NAGA 64 th National & 23 rd Senior Open Golf Championships Tournament Information

GOLF COURSE: The Brickyard Crossing at the Indianapolis Motor Speedway, in the heart of downtown Indianapolis, is a very exceptional and unique golf experience. Designed by world renowned course designer, PETE DYEDYE,DYE The Brickyard Crossing was voted one of America’s Top 100 Public Golf Courses by and . It is a beautifully designed with four holes inside the Indianapolis Motor Speedway racetrack. Since 1929, this golf course has been host to many PGA Tour, PGA Champions Tour, and LPGA Tour events. TOURNAMENT: The tournament will be a 54-hole event for the invited qualifying players. In the event of a tie for the Championship (Men/Ladies/Seniors) there will be a sudden death beginning on hole #1. All other ties will use a card-off process in accordance with the USGA guidelines. The Overall Gross and Net Champion will come from the Championship . All players have the option to play from the Championship Tees including Ladies and Seniors for all 3 days.

Note to all participants: If you wish to compete for the overall NAGA champiochampionshipnship you must play all three days, score 54 holes, from the Championship Tees. Seniors (50-59), Super Seniors (60-69), and Super-Super-Seniors (70+) will play from the signified Senior Tees. The Senior Championship will be a 36-hole tournament with Tuesday being a practice round and Wednesday and Thursday being the scoring rounds. Please signify on the Entry Form if you will be playing in the Senior Championship Only . To clarify: If a Senior, Super-Senior, or Super-Super-Senior wishes to play for the overall Tournament Title they must play from the Championship Tees, and score 54 holes.

ELIGIBILITY: All NAGA members and Associate members. The field will be limited to 120 golfers , with consideration given first to life members, and then those members who are current with their 2012 NAGA dues. Current NAGA membership is required for participation in the tournament. If necessary, please include your 2012 NAGA dues with your official tournament application, and check the appropriate box. ASSOCIATE DIVISION: There will be an Associate Division for the third time for all non-amputees. To be eligible the participant must be a NAGA Associate Member in good standing. Awards will be given to the top three participants of the overall Championship Division based on a NET score with a verified . There will also be a Senior Associate Division Champion, with one award being given out. Amputees are first in line for the 120 slots. Associate members’ entry forms will be date stamped and they will be notified via e-mail that their entry form has been received. If there is space available after the deadline date , associates will be added according to the date received. First in wins the slot. Associates will be notified via e-mail that they are in the tournament.

REFUND POLICY ::: No full refund of fees will be made after June 28 th , 2012. Should a cancellation of entry and request for refund become necessary, a written request including the reason for the request should be e-mailed to [email protected] or mailed to the NAGA office at: 11 Walnut Hill Road, Amherst, NH 03031 for consideration.

PRACTICE ROUNDS: Practice rounds are available Saturday, August 4th , 2012, Sunday, August 5th , and/or Monday, August 6th on a first-come-first-served basis. The cost is $60/round, cart included. We will have a Sponsor Scramble on Monday, shotgun start at 12:00 (Noon) individual practice times may be limited to the morning only. MONDAY, SPONSOR SCRAMBLE: There will be a Monday, August 6th , Sponsor Scramble with a shotgun start at 12:00 (noon). Lunch will be provided for each participant (11:00am) as well as a small awards ceremony directly following completion of play. Teams will consist of , with 1-3 amputees on one team as well as sponsors. There is no cost to the amputee to play in this event, but we do ask that you mark accordingly on your entry application if you are interested in participating. There will be a limit to the amount of amputees playing in this event and it will be determined on a first- come-first-served basis. Guests will be welcome to play at a fee of $75.00, if there are spots available.

BAG STORAGE: There will be bag storage at The Brickyard Crossing for security and tournament convenience. Please help our golf professionals by using the bag storage and generously compensating them for their services.

GOLF HANDICAP: A NET Champion will be determined at the National Championship in 2012. There will be only one award and the winner may come from any Division. Only those who have a current valid golf handicap either from the USGA/RCGA or a locally recognized organization will be considered and will be verified prior to the competition. DRESS CODE ::: “Our dress code is designed to maintain an atmosphere consistent with traditions of golf.” The Brickyard Crossing is a soft spike facility. Men are required to wear golf shirts with collars and sleeves, and either slacks or shorts of appropriate length. Women can wear slacks, shorts, culottes, and golf skirts of the appropriate length with golf shirts (sleeveless tops are permitted).

Check and Credit Card Payments As a service to our members, payment of dues and entry fees for tournaments is available by personal check and credit card. The average charge to NAGA for credit card charges is 6%. In recent years, returned checks and denial of payment on credit card charges have become more pronounced and much time is being expended to resolve the issue.

Beginning January 1, 2003, the following policy will apply to any payment to NAGA for dues, entry fees or miscellaneous charges for returned checks or denial of payment on credit card remittances: A letter or e-mail will be sent to notify the player that the check or credit card charge has been denied. The notification will specify that the amount due must be paid by a certified check or money order (US funds) to satisfy the obligation. If a payment is rejected for a tournament entry fee, the entry will be denied until the obligation is paid. The member will be placed in “non-member status” until all obligations are satisfied, including a fee of $25.00 US added to all applicable charges of rejected check/credit card. Should the NAGA member fail to rectify within thirty (30) days of notification, a new application for membership and the appropriate fees will have to be made for reinstatement. Denial of Entry means that the entry to the event is not accepted. In the case where entries are limited by player spot availability, it could result in not participating in the event.
