put their fees online and you have to call. We’re completely transparent and we make it easy for people. And it’s working. We’re finding families for kids.”

9 Central Region

Many of the youth in our care have few or no family members on whom they can depend. Without these essential connections, youth have little chance for healthy development or living productive lives after they are discharged from our care. The Family Finding model for reconnecting youth with family members is helping us to build better futures with youth and families. Our staff use online search tools to find long lost family members and begin the process of re-establishing broken relationships. Youth who complete Family Finding have a network of caring adults who have made a long-term commitment to provide support. Participating in the process is a life changing experience for these youth. “I finally know what happened to me,” one youth says. “I thought my mother had forgotten all about me, but she told me she thought about me every day we were apart, and she cried and lit a candle for me every year on my birthday.” Family Finding has also yielded unanticipated benefits to quality of care at Hillside. According to one program director, “Our residential program has been trying to reduce physical restraints, so we started looking at the data on the kids who were getting restrained. We looked at age, gender, race, diagnosis, and everything else we could think of. But the pattern that finally emerged is that the kids who were getting restrained the most were the kids who were most disconnected from their families and had lost hope of ever going home again. So we started Family Finding…and we’re starting to see some results as these kids get reconnected and start allowing themselves to hope again in their future.“

Hillside Work-Scholarship Connection (HW-SC) students in Prince George’s County have learned that the expression, “a picture is worth a thousand words,” is true. HW-SC staff have been using digital cameras to capture and share “epiphany” moments – awards, academic breakthroughs, graduation ceremonies, and other student achievements. The photos have provided so much motivation to students, that HW-SC has started to give students single-use cameras they can use to capture their own achievements. “This has been very meaningful for our students,” says Executive Director Karene Brodie. “It reinforces the idea that, if they work hard, they can accomplish their goals.” The availability of laptop computers is also helping students stay on track to graduate and go on to college. The laptops allow students to research prospective colleges. Staff also use them to monitor student progress. “If a student has missed enrichments for a couple of weeks, we can see it quickly and find out what the problem is,” Karene says. “We can also identify academic problems quickly and intervene.”
