To put it simply, it is inflammation of your tendon. There are a couple forms of Achilles tendinitis, which are determined primarily by the area of the tendon that is experiencing inflammation. There are two common types. Noninsertional Achilles Tendinitis. Patients who are between the ages of 30 and 40 with an increased level of activity tend to suffer with Noninsertional Achilles tendinitis. Patients with noninsertional Achilles tendinitis are often treated with non-surgical therapy and are able to gradually increase activity. Insertional Achilles Tendinitis. When the area that the heel bone and Achilles tendon connects becomes painful with swelling, this is known as Insertional Achilles tendinitis. FALLET From the Department of Medicine, Division of Rheumatology, University Hospital, Geneva, Switzerland SUMMARY This study presents the frequency of severe and mild talalgias in unselected, consecutive patients with rheumatoid arthritis, ankylosing spondylitis, Reiter's syndrome, and generalized osteoarthosis. Achilles tendinitis and plantar fasciitis caused a severe talalgia and they were observed mainly in males with Reiter's syndrome or ankylosing spondylitis. On the other hand, sub-Achilles bursitis more frequently affected women with rheumatoid arthritis and rarely gave rise to severe talalgias. The simple calcaneal spur was associated with generalized osteoarthrosis and its frequency increased with age. This condition was not related to talalgias. Finally, clinical and radiological involvement of the subtalar and midtarsal joints were observed mainly in rheumatoid arthritis and occasionally caused apes valgoplanus. copyright. Tucker, MD, Drexel University College of Medicine, Philadelphia, Pennsylvania Plantar fasciitis is a common problem that one in 10 people will experience in their lifetime. Plantar fasciopathy is an appro- priate descriptor because the condition is not inflammatory. Risk factors include limited ankle dorsiflexion, increased body mass index, and standing for prolonged periods of time. Plantar fasciitis is common in runners but can also affect sedentary people. With proper treatment, 80% of patients with plantar fasciitis improve within 12 months. Plantar fasciitis is predominantly a clinical diagnosis. Symp- toms are stabbing, nonradiating pain first thing in the morning in the proximal medioplantar surface of the foot; the pain becomes worse at the end of the day. Physical examination findings are often limited to tenderness to palpation of the proximal plantar fascial insertion at the anteromedial calcaneus. Ultrasonogra- phy is a reasonable and inexpensive diagnostic tool for patients with pain that persists beyond three months despite treatment. Treatment should start with stretching of the plantar fascia, ice massage, and nonsteroidal anti-inflamma- tory drugs. Many standard treatments such as night splints and orthoses have not shown benefit over placebo. Recalcitrant plantar fasciitis can be treated with injections, extracorporeal shock wave therapy, or surgical procedures, although evidence is lacking. Endoscopic fasciotomy may be required in patients who continue to have pain that limits activity and function despite exhausting nonoperative treatment options. The cause is multifactorial and controversial, involving biological, anatomical, and mechanical factors. A variety of conditions characterized by Achilles tendon inflammation and/or degeneration can be clinically and histologically differentiated. These include in- sertional Achilles tendinopathy, retrocalcaneal bursitis, Achilles paratenonitis, Achilles tendinosis, and Achilles para- tenonitis with tendinosis. The mainstay of treatment for all of these diagnoses is nonoperative. There is a large body of evidence addressing treatment of acute and chronic Achilles tendon ruptures; however, controversy remains. Disorders of the Achilles Tendon
CHA PT ER 2 1 DISORDERS OF THE ACHILLES TENDON William D. Fishco, DPM A common podiatric complaint is pain in the region of the Achilles tendon. A careful examination and history taking helps decipher the condition(s), which ultimately may be inter-related. Rather than just call the diagnosis an Achilles tendinitis or a heel spur, which in part may be true, a straightforward examination, which will be illustrated, gives a more accurate diagnosis and ultimately an appropriate treatment protocol. The common diagnoses that affect the Achilles tendon include bone conditions such as a Haglund’s deformity Figure 1. Typical appearance of a Haglund’s deformity. Even though the treatment may be similar for these conditions, there are some notable exceptions. It is important to ask pertinent questions regarding the patient’s symptoms. First, one should determine whether there is an element of post-static dyskinesia, pain after rest and improvement (loosening) as one ambulates. If so, then it is likely that there is at least a condition of tendinitis. Secondly, it important to assess whether or not pressure to the back of the heel with a closed-in shoe is a source of pain. This would be consistent with a posterior heel spur, Haglund’s deformity, and/or distal Achilles tendinosis. Examination of the Achilles complex is performed first by visualization of both extremities. -

ANATOMY the Achilles Tendon Is a Strong Tendon That Connects the Calf
ACHILLES TENDON RUPTURE Dr. Abigail R. Hamilton, MD ANATOMY The Achilles tendon is a strong tendon that connects the calf muscles to the heel. Two muscles form the calf: the underlying soleus muscle and the thick outer gastrocnemius muscle. When they contract, they pull on the Achilles tendon causing your foot to point down (plantar flexion) and helping you raise up on your toes. This powerful muscle group helps when you sprint, jump, or climb. With aging and overuse, the Achilles tendon is subject to degeneration within the substance of the tendon. The term degeneration means that wear and tear occurs in the tendon over time and leads to a weakening of the tendon. Degeneration in a tendon usually shows up as a loss of the normal arrangement of the fibers of the tendon. Tendons are made up of strands of a material called collagen (think of a tendon as similar to a nylon rope with the strands of collagen being the nylon strands). Some of the individual strands of the tendon become jumbled due to the degeneration, other fibers break, and the tendon loses strength. The healing process in the tendon can cause the tendon to become thickened as scar tissue tries to repair the tendon. This process can continue to the extent that a nodule forms within the tendon. This condition is called tendinosis. The area of tendinosis in the tendon is weaker than normal tendon and is sometimes painful. Spontaneous rupture of the Achilles tendon can occur in patients in their third to fifth decade. Rupture is more common in men than women and most injuries occur during sporting activities. -

Your Achilles tendon is an important part of your leg. It is found just behind and above your heel. It joins your heel bone (calcaneum) to your calf muscles. The function of your Achilles tendon is to help in bending your foot downwards at the ankle: this movement is called plantar flexion. Insertional Achilles tendinopathy, tendinopathy up to 2cm from the Achilles tendon insertion to the calcaneous/heel bone is affected. This leaflet describes both conditions and their treatment options. Achilles tendinopathy is a condition that causes pain, swelling and stiffness of the Achilles tendon. It is thought to be caused by repeated tiny injuries (known as micro-trauma) to the Achilles tendon. After each injury, the tendon does not heal completely, as should normally happen. This means that over time, damage to the Achilles tendon builds up and Achilles tendinopathy can develop. There are a number of things that may lead to these repeated tiny injuries to the Achilles tendon, for example: • Overuse of the Achilles tendon. This can be a problem for people who run regularly.
