Swamp Dog Agility Club will follow CDC guidelines in place at the time of our event regarding COVID-19. Be aware that State and Local guidelines may be more stringent. At the time this premium is published, there are no restrictions for masks, social distancing nor limits to indoor gatherings. Any changes to these guidelines will be announced in the confirmation, judging program, and/ or be posted by social media. By signing below, exhibitors agree to abide by these guidelines.

The Swamp Dog Agility Club of Greater New Orleans (SDAC) cannot guarantee that exhibitors will not contract COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected with COVID-19 by attending this event and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
