How to Register

New Patient Questionnaire

Complete the enclosed New Patient Registration Form as far as the ‘For Admin Use Only’ section.

Mobile Phone Number Consent

If you are happy for us to contact you via text message, please complete the attached Mobile Phone Number Consent Form.

Electronic Sharing of Records

The NHS is now using an electronic record called the Summary Care Record (SCR), which is being used to support patient care. Your SCR contains important information which includes your name, address, date of birth and your NHS number to help identify you correctly. It also contains information about any medicines you are taking, any allergies you suffer from and bad reactions to medicines that you have previously experienced.

Allowing authorised healthcare providers to have access to this information will improve decision making by doctors/healthcare professionals and prevent mistakes being made when patients are being cared for in an emergency or when their GP Practice is closed. Information can only be viewed by professionals directly involved in your care, e.g. hospitals. Staff will always ask for your permission, before accessing this information, every time they need to use it.

You can, however, choose to opt out of having your details on the SCR. If you do not want this information shared, please contact reception for an opt out form.

Your health record

Your health and care information is used to improve your individual care. It is also used to help us research new treatments, decide where to put GP clinics and plan for the number of doctors and nurses in your local hospital. Wherever possible we try to use data that does not identify you, but sometimes it is necessary to use your confidential patient information.

You can choose to opt out of sharing your confidential patient information for research and planning. There may still be times when your confidential patient information is used: for example, during an epidemic where there might be a risk to you or to other people’s health. You can also still consent to take part in a specific research project.

Check List

When you have completed everything, please bring the following into reception:-

 New Patient Questionnaire  Mobile Phone Number Consent  Patient Charter – signed on the back  Two forms of ID – one being photographic (e.g. passport, driving licence, etc) and one being proof of your address (e.g. a utility bill or statement).  If you have a repeat prescription slip from your previous GP, please let us have a copy.

When you bring your forms in to register, you will need to book your New Patient Health Check.

Please could you avoid bringing your details into Reception to register either on a Monday or before 11am. Thank you.

Your Medical Records

When we register you onto our computer system, an automatic request is sent to the Health Authority, for your records. The Health Authority will then request your records from your previous GP and will be sent to us via NHS . This can sometimes take up to 8 weeks. We do use electronic transfer of records and, if your previous Practice also uses this system, the electronic records will be transferred from your previous GP immediately.
