These include services provided by health visitors, social workers, district nurses, school nurses, community physiotherapists, dieticians and podiatrists (foot care specialists). The Partnership will also be responsible for local learning disability, mental health and addiction services in the South West of the city and will work closely with local GPs, pharmacists, dentists and opticians. How many CHPs are being developed across Greater Glasgow? Nine CHPs are being developed across Greater Glasgow. Five of these are within the boundaries of Glasgow City Council, including one covering the South West of the city. The new South West Glasgow Community Heath and Social Care Partnership covers the South West of the city including the following communities: , , , , , , , , West , and . Although the Thornliebank Health Centre lies within the boundaries of the new East Community Health and Care Partnership, as the majority of patients registered with GPs in this centre live in Glasgow, local health services within this centre will also be managed by the South West Glasgow Community Health and Social Care Partnership.

The present boundaries for the Partnership may be altered slightly to reflect future changes to Glasgow Community Planning Partnership boundaries and Glasgow City Council electoral ward boundaries. What difference will this new Partnership make to patients? The South West Glasgow Community Health and Social Care Partnership will develop closer partnerships with local council and hospital based health services to break down some of the barriers that have traditionally existed between community health, social care and specialist health services. This will create new opportunities to develop additional health services at a local level. It will also make it easier for you to access the local services you require regardless of whether these are provided by your CHSCP, local hospital or local council. How will I access local health and social care services in the future? Local health and social care services will continue to be provided in local health centres, clinics and social work offices across South West Glasgow. How will the new Partnership be developed? A draft ‘Scheme of Establishment’, which sets out the broad principles for the development of the five new Glasgow CHSCPs, including details of the services they will manage and how these will be organised, has been submitted to the Scottish Executive for final approval. A local Development Group has also been formed to develop the new South West Glasgow Community Health and Social Care Partnership over the next year, working closely with local staff, community groups and others with an interest in the development of local health and social care services.

How can I get involved in the development of local health and social care services? All CHPs are required to develop a Public Partnership Forum (PPF) by 1 April 2006. This will give service users, carers, local community groups and voluntary organisations an opportunity to get involved in the development of local services and provide feedback on a wide range of local issues. The PPF will build on existing local patient and public involvement arrangements to develop closer links with community groups and voluntary organisations across South West Glasgow.
