7 Page 1 The RUNDOWN - October 2014 Tidewater Striders Board of Directors Minutes for August 11, 2014 The meeting was called to order at 6:30 by Vice President, Steve Shapiro, at the Neptune Festival Offices. President, Rich Hildreth and Todd Humphrey were absent. The July meeting minutes were approved. Vice President’s report: The next “general meeting” of the members will be held at the Strider picnic August 16 where two amendments to the bylaws will be voted on. Treasurer’s Report: Dan presented the Profit and Loss Statement January through July, 2014 and reviewed the July P & L Detail and the July Balance Sheet. Dan also presented and explained the IRS Form 990, which is the club’s annual tax return. Committees: Banquet: Ada Lester submitted cost figures for the awards banquet and proposed a February 7th date. The banquet location, need for a D. J. and open or cash bar were discussed. After approving the date, the discussion of final arrangements was tabled pending more research. Constitution & Bylaws: Mel distributed a recap illustrating progress toward updating all committee Policies and Procedures. Six committees’ P & P are yet to be completed. Hall of Fame: Nominations due by November 1st. Marketplace: Dave distributed an inventory of Strider merchandise and discussed the need to re-order. A call for volunteers to man the booth at the Rock & Roll Expo was made. Dave plans to raffle some Strider merchandise in the booth. Membership: Bob Brunner submitted a report indicating we had 1,806 members at the end of June. Although we are once again over the 1,800 level, only 33 of 133 lapsing memberships were renewed. Sherry estimated more than 40 people joined or renewed their memberships at the Summer Series. Race: Dan gave an overview of recent and upcoming races. He commented the Strider Mile needed both participants and volunteers; the Striders need a race co-director for both the Turkey Trot and the Distance Series; the Neptune 8k is the next Grand Prix running event; the Mud Run had over 1,200 participants; a record amount of money was raised at the memorial 5k; the Seashore 50k is expected to sell out. Scholarship: Dan reported that all scholarship awards had been distributed. Volunteer: Lisa reported 259 individuals volunteering 586 times, year-to-date. Walking: Steve presented the Walking Committee report in writing. The complete report will appear in the Rundown. Youth: 9 kids are participating in the summer running camp. Striders are contributing to the camp fees. New Business: Dan proposed reprinting the club sticker, currently a blue square, in an oval format. Consensus was in favor and Dan is checking the price to reprint. Tom described the Competitor Group’s plan to provide first responders on bicycle at the Rock & Roll Half and asked for help getting volunteer riders. Since the next typical meeting day falls on a national holiday, the next board meeting will be on Tuesday, September 2, at the Neptune Festival offices. Old Business: Angelo demonstrated the new computer and our capabilities for transacting marketplace sales, enrolling new members or renewing memberships, and showing audio visual presentations. Call for Hall of Fame Nominations Tidewater Striders Hall of Fame Nomination for the Class of 2015 Nominations are open for election to the Tidewater Striders Hall of Fame, Class of 2015. Individuals may be nominated for their accomplishments in the areas of competition, volunteerism, or profession-related endeavors, or a combination of these areas. A maximum of five (5) members will be inducted for the class of 2015. Individuals nominated for membership in the Hall of Fame must have been members of the Tidewater Striders for a minimum of ten (10) years. Individuals may use a self-nomination process or be nominated by other club members, the latter having been members of the Tidewater Striders for at least five (5) years. Self-nominations will receive the same respect and consideration as member-sponsored nominations. The nomination form may be found on the club’s website, www.tidewaterstriders.com. 13 The RUNDOWN - October 2014 Healthy Running - When the Labs Come Back Abnormal! by Sam Wittenberg In last month’s “Healthy Running” I discussed running with fatigue or labored effort and touched on the role of laboratory testing to evaluate these problems. This month, I’ll expand on lab tests to review abnormal lab results in normal healthy runners. OK, you’re thinking, that’s weird. Not really. In the past few months, I had several occasions to review a variety of abnormal test results in runners who had no apparent medical problems. A couple of these people were already undergoing further evaluation for these abnormal results, and all came back confirming good health. In the interim, frustration and anxiety levels were elevated. Could this have been avoided if we know that lab results may be different in runners? As early as the 1970’s, it was observed that healthy endurance athletes can have laboratory values that might indicate myocardial ischemia (inadequate oxygen to heart muscle), liver disease, kidney disease, and anemia, and physicians were advised to inquire about a person’s exercise habits. To discuss this further, I’ll summarize information on 3 common blood tests: blood count, liver chemistries, and muscle chemistries. The complete blood count (CBC) measures the number of white blood cells (WBC), red blood cells (RBC), hemoglobin and hematocrit (amount and concentration of the blood’s oxygen carrying protein), the size and shape of red blood cells, and the number of platelets (which play a role in normal clotting). A low RBC count or low hemoglobin/hematocrit is an indicator of anemia. As previously noted, the most common cause of anemia in distance runners is iron deficiency, due to inadequate dietary iron intake, normal iron losses (menses), or abnormal iron losses due to disease. On the other hand, though, many runners have a mild anemia that is not due to iron deficiency. A normal response to endurance training is an expansion of blood volume that results in an increase in the delivery of oxygen to exercising muscles. Blood volume increases in 2 ways: an increased production of red blood cells and increased amount of plasma (fluid). When the fluid increase is proportionately greater than the increase in blood cells, the red blood cells are “diluted” and there is a mild drop in blood count as measured by concentration (hematocrit). This is termed dilutional pseudo-anemia, has no adverse effects on training or performance, and requires no treatment. According to some sources, testing for iron deficiency is undertaken if the hemoglobin level is under 12 in women or under 14 for men and when the blood count also shows a small red blood cell size (a sign of iron deficiency). (A future article will discuss iron deficiency in further detail.) The white blood cell count is typically normal in most runners. However, there are several literature reports of mildly low white blood cell counts in endurance runners. These low WBC were not associated with impaired ability to fight infection. At the other extreme, several studies reported mildly elevated WBC and platelets after marathon races. The rise in WBC and platelets is temporary and ultimately returns to normal in under a week’s time, and may be a transient inflammatory response to tissue injury in endurance running. To discuss liver chemistry tests (AST, ALT, LDH, bilirubin) and muscle chemistry tests (CPK, sometimes referred to as CK), keep the following in mind. CPK comes from both skeletal and heart muscle. LDH, AST, and ALT are found in both liver and muscle cells. LDH is also found in blood cells. (There will not be a pop quiz!) LDH and bilirubin can be briefly elevated due to a phenomenon called foot-strike hemolysis. This is also well known in military recruits as march hemolysis. In this situation, the force of repeated foot strikes results in a breakdown of red blood cells in the small capillaries of the feet. The result is an elevated LDH and, particularly in ultra events, elevated bilirubin. Though testing laboratories include these tests in what are called “liver panels”, these results are not associated with damage to the liver. Of note, in foot strike hemolysis, hemoglobin released from the blood cells can pass through the kidneys and impart a temporary pink tone to urine. The important point here is “temporary” and the urine color should clear quickly with hydration. Members of the running community are aware of elevations in CPK with endurance efforts. CPK is a traditional marker for heart muscle damage of a heart attack. Several studies of runners, including marathon runners, have demonstrated normal cardiac testing in these participants and no evidence of myocardial ischemia (compromised blood and oxygen flow to heart muscle). A study of runners of the 2002 London Marathon observed this CPK elevation and concluded, “a full history is essential before interpreting any abnormality in these indices, in order to avoid the marathon runner being mistakenly confined to a hospital bed for spurious reasons.” (A future article will discuss the current reports related to running and possible cardiac injury.) The rise in CPK is known to come from the effects of endurance running on skeletal muscle. Some articles in the medical literature call this “tissue damage” or “transient muscle injury” though some use the explicit term “exertional rhabdomyolysis”. Dr. Timothy Noakes, physiologist and author of The Lore of Running, found that the rise in CPK is in proportion to the intensity and duration of the preceding exercise. The 2002 London Marathon study observed that the rise in CPK was higher in less trained finishers despite their slower times. There is, though, uncertainty about what is a “normal” CPK level in the exercising population. Reports from the military, where formal efforts are in place to recognize and promptly treat “rhabdo”, note that the treating physician must exercise caution in evaluating the recruit (in our case, the runner) with an elevated CPK who has NO symptoms. Some in military medicine suggest that the CPK elevation should be greater than 5 times the normal value before considering rhabdomyolysis, and even this suggestion is debated. Research in this field is ongoing. For our purposes, the important point is that knowledge of a runner’s training, mileage, and intensity is essential when evaluating CPK elevation. (I’ll review exertional rhabdomyolysis as well in a future article.) Of the medical reports on marathon runners, a study of finishers of the 2001 is very informative. Blood samples were drawn from 37 participants who were attending the pre-marathon science symposium of the American Medical Athletic Association. These racers ran an average of 25 training miles per week, completed 5 previous marathons, and ran a qualifying race of under 4 hours in the preceding year. Samples were taken from these runners the day before the marathon and then again at 4 hours and, for 11 of the 14 The RUNDOWN - October 2014 runners, 24 hours after the race. The authors reported a statistically significant change in multiple laboratory parameters at the 4-hour and 24-hour sampling points. Of interest to this discussion are the results for AST, ALT, bilirubin, and CPK (the liver and muscle related tests). All remained significantly elevated at the 24-hour point and were outside of the standard lab reference ranges for “normal” in 30% of these runners. ALT was elevated over the pre-race level but was not outside normal laboratory reference values. The authors comment that CPK and AST elevations indicate “exertional rhabdomyolysis and leakage from skeletal muscle” while ALT, which is a more specific for liver injury, showed little change. These results are very similar to a 1999 report of a 1600K ultramarathon in Australia – that’s 1600K, no typo! The authors of the Boston Marathon note that the purpose of normal laboratory reference ranges is to compare a patient’s results to similar control subjects. Unfortunately, that is not the case when evaluating lab results in runners. Reference ranges in most laboratories are statistically determined from the results on various volunteer populations that do not specifically include endurance athletes and runners. The authors of the Boston Marathon study proposed a table of normal results for marathon runners, and they encouraged further study on a larger sample of marathon participants. So what should a runner do when faced with abnormal lab result? If the labs were obtained to evaluate specific symptoms, the focus should first and foremost remain on the symptoms. Lab results related to specific symptoms should not be attributed to the effects of running until further tests demonstrate good health. On the other hand, what if the labs were drawn “as routine”? In this situation, the runner has no symptoms, and the assumption is that the labs will assist in the early detection of problems, referred to as screening. For the labs discussed here, there is no evidence that they are valid screening tests. In this instance, abnormal labs can be understood by reviewing running mileage, intensity, training, and recent race participation. This information will guide the physician in deciding if your labs are abnormal but you are fine. At times, repeat labs after a brief period of reduced running will demonstrate a return to normal values. This typically does not require a complete cessation of running to clarify this. If the labs remain elevated but come down to values closer to reference ranges, they are very likely “your normal”. To repeat these periodically, for example every 6 months, for reassurance should be decided on a case-by-case basis. If you are having lab work done, it’s helpful to discuss what the tests will be looking for and what the results might reasonably reveal. It’s also helpful, in advance of the results, to discuss what steps might be taken if unexpected abnormal results return. Need Volunteers for the Seashore Nature Trail 50K Planning for the 50 K is underway, and we need your help again. Our volunteers have made this 50K one of the most popular races in the area. Every year, the runners attribute their success to the enthusiasm and assistance of the volunteers along the course. If you are running the race this year, encourage some of your friends or family members to volunteer with us. We have a variety of volunteer positions available: • Course Marking (Friday, Dec. 19) • Packet pick up (Friday, Dec. 19) • Packet pick up and chip distribution (Saturday, Dec 20) • Start and Finish line set up • Road guards • Aid station workers • Finish line assistance (removing chips, distributing food, water, etc.) • Course Marshalls • Course Sweeper ( behind last runner starting at mile 25 to track last runner and gather flags and trash) • Post-race party assistance. PLAN TO JOIN the Tidewater Striders' New Energy Youth 2014 Running Team New Energy is an informal youth group training together to improve their times in road races as well as their performance in track and field and cross-country events sanctioned by the USA Track and Field (USATF) and AAU. Motivations vary from those who simply enjoy running, to those who are goal oriented and recognize the need to train in order to perform well in races. With its group approach to running practices, New Energy provides a fun, sociable forum for running preparation and development.

New Energy welcomes all youth runners, ages 6-16, regardless of ability. Practices are informal, and participants are encouraged to attend as many as possible. Runners must have a good pair of running shoes to avoid injuries. Uniforms (singlet and shorts) and Warm ups ( Hoodie and Pants) can be purchased, but are optional. Membership in the Tidewater Striders is required by the third practice in order to participate in sessions and compete on a team basis in Track and Relay teams. The cost is $12.00 per year or $18.00 for two years. The Team participates in road races held in the Tidewater area, which are part of the Youth Grand Prix 1-mile series and are sponsored by local organizations such as school PTAs. A New Energy Team race schedule will be part of the program and can be found under ‘Youth Programs’ on our Web Site.

Coaching is provided by volunteers who are Tidewater Striders members. Again this year, Angelo Celesia will provide leadership for training. Other running enthusiasts/parents will also assist.

New Energy provides youth runners and their families an enriching environment which can be a building block to the youth runners' future. In the past, New Energy runners attended National Cross Country and Track and Field Championships around the Country. Many previous New Energy runners are currently participating with high school and college teams where they have excelled in Cross Country, Indoor and Outdoor Track and Field with All State and National recognition. New Energy has produced numerous All-American medalists, State Champs and one National Champion in Cross Country.

RACE TIME: Run/Walk begins promptly at 5:00pm at the West Point Memorial

THE COURSE: The USA Track & Field certified 5K Run/Walk course begins at the West Point Memorial & continues through Elmwood Cemetery, ending at the West Point Memorial. Terrain varies & includes asphalt, grass, & gravel. A map of the course is posted online with registration information. Run/Walk is professionally timed by the Tidewater Striders Running Club (tidewaterstriders.com).

EVENT HIGHLIGHTS: Victorian era “course marshals”, live vignettes of Victorian mourning practices, period entertainment, award ceremony & after party at The Plot on Granby presenting live music, a costume contest, Victorian era film montages, a raffle with prizes including two Wicked 10K adult registrations, food, drink & fun for all ages! Complimentary shuttle service provided from Elmwood & West Point Cemeteries to The Plot on Granby every 10 minutes beginning at 6:00pm & ending at 8:30pm. Event registration fee includes a meal ticket to the food truck of your choice, unlimited non-alcoholic beverages, and one (1) free beer per participant over age 21. Age is verified upon entry. *A Walking Grand Prix Event * *A Golden Runner Race* The track will open at 7:00 am; 1st Heat starts at 8:00 am This is the 2014 Virginia Association USATF Open One Hour Race Walk Championship on the track. Non USATF members and other NonVA-USATF members are welcome to participate and are eligible for Tidewater Strider awards for open M & W 1st - 3rd places. In accordance with Rule 3 of the USATF Competition Rules, only VA-USATF members are eligible for VA- USATF open awards M & W 1st – 3rd places. In past years the one hour race walk results were submitted by mail to the Shore AC of New Jersey, where the term “Postal” originated, and combined with one hour races from other world venues. Walk On! Minnesota took over as national sponsor for two years using digital reporting of results. The Shore AC has returned as the country-wide race sponsor, they have retained the “Postal” name, and have returned to the postal method of submitting race results. Competitors may submit, or have their race results submitted to the Shore AC 2014 country-wide One Hour Postal Race Walk for a separate $5 fee. This is in addition to the $10 Tidewater Striders race entry fee used to offset the cost of using the track at Tallwood High School. Participation in the Shore AC & National Postal One Hour Race Walk is encouraged. $15 is the total fee for race entry and participation in The Shore AC National Postal Competition (one $10 check payable to Tidewater Striders; and one $5 check payable to Dave Talcott for Shore AC race registration). Lap forms will be available at the race site. THE USATF COMPETITIVE RULES OF RACEWALKING WILL BE FOLLOWED. A minimum of three (3) USATF certified racewalk judges will be assigned to judge the racewalkers in each heat. One caution (yellow paddle) may be given by a judge to walkers observed to be in danger of ceasing to comply with the rules of racewalking during their heat. Failure to (1) maintain contact with the ground at all times, and/or (2) failure to insure that the advancing leg is straightened immediately upon contact with the ground, and maintained straightened until the body passes over the leg, will result in warnings (red cards) being issued by the judges. Walkers receiving three warnings (1 red card per judge) will be disqualified and their names will not be submitted to the national sponsor: Shore AC of NJ. An air horn will be used to start each heat. A large digital race clock, located in the vicinity of the starting line and visible to all competitors, will display the time. Each walker’s lap recorder will use the digital clock, or their own digital clock/watch, to record the cumulative time of each of the walker’s laps. When five minutes remain to be walked, each walker will receive a marker (Bean Bag) to carry during the remaining time. A hand bell will be used to indicate when there is one minute remaining. The air horn will sound when the hour time has elapsed. Walkers will drop their markers at their location on the track when hearing the final air horn blast. 2014 Virginia State RRCA Ultradistance Championship Race

ENTRY FEE: $80 before November 1; $100 from November 1 to December 19. Entry fee for Tidewater Striders members who have a Golden Runner membership is $40 before November 1, and $60 from November 1 to December 19. Golden Runners must register by mail. Eighteen (18) is the minimum age to enter. LIMITED NUMBER OF PARTICIPANTS: The race field is limited to the first 300 registered runners and to the first 30 registered Tidewater Strider Golden Runners, after which the regular registration fee applies to Golden Runners. REGISTRATION: Register by mail or online. Registration by mail, complete the form on the reverse side. Online registration, go to www.raceit.com. If the race is not filled, runners may register at packet pick up Friday, December 19. NO RACE DAY REGISTRATION. The race sold out in 2013. COURSE: This 50K (31.1 miles) race is run on the trails of First Landing State Park in Virginia Beach, with less than 1/2 mile on a paved road. The two-loop course is lined with trees and follows hardpacked dirt trails through a variety of ecosystems. The main trail is mostly flat, while side trails are slightly hilly with some tree roots to negotiate in certain sections. Wooden bridges are also found on several trails. FLUIDS AND FOOD: Various fluids and snack-type foods will be provided both prior to and after the race. Two aid stations on the course, each passed multiple times, will provide fluids, sports drinks, and carbohydrate/protein snacks. TIME LIMITS: The race starts at 8:30 AM with a time limit of 8 hours for official finish. Runners must reach Aid Station 6 (22 miles) by 2 PM and Aid Station 7 (25 miles) by 2:45 PM. These cutoffs are strictly enforced. No official finish will be given if runners do not exit these Aid Stations by these times. HOST HOTEL: Virginia Beach Resort Hotel and Conference Center is the host hotel. A room rate of $89 per night is available to participants. PACKET PICK-UP: To avoid the race day rush, please pick up your race packet on Friday, December 20 at the Host Hotel between 4:00 and 8:00 PM.
