STEP I GENERAL REQUIREMENTS FOR APPLICATION

1. Application form for College of Medicine duly accomplished; 2. Original Transcript of Records duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence with red ribbon (2 original and 2 xerox copies); 3. Diploma or Certificate of Graduation (1 original and 2 xerox copies); 4. Personal History Statement Form (PHS, Revised 1998) (2 original copies-downloadable from our website); 5. A Notarized Affidavit of Support and Proof of Adequate Financial Support, i.e., bank statement to cover expenses for student’s accommodation and subsistence, as well as school dues and other incidental expense, (1 original and 2 xerox copies); 6. Birth Certificate (1 original and 2 xerox copies); 7. Eight (8) pieces 2”x 2” photos (colored white background) 8. Police Clearance duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence with red ribbon (1 original and 2 xerox copies); 9. Recommendation Letter from 2 former professors (original copy); 10. National Medical Admission Test (NMAT) Result (original and 2 xerox copies); VALID NMAT: March & December 2012; April & November 2013 11. Passport Pages where name, photo, birth date and birth place appeared (2 xerox copies); 12. Application and Processing Fees:

USD 300.00 (subject to increase) = NON-REFUNDABLE/NON- TRANSFERABLE Payment shall be made in a form of Manager’s Check addressed to: UERMMMCI-College of Medicine

MINIMUM REQUIREMENTS:

a. Applicant must be a holder of baccalaureate degree; b. Interview upon submission of required documents; and c. Applicant must have taken the National Medical Admission Test (NMAT) within the validity period and must meet the prescribed NMAT percentile cut off score.

DEADLINE FOR SUBMISSION OF APPLICATION IS EXTENDED UP TO JANUARY 24, 2014.

NOTE: APPLICANT APPLYING FOR CITIZENSHIP RETENTION AND RE-ACQUISITION (DUAL CITIZENSHIP under R. A. 9225) MUST SUBMIT THE FOLLOWING DOCUMENTS:

STEP III ADMISSION PROCEDURE

1. Applicant shall submit the accomplished application form together with the documents required to the Office of the Registrar for processing; 2. The Committee on Admissions based on the transcript of records, and other documents submitted by the applicant, recommends acceptance to the Dean, College of Medicine for issuance of NOTICE OF ACCEPTANCE (NOA); 3. NOTICE OF ACCEPTANCE (NOA) will be sent to each respective accepted applicant through email.

STEP IV ENROLLMENT PROCEDURE

1. Proceed to the Registrar’s Office and submit the other lacking documents (if there is any); 2. Secure and fill-out the Information and Advising Form (2 copies). Have it signed by the Registrar; 3. Proceed to Information Technology Department (2nd Floor, Admin. Bldg.), submit one copy of the accomplished Information and Advising Form to IT Department authorized personnel for assessment and wait for printed registration form for payment to the Cashier;

MATRICULATION FEES:

USD10, 000.00 – Foreign Miscellaneous Fee - PAYABLE UPON ENROLLMENT PhP 122, 304.00 – Regular Tuition and Other Fees (Per Semester) - SUBJECT TO INCREASE

4. Pay the Matriculation and ID Fees at the Cashier located at the 2nd Floor, Administration Building; 5. Proceed to IT Dept. (2nd Floor, Admin. Bldg) picture taking room; present O. R. ID Fee and have picture taken and wait for its issuance. 6. Proceed to the Infirmary Section (2nd Floor near Telecom Dept.) of the Hospital for schedule of Physical Examination.

STEP V STUDENT VISA PROCEDURE (FOR FOREIGN)

1. Upon enrollment, proceed to the Office of the Registrar; 2. Secure and fill up the following:

 Bureau of Immigration Basic Information Sheet  General Application Form (BI FORM: MCL-07-01)  Letter Request for Student Visa Conversion

3. Proceed to the Bureau of Quarantine for medical examination; 4. Upon release of the medical examination result, proceed to the Bureau of Immigration and submit the Registrar’s endorsement together with the above documents for application for Student Visa Conversion and 5. Approved Student Visa will be notified by the Liaison Officer of the Registrar’s Office.

NOTE: THE SCHOOL HAS NOT AUTHORIZED ANY PERSON/AGENCY OR ORGANIZATION TO WORK ON BEHALF OF ANY APPLICANT FOR ADMISSION.

THE ADMISSIONS COMMITTEE WILL NOT PROCESS ANY APPLICATION SUBMITTED OR FOLLOWED UP BY THIRD PERSON.
