CLINICAL ACTIVITIES

CLINICAL ACTIVITIES SUMMARY My main clinical activity is skin biopsy interpretation, both of specimens sent directly to our laboratory and of slides sent in consultation. These include biopsy specimens from UCSF physicians, but are mostly from community dermatologists and slides from community dermatologists, pathologists and dermatopathologists, both in the United States and abroad. In the last few years, I have on average reported on approximately 22,000 cases of all types. I also founded, and co-direct the UCSF Dermatopathology Service, which is the clinical unit via which this extramural material reaches UCSF. Among the cases that I signed out last academic year are approximately 6,000 consultations. These come from Asia, Europe, Canada and South America, and include many third, fourth or fifth opinions, from pathology and dermatopathology laboratories, both private and academic.

SERVICE ACTIVITIES SUMMARY My colleagues and I sign-out 60-80 specimens/week from Dermatology Clinic at UCSF. We, along with other Dermatopathology faculty, present (or supervise presentation by the dermatopathology fellow) all pathology for the UCSF Dermatology Department staff conferences (10 hrs/year).

In 1986 and 1987, in collaboration with the Department Charis of Pathology and Dermatology, I organized the UCSF Dermatopathology Service, a unit located in the Dermatology clinical area at 1701 Divisadero Street. The service opened for business in February, 1987. The service picks up and processes skin biopsies taken by community dermatologists, examines them, and issue reports. Our workload includes consultations from dermatologists who interpret biopsy specimens themselves, but request second opinions from us (about 12,000 cases per year). We examine all of this material with residents from both the Dermatology and pathology departments, the dermatopathology fellows, visiting fellows, and post-doctoral scholars at the multiheaded microscope. I presently serve as a co-director of the Service (my colleague, Timothy McCalmont became the other co-directory in 1994), which reported on over 145,00

Our service is the largest dermatopathology operation in the western United States, and we believe it is the largest university based dermapathology unit in the world. We have achieved this benchmark despite practicing in an environment in which many dermatologists interpret biopsies themselves.

TEACHING AND MENTORING

TEACHING SUMMARY Medical Student Teaching Several medical students in their fourth year of training, have chosen to spend a month with me as a preceptor in dermatopathology. This elective appears on their schedules as a generic "Pathology" elec­tive. Medical students who are post-sophomore fellows in pathology have also rotated through dermatopathology on many occasions (see list of Pathology Clerkships). Several have done projects that resulted in publications. Resident Teaching As a dermatopathologist who does not practice general pathol­ogy, my expertise is best utilized in postgraduate train­ing. One half day per week is spent on the multi-headed microscope with path­ology and dermatology residents. Although I sign-out a large volume of biopsy slides, I try to find a teaching point in each one, and try to engage pathology and

30 of 55 Prepared: December 1, 2020 dermatology residents in an approach to diagnosis that can serve them outside of dermatopathology. Dermatopathology is an elective in the Pathology residency program. Der­matopathology is an integral part of the residency program in dermatology, and training in it is indeed required by the American Board of Dermatology for certifi­ca­tion. Derma­tology residents thus have three full months of training at the microscope during their residencies. We have visiting dermatol­ogy and pathology residents from other training programs on a regular basis, and many of them have gone on to careers in dermatopathology. Aside from teaching at the microscope, the dermatopathology faculty provide a comprehensive formal lecture series (30- 40 one hour sessions/year), covering the important topics in the field. These are attended by all of the pathology and dermatology residents, as well as some rotating students and fellows. I deliver eight such lectures per year. Fellowship Teaching The subspecialty of dermatopathology is recognized by a cer­tifi­cate of special competence, issued jointly by the American Board of Path­ology and the American Board of Dermatology. The ex­amination for the certificate is only open to graduates of accred­ited fellowship pro­grams, of which there are only approximately 35 currently accepting can­didates in the United States. The fellow­ship is open to board certified or eligible graduates of either Pathology or Dermatology resi­dencies and is for either one or two years (the board requirement is one year). Half of the required year is spent training in the opposite discipline (i.e., pathology for a gradu­ate of dermatol­ogy, dermatology for a pathology graduate) and half in der­matopathology. As program Director, I initiated our ACGME-accred­ited fellowship in 1989. We have now trained over thirty fellows, and all have passed the certifying exam. The American Board of Pathology asked me in 1992 to serve on their Test Commit­tee for derma­to­pathology. This commit­tee writes questions for both the Anatomic Pathology Board exami­nation and for the examination for the certificate of special com­petence in dermatopathology. I served on the committee from 1992 through 1994. I was director of the fellowship program at UCSF, from its foundation until 2010. Graduates of our fellowship program have gone on to positions in both academic departments and in private practice. Directors of dermatopathology among our graduates include Dr. J. Andrew Carlson (Albany Medical College) and Brian Hinds (Univ. of California, San Diego) and graduates serve on the faculty at Yale (Shawn Cowper), UCSF (Drs. Timothy McCalmont, Laura Pincus, Iwei Yeh and Jeffrey North), and PAMF (Dr. Beth Ruben)

MENTORING SUMMARY I typically work closely with our dermatopathology fellows, both in our ACGME accredited year, and in the second year of their fellowship (non-ACGME year). This includes having the fellows preview my consultation cases, and present their opinions to me, as well as looking at the pathology reports “mocked up” by our fellows on routine cases. I have worked with our fellows both toward platform and poster presentations at the American Society of Dermatopathology meeting, and in peer reviewed publications. With our visiting scholars (dermatologists and pathologists from other countries who come to UCSF from between several weeks to a year), I have worked to develop many peer reviewed publications. -
