7.2 Incidents of Note

The following incidents and activities are considered noteworthy: BURNLEY TUNNEL B01 – B59 Thursday 2nd January – Tuesday 7th January Burnley tunnel resurfacing works. Tunnel closed for the duration with several detours in place. Minimal delays were caused by the closure with minor delays on the Monday peaks. The tunnel was opened well before its deadline.

B03 Wednesday 15th January 13:14 – 14:16 Three car noise to tail in the right lane at B03. SIRO and police on site. Nil injuries and nil asset damage. Right lane closed which caused some traffic delays 33(1) back to until all vehicles were moved to 33(1) the emergency lane. One driver possibly intoxicated. 33(1) One accident allocation required.

B03 Wednesday 29th January 13:28 – 14:05 Nose to tail collision in the centre lane. SIRO on site. Nil injuries and nil asset damage. Both vehicles were moved to the emergency lane and SIRO collected debris from the centre lane. One accident allocation 33(1) required. 33(1)

DOMAIN TUNNEL D28 Friday 17th January 20:11 – 20:30 A truck side swiped a car while changing lanes to the left. Both vehicles stopped at the end of the tunnel exit to exchange details. SIRO on site. Nil injuries and nil asset damage. Both vehicles were able to drive 33(1) away. 33(1)

7.2 Incidents of Note

The following incidents and activities are considered noteworthy: BURNLEY TUNNEL B28 Thursday 6th February 20:47 – 20:55 A cyclist entered the tunnel riding in the left lane. Police requested and caught up with the female and escorted her to the Burnley St exit. The cyclist advised police she entered the tunnel as she thought she was being followed.

B04 Tuesday 11th February 10:49 – 11:47 Two trucks were involved in a minor nose to tail blocking the running lane. SIRO on site and moved trucks to the emergency lane. Nil injuries and nil asset damage. Both truck were able to drive out of the tunnel. 33(1)

B58 Friday 14th February 17:30 – 20:02 A vehicle with no registration plates or sticker was found abandoned at the exit portal. SIRO and policeRoads Corporation attended as the vehicle was initially Informationtreated as Act 1982 suspicious. The occupants were later found walking away from the vehicle which had over heated. B25 Monday 24th February 03:55 – 06:15 A 5.19m high truck triggered the overheight detectors and struck the overheight barriers before proceeding into the tunnel. The truck caused damage to several signs before stopping at B25 in the left lane. The truck also clipped a couple of jet fans. SIRO, police, TSS and TSL on site. Two lanes were closed during the incident while TSL were assessing damage. The truck could only be lowered to 4.88m and was escorted out of the tunnel with TSL observing the clearance.

DOMAIN TUNNEL D26 Sunday 2n February 00:22 – 00:41 A pedestrian walked into the tunnel from the Kings 33(1) Way exit. SIRO dispatched and lane closed in the tunnel. Police were requested however the SIRO escorted male out of the tunnel who was lost, had no money or phone. Police cancelled.

D20 Wednesday 19th February 18:56 – 19:55 Minor two car collision in the right lane. Both vehicles 33(1) moved to the left lane which was closed at the time 33(1) 33(1) 33(1) before being asked by TOCO to drive out to the exit 33(1) ramp. SIRO and police on site. One driver was intoxicated and unlicensed and removed by police. Nil 33(1) injuries and nil asset damage. No tows required. 33(1)

SOUTHERN LINK Burnley Tuesday 18th February 15:31 – 16:28 A motorcyclist entering from Gibdon St lost control and collided with the NJB on the right side closer to Burnley. SIRO, police and ambulance on site. The rider sustained serious injuries and was transported to hospital. Two right lanes were closed during the incident resulting in major delays. BURNLEY TUNNEL B07 Thursday 3rd April 00:55 – 01:11 A cyclist was lost after being in the city and took the Hannah St entry into the tunnel. SIRO, police and ambulance on site. Left lane was closed for the cyclist. SIRO escorted the cyclist out of the tunnel after the cyclist was questioned by the police.

Entry Portal Monday 14th April 16:15 Operations centre was advised of a pedestrian sitting on the wall above the Burnley Tunnel entry 33(1) portal (Sturt St). Male location who appeared to be eating his lunch. Police advised and attended. The pedestrian was enjoying the view. Police moved him on.

B58 Wednesday 30th April 10:11 – 12:29 A B double truck stopped in the right lane at B58 due to a driveline failure. The truck was not able to be moved. Modern towing attended and identified the truck had a differential failed requiring four axles to be removed to allow the truck to be towed from the tunnel. A 2nd Incident response officer was sent to assist with managing traffic and works to remove the axles. 7.2 Incidents of Note

The following incidents and activities are considered noteworthy: BURNLEY TUNNEL B30 Tuesday 24th June 08:11 – 09:17 There was a two vehicle collision in the right lane when the rear vehicle ran into the back of the front 33(1) vehicle while trying to move to the centre lane. The front vehicle was forced up onto the NJB and ended up stopped in the centre lane with the rear vehicle stopping further up in the left hand lane. The SIRO responded and a tow was required to remove one of the vehicles. There were no injuries and traffic was 33(1) affected to beyond Montague Street. B59 Tuesday 24th June 22:39 – 00:04 A semi trailer was seen to have a small fire visible from the underside of the truck which resulted from the brakes being locked on. TSL was on site and used their extinguisher to put out the fire and move the truck to the exit ramp where MFB attended. The ramp was closed whilst the emergency services were on scene and the truck was towed. There were no injuries or delays.

B20 Thursday 26th June 17:09 – 18:07 Two vehicles were involved in a nose to tail collision in the right hand lane of the tunnel. There were no injuries and the SIRO attended and two tows were arranged. There was a minor secondary nose to tail in the left lane adjacent to the initial incident and those vehicles were moved to the already closed 33(1) right lane.

B39 Monday 30th June 12:25 – 13:50 A vehicle with its hazard lights on was travelling slowly in the left lane when the vehicles behind it failed to stop and a truck ran into the back of 3 cars. The 2 left lanes had to be closed as the truck was 33(1) partially in the centre running lane. SIRO, Police, 33(1) MFB and ambulance assisted on scene and Roads3 Corporation patients were transported. Traffic was Informationaffected to Act 1982 the Westgate Bridge and the Bolte Bridge. BURNLEY TUNNEL B52 Tuesday 5th August 18:54 – 19:11 A vehicle merged from the centre lane to the left lane right into the side of a SIRO vehicle. Nil injuries. 33(1) Left lane was closed for the incident but both vehicles drove out to the exit ramp. SIRO had some minor fabric damage to its shunt bar. One accident allocation was required for the other vehicle.

B04 Sunday 24th August 22:11 – 23:09 A truck changed lanes to the left outside the tunnel entrance and collided with a car, pushing the car in the left lane into the tunnel and into the emergency lane. SIRO, police and ambulance on site. Nil injuries and nil asset damage. One accident allocation required. Entry merge closed during the incident but nil delays.

B29 Monday 25th August 10:22 – 11:05 A truck changed lanes to the left and collided with a car, spinning the car 360 degrees before coming to Roads Corporation rest against the right NJB. SIRO and policeInformation on site. Act 1982 Nil injuries and nil asset damage. Police drove car 33(1) 33(1) out of the tunnel to await accident allocation and 33(1) 33(1) truck did not require a Released tow. 2 right lanes Under closed the Freedom of initially until SIRO moved truck into the right lane.

Long traffic delays resulted. B35 Monday 25th August 18:56 – 19:43 Nose to tail collision in the centre lane between a car and van. Both vehicles moved to the left lane which was closed due to congestion. SIRO on site with nil injuries and nil asset damage. One vehicle was shunted out of the tunnel to Burnley exit. One accident allocation required and one vehicle arranged their own tow.

DOMAIN TUNNEL D01 Friday 15th August 22:43 – 01:57 A fault from ERW01 / SUB05 caused some of the safety systems to become inoperable in part of the tunnel. As per procedure the tunnel was closed. TSL on site to repair and the tunnel was reopened once the fault was rectified. Nil delays resulted although traffic was diverted through City Rd to Power St.

Released Under the Freedom of Burnley Thursday 21st August 07:02 – 08:23 A truck coming out of the tunnel caught fire in the emergency lane. SIRO, police, ambulance and MFB on site. Nil injuries and nil asset damage. The cabin of the truck was fully engulfed and extinguished by MFB. The tunnel was closed as per MFB request and due to the amount of smoke to stop all traffic. Traffic was diverted along City Rd. Modern towing were required to tow the remains of the truck.

BURNLEY TUNNEL B08 Monday 6th October 05:06 – 06:07 A cattle truck traveling on the tripped the 4.65m and 4.7m detectors. The vehicle continued driving into the tunnel in the right lane when one of the livestock raised its head and struck two rows of overhead signs. SIRO and maintenance dispatched and removed two of the damaged signs. Two lanes were closed to fix and remove damaged signs, there were no significant delays. B31 Friday 10th October 17:32 -18:34 A truck stopped in the left lane of the Burnley tunnel, suffering an overheat issue which resulted in the truck shutting down. The truck could not move under own power and required modern towing to remove it. Heavy delays were a result from this breakdown.

BURNLEY TUNNEL B4 Saturday 8th November 04:18 – 04:28 A male pedestrian entered the Burnley Tunnel 33(1) undetected until he opened the B4 Firebox door. 33(1) The male manipulated some of the equipment in the Firebox before securing the door and continuing eastbound attempting to hitch a ride. He was apprehended by Police and removed from the tunnel.

B17 Monday 10th November 07:59 – 09:02 Two light trucks were involved in a nose-to-tail collision in the right hand running lane. The force of the impact resulted in the driver of the second vehicle being trapped in his cab. Two right lanes were closed and all Emergency Services attended. It took approximately 1 hour to release the driver, uninjured, and remove the vehicle from the tunnel resulting in extensive congestion on the network. B40 Sunday 23 November 17:33 – 18:24 The left lane was closed when a truck stopped Roads Corporation following a clutch failure. Unable to proceedInformation under Act 1982 its own power the truck was strap towed clear of the tunnel by the SIRO. Modern Towing attended. There were significant delaysReleased in the tunnel Under and on the Freedom of the Westgate Freeway back to the Bolte Bridge.

DOMAIN TUNNEL D06 Friday 7th November 12:13 – 13:02 Slow moving traffic suddenly came to a stop resulting in a 4 car nose to tail collision. The left lane was closed and a SIRO and 2 Accident 33(1) Allocation tows dispatched. There were no injuries. A minor oil and coolant spill was cleaned up before the lane was reopened. Traffic was congested back to Toorak Rd.

Burnley Tunnel Exit Portal 8th January 02:47 – 04:22 Driver of ute suffered a medical condition which required police and ambulance attendance. Ambulance transported driver to hospital for observation. Vehicle was towed back to the Operations and Maintenance Building until driver was released from hospital.

Burnley Tunnel 9th January 13:06 – 13:50 Overheight Vehicle Detector 1 was triggered by a semi-trailer vehicle with a damaged roof and piece of metal flapping. Tunnel closure plans were initiated. The over dimensional continued into the Burnley Tunnel travelling in the left lane, TSL 33(1) attended to assist with lowering the roof of the truck. BURNLEY TUNNEL

Firebox B26 Wednesday February 4th 11.42pm – 4.25am Maintenance staff came across a large amountRoads of Corporation water running down the new jersey barrierInformation at B26. It Act 1982 was originally presumed to be from the fire box but upon closer inspection it appeared to be a large leak from behind the fire box.Released It was estimated Under that the the Freedom of water was flowing at approximately 10 litres every 10 seconds. Temporary repairs have occurred and a more permanent solution is being assessed.

Firebox B49 – B59 Thursday February 12th 10.59am – 11.54pm The control room officers noticed a concrete spill in the left hand lane. The lane was closed and maintenance sent to inspect the spill. It was cleaned up without any residual damage to the road surface later that night. Significant delays were experienced during the daytime lane closure. The vehicle that dropped the concrete has been identified and cost recovery measures will follow.

Tunnel entrance to B52 Thursday February 12th 3.47pm – 4.00pm An over-height alarm was received and a top loaded cattle truck was located on CCTV. Multiple lanes were closed and the truck instructed to stop using the tunnel public address system. CityLink staff assisted the driver reposition the animal and the truck was escorted out of the tunnel. There was no damage to any tunnel assets. There were heavy delays on the Westgate Fwy during the incident

Tunnel entrance Monday February 23rd 7.47pm – 8.15pm A 4.85m over height truck was detected and the OHVD plan was implemented to close the Burnley tunnel. Despite this, the truck entered the tunnel and was finally stopped by operators using the in-tunnel PA system. CityLink staff and Transfield were sent to inspect the truck and a damaged roof panel was found to be the cause. The panel was secured and the truck escorted through the tunnel. No damage to Infrastructure was reported.

DOMAIN TUNNEL

D5 Wednesday February 18th 10.54am – 11.41am A driver lost control of his vehicle travelling in the right lane and collided with the barrier coming to rest in the centre lane. 2 lanes were closed and CityLink 33(1) staff along with Police, Ambulance and the fire 33(1)33(1) brigade were sent. The driver suffered minor injuries but was not transported to hospital. A tow was also ordered for the vehicle and traffic delays were significant back to the Bolte and Westgate bridges.

D25 Friday February 28th 15:20 – 17:00 A three vehicle nose to tail collision occurred in the centre lane due to slowing traffic. SIRO, police and ambulance on site. Nil injuries and nil asset damage. B40 Monday 2nd March 2015 14:00 – 15:30 A truck changed lanes to the centre lane and collided 33(1) with a car spinning it around and pushing it through 33(1) 33(1) the tunnel for a short distance facing sideways. Both vehicles drove out to the Burnley exit and exchanged details. M1IRO met up with drivers and no other services were required. Minor delays on initial lane closure. B47 Wednesday 4th March 2015 10:42 – 10:46 A cyclist was sighted at B47 by TOCO on CCTV scan and the left lane was closed. Police and M1IRO were called to attend but the cyclist exited on Burnley St before Police or M1IRO arrival.

B33 Sunday 8th March 2015 06:54 – 07:08 Two cyclists entered the Burnley tunnel from the Kingsway entry ramp. The left lane was closed with attempts to stop the cyclists utilizing the RRB/PA system. The cyclists were stopped at fire box B33. Police attended and issued fines to the both cyclists, the SIRO assisted in transporting the cyclists out of the tunnel onto Burnley St. There were no delays. B35 Monday 9th March 2015 05:07 – 05:19 A male person was detected in the right lane riding a motorised scooter via AID system. Police and Siro 33(1) called to attend. The right lane was closed. The rider then exited to the Burnley Street exit ramp into 33(1) Burnley Street before services arrival. There were no delays.

B27 Thursday 26th March 2015 16:27 – 17:54 A semi B-Double broke down in the left lane at Firebox B27 due to a mechanical failure and required a heavy haulage tow. The left lane was closed and caused significant delays to both the Westgate Fwy and Citylink to Racecourse Road

D22 Wednesday 11th March 16:50 – 17:17 An elderly man walked into the Domain Tunnel from Kingsway exit ramp up to D22 and the two left lanes were closed. Police attended and removed the male 33(1) from the tunnel. Significant delays back to Toorak Rd.

Run 4 Kids Event Sunday 22nd March 07:00 – 14:30 The 10th annual Herald-Sun Run 4 the Kids event was held with the closure of the Domain Tunnel and various roadways. There were approximately 32K entrants and there were three reported minor injuries. $1.8 M was raised for the RCH. A big thank you to all who participated and set the stage.

D28 Friday 27th March 2015 09:00 – 09:56 A fully loaded scrap metal dump truck had triggered the 4.85m over height detector and Phase 1-2 right lane closure was implemented. The truck was monitored travelling in the centre lane to enter the tunnel. A closure was initiated and the tunnel was closed briefly until the truck stopped in the left lane at D25. The driver then drove off. Transfield and SIRO

BURNLEY TUNNEL B57 Thursday 7th May 2015 15:53 – 17:02 A minor nose to tail collision occurred in the right lane. SIRO on site. Nil injuries and nil asset damage. One vehicle could not be moved and required a tow from the tunnel. The right lane was closed throughout the incident. 33(1)

B24 Thursday 21st May 2015 16:42 – 17:06 A cyclist entered the tunnel from Kings Way. The left lane was closed to protect the cyclist causing long 33(1) delays. The cyclist then stopped at B24 and called the METS. SIRO was dispatched to collect the cyclist and liaised with police outside the tunnel. The cyclist was issued with an infringement notice by police.

DOMAIN TUNNEL D28 Wednesday 27th May 2015 23:59 – 00:29 A vehicle exiting the tunnel collided with a road sign. The vehicle did not stop. Nil injuries. The exit sign was badly damaged. Maintenance attended on site and removed the damaged sign. The incident was reported to police and the driver was later apprehended and taken to hospital for a blood test.

BURNLEY TUNNEL B20 Tuesday 2nd June 2015 – 1951-1956 A tray-type utility was travelling along the left lane when the car in front slowed (braking). The ute applied the brakes, locking the rear wheels and veered left running into and up the NJB before veering back across the tunnel and stopping in the right lane. Traffic stopped and the driver alighted, picked-up and re-secured the sides for the tray and drove away.

DOMAIN TUNNEL D14 Friday 5th June, 2015 – 1652-1742 Unknown leak from disabled vehicle. MFB attended and confirmed to be radiator leak. Vehicle was shunted from tunnel by SIRO.

BURNLEY TUNNEL B24 Wednesday 1st July 2015 0334 - 0347 A pedestrian entered the Burnley tunnel from the exit portal, walking along the elevated walkway. The pedestrian appeared to be disorientated and was 33(1) 33(1) closely monitored throughout the tunnel via CCTV. The pedestrian was stopped and apprehended by Police at fire box B24. The right lane of the Burnley tunnel was closed throughout the duration of the incident. No delays caused due to the time of the incident. B01 Friday 10th July 2015 1250 - 1447 Over Height Vehicle 4.85m detected at OHVD1 (West Gate Fwy). Driver continued past the Power St exit toward tunnel. Phase 2 (Full Closure) activated but truck continued at reasonably low speed into tunnel and was finally stopped at B18 by a motorist. Incident Response attended and monitored as the load height (excavator) was lowered, measured and allowed the truck to proceed to the Burnley St exit to await VicRoads Transport Safety Services (TSS). MinorRoads Corporation delays caused due to tunnel lane closures.Information Act 1982 B59 Friday 17th July 2015 2310 - 0022 A vehicle exiting the Burnley tunnel in the centre lane collided with the rear of aReleased truck it was following. Under The the Freedom of 33(1) two left lanes were closed for the duration of the 33(1) 33(1) incident with minor delays caused due to the time of the incident. The driver was assessed on scene by a passing ambulance, no injuries reported. The driver was later transported to hospital by Vic Pol for further investigation.

DOMAIN TUNNEL D06 Friday 17th July 2015 0925 - 1052 Multi vehicle nose to tail collision involving a truck blocking the left lane of the Domain tunnel. All vehicles were moved from the tunnel to the Kingsway Exit ramp to exchange details. One driver was treated by ambulance with minor injuries and remained on scene. Heavy traffic delays back to Glenferrie Road, due to the Tunnel lane closure. D08 Wednesday 29th July 2015 2156 - 2308 A truck travelling in the centre lane of the Domain Tunnel changed lanes to the left and side swiped a Toyota pushing it into the new jersey barrier. After both vehicles exited the tunnel and stopped in the left hand emergency lane, an ambulance was requested for a female driver complaining of sore neck, shoulders and leg. One accident tow allocation attended. The left lane in the Domain Tunnel was closed to slow traffic past the incident. There were no delays due to the time and location of the incident.

The truck caused damage to one sodium light fitting. Major delays resulted.

7.2 Incidents of Note Roads Corporation Information Act 1982 The following incidents and activities are considered noteworthy: BURNLEY TUNNEL Released Under the Freedom of B30 Sunday 25th October 2015 14:13 – 14:45 A vehicle travelling in the right lane of the Burnley Tunnel suffered a mechanical failure where the trailer detached from the vehicle and required a metal pin to re secure the trailer. A temporary repair was made to the trailer and was able to be driven clear of the tunnel where a tow was ordered. Heavy traffic delays were caused back to Montague Street due to this breakdown.

DOMAIN TUNNEL

D22 Monday 12th October 2015 13:54 – 15:21 A truck suffered a drive line failure whilst driving through the Domain Tunnel and was unable to continue beyond firebox D22, blocking the left lane. A heavy haulage tow was required to remove the broken down vehicle. The left lane was closed for the duration of the incident causing heavy traffic delays back to Glenferrie Rd.

D08 Monday 19th October 2015 15:18 – 15:56 A vehicle stopped in the left lane at box D08. Upon arrival, the SIRO found a 2 year old child unresponsive and unconscious in the vehicle with a parent. An Ambulance was called and the SIRO supported and guided the parents with advice until paramedics arrived. The infant was transported to hospital. Significant traffic delays caused with traffic delays back to Glenferrie Rd.

The following incidents and activities are considered noteworthy: BURNLEY TUNNEL

B01 Sunday 1st November 2015 06:03 – 06:14 A pedestrian entered the tunnel from Kingsway in the left emergency lane. The left lane was closed for safety purposes until the pedestrian ran across the road and proceeded to walk along the raised walkway, after which time the right lane was closed. Police arrived on site and 33(1) escorted the male out of the tunnel. No traffic delays were 33(1) 33(1) experienced.

B55 Thursday 5th November 08:43 – 10:59 A container truck that was travelling in the centre lane of the tunnel suffered transmission failure and was unable to proceed. Modern towing was requested to attend and the centre lane was closed for the duration of the incident. As Modern towing was hooking up the first truck, a second truck broke down in the left running lane at the exit portal of the tunnel and the left and centre lanes had to be closed. Modern towing took the first truck off the link and returned with assistance travelling contraflow on the Burnley Street exit ramp and recovering the second truck. Traffic delays were very large with report of issues as far as the Western Ring Road and beyond English Street on the .

B32 Monday 9th November 16:09 – 16:20 A cyclist entered the Burnley Tunnel from the Kingsway entry ramp and continued through the tunnel travelling in the left lane. The SIRO was sent and the left lane was closed with the cyclist being intercepted at B32. The SIRO loaded the bike and cyclist into the van and escorted them off the roadway without further incident. There were localised delays while the cyclist was removed duringRoads peak hour Corporation traffic. Information Act 1982

B32 FridayReleased 20th November 06:50 Under – 07:54 the Freedom of The left lane was already closed due a breakdown when a spill of concrete dust was observed in the closed lane. Maintenance were advised and sent into the tunnel to inspect the spill. The spill was spread from B32 to B53. The Sweeper was sent to remove dust from roadway. There were significant delays experienced as far back as the Westgate Bridge.

DOMAIN TUNNEL

D11 Thursday 5th November 2015 10:30 – 10:40 Two vehicles collided in the left lane after one vehicle failed to stop during congested traffic conditions. Both drivers exited their vehicles to exchange details in the tunnel. SIRO arrived on site and moved all vehicles out of the tunnel. Nil injuries and nil asset damage. No tows were required. Traffic was congested prior to the incident but further delays resulted to Yarra Blvd.

Power St exit Monday 23rd November 2015 05:11 – 06:29 TCRO received a call from a motorist reporting a grass fire at the entrance to the Burnley Tunnel. TCRO located fire on Mini Magic Mountain area via CCTV. The fire was located on the left side near the centre of the grass area. Fire services were on scene blocking the Power Street exit ramp near Sturt Street. The SIRO was advised to close both Kingsway and Power St exits from Domain Tunnel. Smoke was entering the Domain Tunnel and longitudinal ventilation was activated and all lanes reduced speed to 60KPH. It is unknown how the fire started. There were no delays during this incident.

Entry Portal Monday 23rd November 2015 12:30-13:11 A truck and car collided during a lane change resulting in two right lanes being closed. The truck clipped the car resulting in the vehicle coming to rest across the right running lane and backing onto the new jersey barrier. Two right lanes were closed to ensure vehicle and scene safety as the vehicle could not be moved due to axle and front wheel damage. Both Police and Ambulance attended, with nil injuries recorded. There were heavy delays on the Southern Link beyond Toorak Road.

B12 Wednesday 2nd December 2015 00:46 – 01:37 Two vehicle lane side swipe collision involving a truck. The truck changed lanes and collided with a second vehicle spinning and pushing the vehicle before stopping in the right lane at fire box B12. Emergency services attended the collision and the traffic control 33(1) room. No injuries were reported. Minor delays caused passing the incident site. 33(1)

B16 Friday 4th December 2015 09:17 – 10:03 OHVD-01 detected an over height vehicle (4.85M) inbound to the Burnley Tunnel and initiated Phase 1 of the Tunnel Closure. The vehicle, a low-loader with excavator on board, was observed travelling in the right lane. When the vehicle made no attempt to exit at Power Street the TCRO implemented a full tunnel closure. The OHVB fingers were struck by the extended arm of the excavator and the truck continued into the tunnel in the right lane. The TCRO made several calls on the RRB/PA instructing the driver to stop, doing so at B10 where a SIRO attended and measured the truck at 5 Metres high. After lowering the load the vehicle was escorted from the tunnel. Maintenance conducted an inspection of the tunnel infrastructure and found that 3 TIM’s signs had been struck and damaged.

D14 Friday 4th December 2015 07:42 – 08:52 Three vehicle nose to tail collision blocking the left lane of the Domain tunnel at fire box D14. Emergency services attended and treated one driver with a minor head injury with the driver remaining on scene after refusing to be transported to hospital. The left lane was closed 33(1) for the duration of the incident causing heavy traffic delays beyond Toorak Road. 33(1)

B45 Wednesday 10th February 2016 14:38 – 16:40 A truck suffered a terminal mechanical fault blocking the right lane at fire box B45. Due to the mechanical fault the vehicle was unable to be moved and a heavy haulage tow was required. The right lane remained closed for the duration of the breakdown causing heavy traffic delays beyond the and Dynon Road on the elevated roadway..

B14 Tuesday 16th February 2016 21:48 – 21:58 A cyclist entered the Burnley Tunnel via the Kingsway entry ramp and was picked up on CCTV at fire box B14 travelling in the left running lane. In order to minimize the risk the left lane was closed in the Burnley tunnel with a SIRO dispatched from the OMB, Vic Pol were also called to attend. The SIRO was able to intercept the cyclist at the Burnley street exit, where he advised the SIRO he was lost looking to go to Kingsway. No effect on passing traffic due to the time and location of the incident.

DOMAIN TUNNEL

D09 Sunday 14th February 2016 15:22 – 16:19 A vehicle travelling in the left lane of the Domain tunnel suffered a mechanical fault snapping the tail shaft. The following incidents and activities are considered noteworthy: BURNLEY TUNNEL

B46 Tuesday 8th March 2016 07:12 – 09:18 A truck was travelling through the Burnley Tunnel in the centre lane when the strap broke on the rear load of the trailer. The load slid off the trailer onto the roadway. The left and centre lanes were closed for the duration of the incident causing heavy traffic delays due to the time and location of the incident. Heavy haulage was required in the salvage operation of the lost load. VicRoads TSS enforcement attended to assess the vehicle and assist with traffic management while on scene.

B12 Sunday 13th March 2016 02:16 – 03:35 Burnley Tunnel wall washing was in progress with two right lanes closed and two attenuators buffering the workers and mobile work site. A vehicle entered the tunnel in the centre lane ignoring all the warning signs and continues to drive east bound in the closed centre lane before colliding with rear of one of the impact attenuators. Nil injuries 33(1) were reported with one accident allocation required and VicPol attendance requested.

No impact on passing traffic due to the time and location of the incident.

DOMAIN TUNNEL

Kingsway exit Wednesday 16th March 2016 09:03 – 10:07 A vehicle lost control on the Kingsway exit ramp fromRoads the Domain Corporation tunnel colliding with a static sign and tree in theInformation left landscaping area. Act 1982 Damaged sign was removed by maintenance with the area made safe. Minor damaged sustained to the landscaping area. Minor effect on passing trafficReleased while the vehicle wasUnder removed fromthe the Freedom of collision site. All lanes were temporally closed to salvage the damaged vehicle. No report of any injuries. D18 Tuesday 29th March 2016 05:25 – 06:19 A vehicle travelling in the left lane of the Domain tunnel lost their rear wheel due to mechanical failure. A tow truck was required to remove the vehicle from the tunnel due to the mechanical damage caused. No damage sustained to the left lane as a result of this incident. The left lane was closed for the duration of the incident with traffic delays pushing back to the Gibdon St entry ramp. All delays cleared quickly once all lanes were re-opened.

BURNLEY TUNNEL

B59 Thursday 5th May 2016 06:05 – 07:26 A vehicle was reported to be driving erratically in the tunnel colliding with two vehicles. The vehicles involved managed to stop the offending vehicle in the centre lane of the exit portal. The driver of the offending vehicle left the collision scene and abandoned his vehicle in the centre lane of the tunnel exit portal. The centre lane of the tunnel was closed until all vehicles could be moved to the Burnley exit ramp to await Vic Pol to attend. Traffic delays pushed back onto the West Gate Fwy to the Power St exit ramp. No injuries were reported as a result of the collision.

B49 Sunday 15th May 2016 18:10 – 18:13 A cyclist entered the Burnley Tunnel from the Kingsway entry ramp and travelled in the left lane before being detected at fire box B49. The left lane was closed with Vic Pol requested to attend. The cyclist managed to clear the tunnel and exit via the Burnley St exit ramp before Vic Pol arrived. The left lane was only closed for a short period of time with no effect on the passing traffic.

B41 Friday 20th May 2016 21:34 – 21:54 A cyclist entered the Burnley Tunnel from the Kingsway entry ramp travelling in the left lane. Report of the cyclist came through from Vic Pol with the rider located on camera at B41. The Cyclist made it through the tunnel stopping at the top of the Burnley St exit ramp with the SIRO and Vic Pol attending shorty after exiting. The left lane was closed briefly with only minor traffic delays caused

Roads Corporation B43 Monday 20th May 2016Information 07:15 – 08:08 Act 1982 Two vehicle nose to tail collision blocking the right lane of the Burnley Tunnel at fire box B43. Attending SIRO shunted one of the vehicles to the Burnley St exit ramp Released to exchange details Under and await accidentthe Freedom of allocation tow. Vic Pol attended and assisted with traffic management. Heavy in tunnel delays and on approach to the tunnel back to the Graham St overpass due to the time and location of the collision. No report of any injuries.

A semi-trailer truck was changing lanes from centre lane to left lane as it was entering the tunnel, colliding with a vehicle travelling in the left lane. The impact caused the vehicle to spin out of control colliding with a second vehicle before coming to a stop contraflow in the right lane. The left and right lanes were initially closed until the truck involved was moved into right lane. The right lane remained closed for the duration of the incident. Minor traffic delays caused due to the time and location of the incident. No report of any injuries as a result of the collision.

D28 Monday 30th May 2016 Two vehicle lane side swipe collision involving a truck blocking the left lane of the Domain Tunnel exit portal. The truck involved changed lanes spinning the car which came to a stop facing sideways blocking the left lane in the tunnel exit portal. The left lane was closed in the Domain Tunnel to accommodate the collision in the exit portal until all vehicles were moved to the left emergency lane. Traffic delays back to the Gibdon St entry ramp. All delays cleared quickly once all lanes were re-opened.

BURNLEY TUNNEL

B37 Tuesday 7th June 2016 15:32 – 15:33 A cyclist entered the Burnley Tunnel from the Kingsway Entry ramp travelling in the left lane of the Burnley Tunnel where it was located via CCTV at B37. Vic Pol was called to attend and the left lane was 33(1) closed briefly until the cyclist exited from the tunnel onto the Burnley Street exit ramp. The cyclist moved on before Vic Pol arrived.

B46 Wednesday 15th June 2016 18:30 - 1833 A cyclist entered the Burnley Tunnel via the Kingsway entry ramp. He managed to ride all the way to the Burnley St exit ramp near firebox B58 before being detected by AID. The cyclist exited the tunnel via the Burnley St exit ramp and continued out to Burnley Street, leaving the scene before emergency services attended. No effect on passing traffic as the cyclist travelled through the Burnley tunnel.

B43 Friday 17th June 2016 11:01 – 11:32 Two cyclists entered the Burnley tunnel via the Kingsway entry ramp travelling in the left lane. 33(1) The left lane was closed with the SIRO and Vic Pol contacted to attend and escort the prohibited users from the tunnel. The riders were towing trailers with advertising boards for the company Deliveroo. Once clear of the tunnel both riders were cautioned by Vic Pol with further investigations to follow. The left lane remained closed until the incident was cleared of the tunnel, minor in tunnel traffic delays pushing back to theRoads Power St exit Corporation ramp on the West Gate Fwy. BURNLEY TUNNEL

B29 Tuesday 5th July 2016 13:04 – 13:20 A cyclist from a food delivery company (Foodora) entered the Burnley Tunnel in the left lane from the Kingsway entry ramp. He rode the length of the tunnel and took the Burnley St exit. The cyclist continued 33(1) towards Swan St where the TCR lost vision. Vic Pol were requested to attend and it is unknown if they intercepted or spoke to the rider. The left lane of the Burnley tunnel was closed until the cyclist exited safely at the Burnley St exit ramp. Traffic delays pushed back to the Lorimer St exit ramp as a result of this incident.

B02 Friday 15th July 2016 22:23 – 00:12 Two vehicle nose to tail collision involving a truck blocking the left lane of the Burnley tunnel at B02. The SIRO arrived and moved all vehicles to the left emergency lane to exchange details and await accident allocation and heavy haulage tows. Vic Pol were requested to attend by both drivers after an alleged road rage incident prior to the collision. No impact on passing traffic due to the time and location of the collision. No reports of any injuries as a result of this collision. B53 Sunday 24th July 2016 03:05 – 03:35 Two cyclists entered the Burnley tunnel from the exit portal riding contra flow in the right lane. The right lane was immediatelyRoads closed Corporation with Vic Pol requested to attend. The cyclistsInformation travelled in the right Act 1982 33(1) lane until they reached B35 they then moved to the elevated walkway randomly opening door as they continued through the tunnel. Once they reached B33 they entered the emergency egress tunnel. Vic Pol later arrived and could not locateReleased the cyclists. Under the Freedom of Sometime later the TCR received a surface door alarm at Burnley St. The cyclists had exited the tunnel and were later spotted in Burnley but managed to evade the police.

B50 Friday 29th July 2016 A male pedestrian entered the Burnley tunnel from the exit portal walking along the elevated walkway. The pedestrian was apprehended by Vic Pol at fire box B42 and 33(1) escorted from the tunnel. The right lane was closed for the duration of the incident with only a minor in tunnel delays.

B30 Saturday 30th July 2016 18:45 – 18:59 A cyclist working for Foodora entered the Burnley Tunnel via the Kingsway on ramp. The left lane was closed to ensure the cyclist was kept safe and away from passing traffic. VicPol attendance was requested however no units where available to assist with the SIRO intercepting the cyclist at the Burnley Street exit. Minor in tunnel delays caused while the left lane remained closed.

DOMAIN TUNNEL

D19 Tuesday 7th July 2016 12:04 – 13:08 Multi vehicle lane side swipe collision involving a truck blocking the left lane of the Domain tunnel. The truck involved stopped in the left emergency lane of the Kingsway exit ramp, the SIRO later shunted the other vehicle involved from the closed left lane. Vic Pol attended and assisted with traffic management, no injuries as a result of the collision. Traffic delays pushed back to Yarra Blvd which cleared quickly once all lanes were re-opened. D01 Friday 22nd July 2016 14:34 – 15:55 OHVD4 activated 4.85 over height detectors and a truck carrying a digger was observed travelling inbound on the Southern Link. Once the truck reached Punt Road the operator decided to close the tunnel in an attempt to stop the truck from entering. The truck continued in the left lane and travelled under the over height fingers knocking them and still continued into the tunnel. The operator utilised the RRB/PA 33(1) system and indicated to the driver that they were required to stop due to the height of their load. The vehicle eventually stopped at D10 in the left lane. The attending SIRO confirmed the vehicle was over height and assisted in having the arm of the boom lowered so the vehicle could continue through the tunnel. Maintenance attended and checked the tunnel with no damage to any equipment as a result of the truck entering the tunnel.

Another vehicle was picked up by the video incident detection system stopped just before entering the inbound tunnel. He then reversed back over 500m along the entry ramp before being stopped by our patrol when he drove off back through the tunnel. This was overnight and the ramp traffic volumes were low at the time. A pedestrian was reported in the Melba tunnel and in the area of Maroondah Hwy, but was substantiated following a search on the CCTV network and by responding units.

A stopped vehicle was shunted by an IR vehicle from the Mullum Mullum tunnel and then subsequently needed to be towed from outside the portal.

Attachment A EastLink Alliance Report January to March 2015.docx 10 of 18 Attachment A Significant incidents of interest April: There was 3 reported fires, 1 vehicle and 2 roadside. The vehicle fire was a car stopped in the tunnel after it had “dropped” its tail shaft early on the 3rd April. The driver and passengers got out to look at what happened and used a fire extinguisher taken from the fire cabinet under the car. No smoke was ever seen. They then left the vehicle in the tunnel after being driven out in a prime mover (with no trailer) before the response unit could arrive. The vehicle was removed by the police. Use of the extinguisher is believe to be possibly due to oil on a hot surface. No fire response was initiated by the tunnel operator.
