MARINE SPILL RESPONSE CORPORATION
The Marine Spill Response Corporation (MSRC) is a not-for-profit, U.S. Coast Guard Classified Oil Spill Removal Organization (OSRO) and a Washington State approved Primary Response Contractor (PRC). MSRC was formed in 1990 to offer oil spill response services and mitigate damage to the environment. MSRC resources are included and cited in the WSMC Plan through WSMC membership in the Marine Preservation Association (MPA) under the WSMC Class Membership Agreement. This Agreement is available for Ecology review upon request. Additionally, the MSRC Certification letter is included in this appendix.
MSRC is recognized for its open-ocean and nearshore mechanical recovery capability and additional response resources which includes:
Responder Class Oil Spill Response Vessels (OSRVs) and other OSRVs Oil Spill Response Barges (OSRBs) Thermal and Multi-spectral aerial imaging capability through exclusive contract X-Band radar and infrared camera systems Fast Response Vessels (FRVs) Shallow Water Barges Marco Skimming Vessels Oil containment boom for all operating environments Skimming systems Fire Boom Dedicated dispersant aircraft Dispersant stockpiles Emergency communication equipment Wildlife facilities Dedicated Personnel

GLOBAL DIVING & SALVAGE, INC.
Global Diving & Salvage, Inc. (GDS) has a long history in both marine and land-based emergency response, with experience ranging from large-scale marine petroleum spills to small petroleum response on land. The Company specializes in near shore, on-water recovery, shoreline clean-up and cargo lightering; all these operations allow the Company to conduct its normal day-to-day environmental services, but also provide available emergency response equipment and personnel should the need arise. Global has over 100 trained response personnel in the Puget Sound region with the ability to cascade in additional personnel from the Western United States. GDS is an approved Primary Response Contractor in the State of Washington, has a basic ordering agreement with the USCG, and has contracts in place with every major facility and marine transporter in the State of Washington.
A GDS resource listing is included in this appendix and indicates the dedicated and non- dedicated equipment available to WSMC. These GDS resources are used when readily available to ensure the most fast and effective response.

This letter confirms that Global Diving & Salvage (GDS) has a Response Agreement with the Washington State Maritime Cooperative (WSMC). This Agreement provides access to resources for spill response services on behalf of the WSMC members’ vessels operating in Washington State waters. GDS confirms that WSMC is authorized to reference GDS vessels and certifications as well as GDS resources, including personnel as available for WSMC’s Spill Management Team, in its Washington State contingency and response planning documents pursuant to the terms of the Agreement,
GDS is an approved Washington State Primary Response Contractor (PRC). GDS is capable of beginning mobilization of response efforts within one hour of a spill notification. ISLANDS’ OIL SPILL ASSOCIATION
Island’s Oil Spill Association (IOSA) is a community-based, private non-profit organization that provides a range of response services including initial assessment, containment and cleanup and oiled wildlife rescue. Since 1988 IOSA has been on-call 24 hours a day throughout the year and, as of December 31, 2005, has been paged for 410 spill reports in and around the San Juan Islands and for wildlife rescue services in 3 major spills in other parts of Washington State. Out of the total, 85 have involved ongoing oil and hazardous material/debris recovery, containment, booming, stopping or removing the source of oil and/or wildlife rescue.
Services include: Quick spill assessment Oil containment, exclusion, diversion Wildlife search and rescue, primary care Spill source removal Oil and oiled debris recovery Disposal Support and safety functions Prevention Hands-on training and practice drills for community members HAZWOPER training (HAZWOPER is an abbreviation of Hazardous Waste Operations and Emergency Response)

Bakercorp is pleased to offer our full range of liquid and solid containers, pumps and filters for rent to Global Diving and Salvage in the event of a release of materials both hazardous and non-hazardous, to land or navigational waters.
Bakercorp has a full array of spill response equipment in our Seattle and Portland branches including over 300 tanks ranging in sizes from 550 gallon stainless steel totes up to 21,000 gallon carbon steel tanks. Our tank sizes include:
550 gallon stainless steel totes 630 gallon forkliftable poly tanks 4000 gallon trailer mounted poly tanks 4000 gallon poly tanks 6500 gallon poly tanks 7000 gallon stainless steel DOT approved tank trailers for transporting over the highway. 19600 gallon tanks with epoxy coated lining 18000 gallon double wall tanks 21000 gallon tanks, some with heating coils 10, 18 and 20 yard roll off containers for soils or other solids 7500 gallon secondary containment for our poly tanks 10 x 50 safe guard berms for larger tanks or to use as a de-con station for boom etc.
Bakercorp also carries a full line of diesel and electric pumps, in various sizes that are capable of pumping up to 6,300 GPM and several miles of 4, 6, 8 and 12” quick connect pipe and hose.
In addition, we have a variety of filtration equipment which includes 2, 3, 4, and 6” bag and cartridge units, as well as carbon media vessels and a wide selection of carbon Media(s) on hand to deal with various contaminates.
BakerCorp is a 24 hour emergency response company with key personnel on call at all times. We will respond to the first call within 30 minutes. Our 24 hour contact numbers are Portland 503-775-7211, Seattle 425 347 8811 or simply 1- 800-BAKER-12.
Baker can commit to deliver eight, 21,000 gallon tanks within 12 hours for a total of 168,000 gallons of capacity. In an extreme emergency situation we will have tanks delivered from our Portland branch as well as common carriers bringing tanks from both Seattle and Portland branches. If needed we can reach our branches in San Francisco, Salt Lake City, and Los Angeles for additional manpower as well as more equipment.
With the combination of these branches and the full range of our equipment; Bakercorp can provide 25,000 BBL’S of temporary storage in the form of 50+ steel tanks. Transportation times will vary but this capacity can be brought to bear quickly and on short notice.

NOTICE FOR WSMC COVERED VESSELS: In accordance with Washington State Law, this WSMC FIELD DOCUMENT must be maintained on board the covered vessel and kept in a conspicuous and accessible location while the vessel is in Washington State waters. Keep this FIELD DOCUMENT on the navigation bridge and filed with any other pollution contingency plan documents for the vessel. This FIELD DOCUMENT is to be used in the event of a spill or the substantial threat of a spill. A vessel emergency could include: loss of propulsion, loss of steering, collision and any other occurrence that affects the operation of the vessel. Determining whether an emergency exists should also take under consideration ship location, weather, currents, seas, traffic, severity and nature of damage and timing or likelihood of vessel repairs.

1. Obtain from Marine Exchange (MarEx) watchstander the reporting party contact information; name, title/position, direct phone number. 2. Confirm it is a WSMC incident: - Product released is oil. - In WSMC coverage area. - Spill comes from a vessel. - Vessel is WSMC enrolled. 3. Obtain incident information from reporting party, - Source secured/not secured - Status of vessel - Location of vessel - Type and amount of product released in total and released to water - Potential spill amount, product left in tank - Description of oil observed on water - Actions taken by vessel 4. Confirm required Federal and state notifications are, or will be made by either the reporting party or the MarEx watchstander. Get the Case Numbers. 5. Call PRC to dispatch initial response resources for assessment, containment and recovery as appropriate. Instruct PRC to use best judgment to contain and control the spill and protect environmental resources through GRP’s. Ensure PRC completes Site Safety Survey. If available, provide pertinent information about the product released such as sour crude, benzene component, flammability, sinking potential, etc. 6. Notify WSMC IC #2 of spill incident and actions being taken. As appropriate request assistance on-scene, conduct of overflight, standing up WSMC Spill Management Team (additional ICs, MarEx staff, Genwest, Polaris, Nexus NW, etc.). 7. Consider contacting vessel agent, port facility or other local reps for additional information.
If aerial surveillance is needed 8. Consider helo or fixed wing; contract or USCG. 9. Establish means to communicate observations back to the IC. 10. Consider alternative surveillance methods; PRC work boats, tugs in the area, WSF command center, King Co FLIR.
Follow on communications 11. Contact WA Ecology (ECY), either through WEMD or appropriate ECY regional office. Provide your contact info and situation update. Ensure ECY concurs that response actions being taken are appropriate. 12. Contact US Coast Guard (USCG), either through appropriate Sector Duty Officer. Post Incident 36. Submit ICS 201 Incident Briefing, ICS 214a Individual Log with response hours and any other pertinent incident documentation to Response Manager.
RESPONSIBLE PARTY’S ACKNOWLEDGEMENT OF TRANSFER
1. An oil spill incident has occurred as set forth below. The Washington State Maritime Cooperative ("WSMC") has made an initial emergency spill response pursuant to its Member Blanket Enrollment Agreement (hereafter jointly referred to as "Enrollment Agreement"), and in accordance with the laws of the State of Washington and WSMC's Contingency Plan on file with the State of Washington's Department of Ecology.
2. The undersigned is considered to be the Responsible Party for the spill and is now prepared to take responsibility for spill response, containment and cleanup.
3. The Federal On-Scene Coordinator and the State On-Scene Coordinators have been made aware of this transition.
4. At the time and date set forth below, the Responsible Party shall relieve WSMC of all further responsibility for organizing, managing or implementing the spill response or cleanup in accordance with the Enrollment Agreement or Contingency Plan.
5. Responsible Party hereby accepts full financial responsibility from the time the spill occurred for all expenditures made or incurred on its behalf by WSMC in contracting for spill response, containment or cleanup activities, and for such other spill related expenses as set forth in the Enrollment Agreement.
6. The person designated below as the Incident Commander, is the person authorized to speak for the Responsible Party in all matters related to the spill and associated containment and cleanup; and, to obligate the resources necessary to carry out the cleanup activities. In so doing, the Responsible Party and Incident Commander shall keep the Federal and State On-Scene Coordinators fully advised of the actions taken or to be taken. Responsible Party shall cooperate fully with the Coordinators in implementing the provisions of the federal and state cleanup requirements and of the WSMC Contingency Plan.
7. By accepting and acknowledging transfer of operations and financial responsibility pursuant to this Acknowledgment of Transfer, Responsible Party does not waive any rights, limitations or defenses available to it under either Washington or Unites States law.

NOTE: Summarized below are some common issues that arise during an oil spill response. However, because every situation in unique, it is impossible to capture every detail for the Responsible Party’s (RP) consideration.
This checklist is provided for the RP’s use and benefit as a tool, or quick reference guide, to assist the RP in the transition process. It should be used before executing the formal RESPONSIBLE PARTY’S ACKNOWLEDGMENT OF TRANSFER document (from WSMC to the Responsible Party).
