1.0 Purpose

The purpose of the FLHA process is to protect people, equipment, property, the environment and the reputation of Japan Canada Oil Sands Limited (JACOS) by ensuring work is executed safely. The Field Level Hazard Assessment (FLHA) process enables workers to assess their workplace for and take steps to reduce risks to acceptable levels. The process is fully documented to demonstrate due diligence. The FLHA identifies any health, safety and environmental hazards present at the specific time and location where the work is to be performed that may not have been identified and controlled by the planning process for that activity. Where such additional hazards are identified, suitable controls must be identified, recorded and implemented by the work group prior to work commencing. The FLHA must be reassessed during the work activity if there are any changes in conditions that could have introduced new hazards to the work. This standard is one of the elements utilized to comply with the Alberta Occupational, Health and Safety Code Part 2 – Hazard Assessment, Elimination and Control. (http://www.hre.gov.ab.ca) 2.0 Scope

This procedure applies to all JACOS operations, sites and offices. It applies to the management of HSE hazards that arise in the course of activities at these sites and offices. Contractors may utilize their own equivalent FLHA process as long as their process meets or exceeds this standard.

3.0 Application

Formal documented FLHAs will be conducted at the place of work for:

• All work executed under the Safe Work Permit system • All tasks and activities identified by the respective work areas as per Appendix B A copy of the FLHA must be readily accessible in all work areas.

4.0 Responsibilities

4.1 All Employees and Contractors

• Identify, correct and document hazards in the workplace using the FLHA • Perform FLHAs at the job site before work begins • Participate in conducting FLHAs • Ensure a copy of the FLHA is readily accessible at the job site • Communicate hazards, controls and residual risks to employees joining the work after the FLHA has been initiated and confirm via signature • Stop and re-initiate the FLHA process when conditions in the work area change and document the time of the review • Complete the final sections of the FLHA form at the conclusion of work • Submit completed FLHA form, together with completed work packages, to the supervisor at the end of each shift

4.2 Frontline Supervisors

• Ensure personnel are aware of and utilize the Field Level Hazard Assessment process • Ensure appropriate personnel assist in identifying hazards and developing plans to mitigate risks to acceptable levels • Ensure FLHAs are completed and signed prior to work starting • Ensure that appropriate Standard Operating Procedures, (SOPs) safe work practices (SWPs) and job hazard analyses (JHAs) are referenced in the FLHA where applicable • Identify methods of control for each hazard identified:  Follow the hierarchy of controls: elimination of the hazard, substitution, engineering control, administrative control (e.g., signage or procedure)  Reinforce that the last line of defense (i.e., least effective) is personal protective equipment (PPE) • Continuously review completed FLHA with employees as work progresses • Ensure FLHAs are archived for a minimum of 30 days and in the case of an incident included with the associated incident records within the Incident Management system • Review and monitor quality and compliance against this standard

4.3 Line Management

• Ensure appropriate risk/hazard assessments such as critical task analysis, task hazard assessment and are conducted at the job site prior to work commencing to support the FLHA process • Ensure that corrective actions or hazard reductions identified in the hazard assessment process are implemented in their areas of responsibility • Ensure appropriate controls are in place to mitigate risks to acceptable levels • Monitor and audit to assess the quality of, and compliance with, the FLHA process and to identify opportunities for continual improvement

4.4 Health, Safety and Environmental Department

• Review/audit all hazard assessments regularly for accuracy and relevance to the work being performed • Assist with hazard assessments where required

4.5 Corporate and Senior Management

• Support the initiatives in their area of responsibility • Ensure that employees and contractors under their supervision complete hazard assessments as required in the JACOS HSE Management System

5.0 Field Level Hazard Assessment Process

The FLHA is performed by using a structured process called S.T.A.R.T Right™ which is composed of 5 steps:

• Stop - Step back and observe • Think - Through the task • Assess - Hazards, pathway, impact • Review - control and document findings • Talk - it through and complete task This section outlines the expectations of the S.T.A.R.T. process.

STEP 1: Stop! Step Back and Observe

Ask: Do I understand the scope of work? Ensure that each employee clearly understands the scope of work they are involved in and therefore does not engage in any activity that is not in that scope and consequently incur any additional risk through unplanned work.

STEP 2: Think through the tasks

Ask: How am I going to do this task? This means considering further questions like: Do I have the proper training, tools and equipment to perform the task? Ask: Are there any uncontrolled hazards? The purpose of this step is to think about the task you are conducting and what hazards are inherent in the work. In assessing the hazards, workers must consider the following hazard zones: 1. Near Ground a. Hazards directly in the immediate work area i. Buried hazards ii. Overhead hazards iii. Airborne hazards iv. Contents of process piping systems v. Energy sources vi. Other scopes of work affecting immediate area

2. Mid Ground a. Hazards that are adjacent to the work area b. Work activities that have the potential to impact the immediate work area 3. Far Ground a. Hazards that have the potential to enter the work area (e.g., other workers, crews or conditions) b. Hazards that you may cause as a result of your work activity

STEP 3: Assess Hazards, Pathway, Impact

In assessing the risks the focus is on the consequence and the likelihood of the risk materializing. Ask:

• Hazards - Have I assessed what can go wrong? • Pathway - Have I assessed how this can occur? • Impact - Have I assessed what is the most likely result of the hazard? Based on the outcome of the assessment the crew will decide on measures to mitigate the identified risks. Identified risks beyond a worker’s control must be reported to the supervisor.

STEP 4: Review and document findings

Ask: Have I documented all the hazards and associated controls? After a hazard is identified, workers must consider methods to eliminate or mitigate the hazard. , if possible, is the preferred method of control. It is best achieved by removing the hazard from the workplace. Examples of eliminating hazards:

• Physically removing an overhanging ice build up • Restricting access for mobile equipment to work area. • Sanding slippery walk ways • Removing tripping hazards – good housekeeping If a hazard cannot be eliminated from the work area, workers must initiate control measures to manage risk to a level as low as is reasonably achievable through the use of measures identified in the hierarchy of controls. Control of hazards starts at the top and works down, with PPE being the last line of defence. A combination of , and personal protective equipment should be used if there is a greater level of safety because of the combination used

STEP 5: Talk it through and complete task

Ask: Have I discussed the hazard impacts and associated control measures with all persons impacted by this task? Work can ONLY commence when:

• The FLHA is completed • The risk mitigation measures are implemented • All members of the crew are aware of the risks and controls Note: Workers are not authorized to conduct work tasks that cannot be performed safely. If the work task cannot be performed safely, workers must immediately contact their supervisor. For all tasks that require isolation, no work will commence until field verification of the isolation has been completed. 1. Site designate will confirm the initial isolation and zero energy in the field and sign the FLHA. 2. Select “Y” for an electrical isolation. Supply the E&I contact name as applicable. Select “N/A” if not applicable. 3. Select “Y” if a blank list, etc., has been obtained. Select “N/A” if not applicable. Include the client contact information as applicable.

4. If the isolation has been carried over to the next shift, verify the isolation and select “Y.” If not applicable select “N/A.” Supply the name of the person that handed over the isolation as applicable. 5. When all hazards have been eliminated, reduced or controlled select “Y” and sign off on the FLHA. Work can now commence. 6. If the identified hazards cannot be eliminated, reduced or controlled, no work is to begin. Contact and inform the supervisor.

6.0 Routine Tasks / Office Work

For some activities the preliminary hazard review will determine that there is no requirement for an FLHA to be completed prior to work commencing. This finding and the procedure(s) for the activity in question are then recorded in the Risk Register. Examples of activities where this will usually be the case:

• Performed frequently and routinely (i.e., daily) • Repetitive and simple • Involves predictable and/or few hazards • Performed in an environment that is not usually subject to change (e.g., office work or cleaning in an office environment) • Where the risks are well controlled by procedures, training and other controls documented in the procedure Note: Even for routine tasks/office work an employee will still be required to go through the START Right™ process section of the FLHA.

Table A-2: Definitions Term Definition Activity A task (or group of related tasks) performed as part of the works on the site or contract that involves safety or environmental hazards and has the potential to cause harm. Tasks performed by Suppliers/Subcontractors over which JACOS has influence shall also be considered (e.g. Suppliers subcontractors working under JACOS systems/processes, directly supervised by JACOS, etc.) FLHA A documented on-the-job analysis for unforeseen hazards in the work area that could affect the work, the workers or other workers, immediately prior to commencing a task. Hazard Something which if not controlled is capable of causing harm to persons, equipment or the environment. Hazards can include energy, chemicals, biological agents etc. JHA A job hazard analysis is a method to identify existing and/or potential hazards of a job. Each task undergoing a JHA will be studied and each step of the job recorded, so that the entire job process is considered. Hazards (or potential hazards) are then more easily identified, and the best way to reduce or eliminate those hazards can be determined. JHAs should be developed for non-routine, high risk or the introduction of new equipment. See Management of Change Procedure. Hierarchy of Control The descending order of effectiveness of different types of control measures that may be applied following an assessment of possible hazards or risks: • Elimination - most effective control measure, involves the removal of the risk (by removing it or changing work processes) • Substitution - replacing the material or process with another that has lower risk • Isolation & Engineering control - removing or separating people from the source of the hazard, changing the physical characteristics of plant or workplace to remove or reduce the risk • Administrative control - using policies, procedures, signs, training etc. to control risk • Personal protective equipment - equipment or clothing designed to provide protection Impact The combination of person(s) or thing(s) that may be harmed by a hazard and the nature, extent and severity of the harm that may be caused. Pathway The mechanism or way in which a hazard can cause harm.
