The team members are a variety of professionals, nursing, medical and recovery support workers.They work closely together as a team to deliver safe and effective care.

What we do? We provide support to adults aged 18 years and over who live in Sutton and are experiencing a mental health crisis.We provide the care and support in the community as an alternative to hospital admission, so you can stay in your home.

How can we help you? • We provide an ongoing assessment in the community to help you through a crisis and stay out of hospital • We will develop a holistic plan of action and support with you • We will direct you to the appropriate services for follow up after the crisis • Before discharge we will work with you to develop a personal crisis plan.

Sutton Home Treatment Team.indd 3 25/05/2016 17:04 Where can we see you? • At your home • You can come and see us at Sutton Crisis & Home Treatment Team if you prefer. • At the home of a family member or friend in Sutton where you may be staying, if you prefer this option. • As we work as a team, different members of the team will be involved in your care and treatment.

The typical patient journey with HTT

1. You are referred to HTT either because you are felt to be becoming unwell or if you are discharged from hospital early. 2. A member of HTT comes to do an assessment with you. 3. We will begin work with you if you meet our referral criteria or refer you to another service more suited to your needs. 4. A Health Care Professional will see you for a review. They work together with you to make a plan that works for you and aims to get you better quickly. 5. We usually see you every day initially to provide support to you and your family. We will monitor your progress with any medication prescribed for you and discuss changes to medications that we think will help. 6. As you get better we will see you less, firstly every other day and then moving to twice a week. We will support you to manage any prescribed medications. 7. Once you are recovering from your illness or crisis we will agree a discharge date with you. We will discharge or refer to the appropriate service who will continue to manage your care and treatment.

Sutton Home Treatment Team.indd 5 25/05/2016 17:04 Are HTT appointments confidential? We are a confidential service and only staff directly involved with your care will know your personal details. If we need to discuss anything with anyone outside our team, we will get your permission first. The only exception to this if we consider that your life or the well-being of someone else could be at risk.

Do HTTs work with other services? We work jointly with all the professionals involved in your care. Sometimes when discussing your care, we might identify a need that is not being met. In that case we can refer you to the appropriate team to help you with that particular issue.

What happens if I am in crisis outside of our working hours at night time?

If you need emergency support outside of our working hours you have the following options: In a life threatening emergency attend the nearest A&E. There are emergency psychiatric services based in A&E, which can help you. In a life threatening emergency dial 999.

If the problem is not immediately life-threatening you can call: Our mental health support line from 5pm to 9am Monday to Friday, 24 hours Saturday and Sunday 0800 028 8000.

However there are sometimes occasions where, despite our intensive input, things do not improve. In this happens, we will work closely with you and any family and/or friends you wish to have involved to make this a smooth transition.
