Vascular response Loss of function 2. Cellular response สูญเสียหนาที่การทํางาน (Migration and activation of leukocytes) (functio laesa)
3. Systemic response Suffix –itis : Appendicitis, Myocarditis Otitis, Neuritis, etc.
1 Vascular and cellular reactions are mediated Inflammatory Cells by chemical factors, derived from plasma proteins or cells (Cytokines) and are Neutrophil produced in response to or activated by the Macrophage stimuli Lymphocyte Plasma cells Inflammation is divided into – Acute inflammation – Chronic inflammation
Neutrophils
เปนเซลลอักเสบ (inflammatory cells) ที่พบเปนสวนมากในการอักเสบ แบบเฉียบพลัน (Acute inflammation) โดยเฉพาะในชวง 6 -24 ชั่วโมง แรก จะมีการกระตุน และเพิ่มจํานวน มาในบริเวณที่เนื้อเยื่อเกิดการ บาดเจ็บ หรือการติดเชื้อ เปนเซลลเม็ดเลือดขาว (Leukocytes) ชนิดที่มีแกรนูลจําเพาะ (specific granules) มีขนาดเสนผานศูนยกลางประมาณ 10-15 μm หรือขนาดประมาณ 2 เทาของเม็ดเลือดแดง นิวเคลียสมีประมาณ 2-5 Neutrophil lobes โครมาตินติดสีเขมทึบ specific granule มีขนาดเล็ก ละเอียด ติด สีชมพูอมมวง เรียกวา Neutophilic granules

2 Neutrophils in Tissue Macrophages
Macrophages (หรือเซลลที่เรียกวา monocytes ในขณะที่อยูในกระแสเลือด) เปนเซลลอักเสบ (inflammatory cells) ที่พบมากในการอักเสบแบบเรื้อรัง (chronic inflammation) โดยจะเริ่มเขามามีบทบาทในเนื้อเยื่อใน 24-48 ชั่วโมง ภายหลังจากการบาดเจ็บ ในเนื้อเยื่อ จะเห็น macrophages เปนเซลลขนาดใหญ มีนิวเคลียสเดี่ยว ภายใน cytoplasm จะประกอบดวยเศษซากเซลลที่ macrophages เก็บกินดวยวิธี phagocytosis Macrophages มีชื่อเรียกแตกตางกันตามชนิดของเนื้อเยื่อที่พบ เชน Kupffer cells ในเนื้อเยื่อตับ, alveolar macrophages ในเนื้อเยื่อปอด, microglial cells ในเนื้อเยื่อระบบประสาทสวนกลาง
Macrophages Lymphocytes
เปนเซลลอักเสบที่มีบทบาทในการอักเสบชนิดเรื้อรัง (chronic inflammation) และในระบบภูมิคุมกัน (immune system) โดยจะมีการ สงสัญญาณกระตุนซึ่งกันและกัน ระหวาง lymphocytes และ macrophages Small lymphocytes จะมีขนาดใหญกวาเม็ดเลือดแดงเล็กนอย นิวเคลียสมีขนาดใหญเกือบเต็มเซลล โครมาตินหยาบ ติดสีเขม และ เรียงตัวชิดติดกันแนน เซลลมี cytoplasm สีฟาออนใส ปริมาณนอยจน Monocyte in บางครั้งแทบมองไมเห็น cytoplasm ได blood smear
3 Lymphocytes Plasma cells
เปนเซลลที่พัฒนาจาก activated B lymphocytes ใหสามารถสราง และ หลั่ง antibody ออกมาจับกับ antigen ในเนื้อเยื่อที่เกิดกระบวนการ อักเสบ โดยเฉพาะในกระบวนการอักเสบแบบเรื้อรัง Plasma cells เปนเซลลที่มีขนาดประมาณ 2-3 เทาของเม็ดเลือดแดง นิวเคลียสรูปกลม หรือรูปไข คอนไปขางใดขางหนึ่งของเซลล (Eccentric nucleus) โครมาตินหยาบเปนปน พบลักษณะนิวเคลียสคลายซี่ลอเกวียน (cartwheel nucleus) cytoplasm มีปริมาณมาก ติดสีฟา และเห็น ชองวางขาวติดกับนิวเคลียส (perinuclear halo)

Rapid response to an injurious agent that serves to deliver mediators of host defense (leukocytes and plasma proteins) to the site of injury
4 Three majors components: Stimuli for Acute inflammation
1. Alterations on vascular caliber that lead Infections and microbial toxins to an increase in blood flow Trauma 2. Increase vascular permeability due to Physical and chemical agents vascular structural change that permits Tissue necrosis fluid, plasma protein (fibrin, complement) and leukocytes to leave the circulation Foreign bodies Immune reactions 3. Emigration of the leukocytes from the microcirculation to the stimulated site
Vascular Changes
Changes in vascular flow and caliber – Brief vasoconstriction and Vasodilation – Earliest manifestation of acute inflammation – Stasis Æ increased blood viscosity – Quickly followed by increased vascular permeability
Increased vascular permeability – Hallmark of acute inflammation – Protein and fluid leakage from the vascular lumen to the interstitial space
5 Extravascular fluid
Exudate : high protein concentration, turbid
Transudate : low protein concentration, clear
Pus or purulent exudate (หนอง) : leukocytes-rich exudate with cellular debris

6 CHEMICAL MEDIATORS OF INFLAMMATION Chemical mediators
Vasoactive amines: Histamine Mediators originate either from plasma or from cells Plasma proteins: complement system, The production is triggers by microbial products or by host proteins, or by other chemical mediators kinin system, clotting and fibrinolytic system Mediators perform activity by binding their specific Arachidonic acid metabolites: prostaglandins, receptors (specificity) leukotrienes, thromboxanes One mediator can stimulate the release of other mediators (cascade) Cytokines and chemokines: Interleukin-1(IL-1), Mediators have different effects on different cell Tumor necrotic factor (TNF) types Nitric oxide (NO) Most mediators are short-lived
Effects of Chemical mediators
Pain: Prostaglandin, bradykinin Increased vascular permeability, edema Vasodilation: histamine, NO Fever, acute phase symptoms: IL-1, TNF Tissue damage Chemotaxis, leukocyte recruitment and activation

8 Skin Abscess Abscess (ฝ)
Focal collection of pus (consisting of neutrophils, necrotic cells, and edema fluid)
Abscess has a central, largely necrotic region rimmed by a layer of preserved neutrophils, and completely walled off by connective tissue Lung abscess

9 CHRONIC INFLAMMATION
Inflammation of prolonged duration (weeks or months) in which active inflammation, tissue destruction, and attempts at repair are proceeding simultaneously.

Infiltration with mononuclear cells, including Persistent infection macrophages, lymphocytes and plasma cells (Acute inflammation = Neutrophils) Prolonged exposure to potentially toxic agents, either exogenous or endogenous Tissue destruction
