Lee Moffitt Cancer Center and Research Institute, University of South Florida, Tampa, Florida 33612-9497 ABSTRACT Malassezia furfur (MF) is a lipophilic yeast which can be found as a member of the indigenous microbiota of human skin. In immunocompro mised transplant patients, MF can cause a distinctive folliculitis which is a clinical look-alike to Candida folliculitis, the latter of more potentially devastating significance. Recovery of MF in culture is dependent upon the addition to culture media of an exogenous source of fatty acids, such as olive oil. The addition of an extra Sabourauds plate with an olive oil overlay to the routine set of media used to inoculate all skin biopsy specimens in order to detect MF is labor-intensive and not cost-effective. Thus, MF may not be isolated in cases of MF folliculitis unless the clinical microbiology laboratory is put on alert by the clinical suspicions of the attending physi cian, or by histopathologic findings suggestive of folliculitis revealed by review of surgical pathology slides. -

GOLDMAN, M.D. 150 Broadway, Suite 1110 NEW YORK, NY 10038 E-MAIL [email protected] INTRODUCTION Genital dermatology encompasses a wide variety of lesions and skin rashes that affect the genital area. Some are found only on the genitals while other usually occur elsewhere and may take on an atypical appearance on the genitals. The genitals are covered by thin skin that is usually moist, hence the dry scaliness associated with skin rashes on other parts of the body may not be present. In addition, genital skin may be more sensitive to cleansers and medications than elsewhere, emphasizing the necessity of taking a good history. The physical examination often requires a thorough skin evaluation to determine the presence or lack of similar lesions on the body which may aid diagnosis. Discussion of genital dermatology can be divided according to morphology or location. This article divides disease entities according to etiology. The clinician must determine whether a genital eruption is related to a sexually transmitted disease, a dermatoses limited to the genitals, or part of a widespread eruption. SEXUALLY TRANSMITTED INFECTIONS AFFECTING THE GENITAL SKIN Genital warts (condyloma) have become widespread. The human papillomavirus (HPV) which causes genital warts can be found on the genitals in at least 10-15% of the population. One study of college students found a prevalence of 44% using polymerase chain reactions on cervical lavages at some point during their enrollment. Most of these infection spontaneously resolved. Only a minority of patients with HPV develop genital warts. Most genital warts are associated with low risk HPV types 6 and 11 which rarely cause cervical cancer. -

Phase 4 Medicine Intended Learning Outcomes (ILOs) This Phase 4 document outlines the listed ILOs for Medicine. This will be examined in the Year 4 and Year 5 summative written examinations. It is important that we impress upon you the limitation of any ILOs in their application to a vocational professional course such as medicine. ILOs may be useful in providing a ‘shopping list’ of conditions that you will be expected to describe and anticipate. The depth and extent of your knowledge of each condition will be a joint function of the condition’s frequency and its gravity. Please use the ILOs to make sure you are familiar with the common and important presentations and conditions. The list does not comprise of the entire coda for successful medical practice but will provide you with a solid platform from which to build upon. More detailed explanations and outlines will be available in the standard textbooks. Any elucidation or expansion can be obtained there. Even more important is the point that ILOs will point you in the correct direction to pass our written exam, but that this is only part of the story. Alpha Thalassemia
Alpha Thalassemia Alpha thalassemia is a genetic disorder called a hemoglobinopathy, or an inherited type of anemia. People who have alpha thalassemia make red blood cells that are not able to carry oxygen as well throughout the body, which can lead to anemia. This chronic anemia can include pale skin, fatigue, and weakness. In more severe cases, people with alpha thalassemia can also develop jaundice (yellowing of the eyes and skin), heart defects, and an enlarged liver and spleen (called hepatosplenomegaly). Alpha thalassemia is more common in people of African, Southeast Asian, Chinese, Middle Eastern, and Mediterranean ancestries. Causes We have over 20,000 different genes in the body. These genes are like instruction manuals for how to build a protein, and each protein has an important function that helps to keep our body working how it should. The HBA1 and HBA2 genes make a protein called alpha-globin. Two of the alpha-globin proteins combine with two other proteins called beta-globins (which are made by the HBB gene) to make a normal red blood cell. Most people have four copies of the genes that make the alpha-globin protein: two copies of the HBA1 gene (one from each parent), and two copies of the HBA2 gene (one from each parent). Having all four copies of these genes is symbolized by writing αα/αα.
