1. I N T R O D U C T I O N Family Welfare Training and Research Centre, ( F.W.T.& R.C.) , was the first Family Planning Training Centre, established in June 1957 Under Union Ministry of Health and Family Welfare at “Parekh Hospital ” at 332, S.V.P. Road, Khetwadi, Mumbai – 400 004.

After the inception of other Central Training Institutes (C.T.I.s) in other parts of the country, the Ministry of Health & Family Welfare allotted certain states to each C.T.I. for the purpose of In-service Training responsibilities and divided the country into six zones, as shown in the map. Family Welfare Training & Research Centre, Mumbai is made responsible to fulfill the training needs of state and district level health personnel of the Western Zones of , which includes the states of Madhya Pradesh, Andhra Pradesh, Gujarat, Goa; and Union Territories of Daman, Diu and Dadra and Nagar Haveli.

In order to have maximum utilization and to prevent wastage of training effort, it has been decided by Rural Health Division, that the Central Training Institute can call for nominations of the candidates from the Health & Family Welfare Training Centres of all the states vide letter No. Z.22025/8/89-RHD dated 16th April 1990.

In December 1986, the Centre was approved by Rural Health Division, Ministry of Health & Family Welfare, New Delhi for conducting a one year Diploma course in Health Education.

Family Welfare Training & Research Centre is also conducting community based Research Projects in the field of health and family welfare, AIDS & RCH. The Ministry of Health & Family Welfare, New Delhi identified FWTRC as a collaborative Training Institute to carry out WHO Fellowship Programmes.

FWTRC is collaborating Training Institute for conducting Training of Mid-Level District Level Manager under Immunization Strengthening Project (RCH) for district level officers from Western Zone of India.

2. O B J E C T I V E S O F T H E C E N T R E

1. To organise and conduct the one-year course of “Diploma in Health Promotion Education” for sponsored candidates from India & abroad. 2. To organise Training of Trainers in key health area. 3. To impart training in Primary Health Care, Family Planning & other integrated National Health Programmes to different categories of Health personnel, deputed from the states in the Western Zone of India. 4. To supervise and guide the students from other institutes, placed for field placement training with the centre. 5. To provide technical guidance and training to the staff of Health & Family Welfare Training Centres & Rural Training Centres of Western Zone of India, which are under the jurisdiction of the Family Welfare Training & Research Centre, Mumbai. 6. To carry out experimental training course for District level staff whenever required by the Training Division, Govt. of India, Ministry of Health & Family Welfare. 7. To arrange educational programmes for workers of Voluntary Agencies, worker groups and general public in different Health and Family Welfare aspects. 8. To organise programmes on population, family life and and education for awareness in AIDS for various colleges, schools, voluntary agencies, out of school youths in slums & for other organisations, on request. 9. To provide maternal and child health, family planning and other welfare services to people through the urban clinic. 10. To carry out research studies related to training and education in the Health & Family Welfare Sector. 11. To carry out Community Based Research in the field of Health & Family Welfare, AIDS, Reproductive & Child Health. 12. To organise out reach activity and health camps to provide preventive, promotive & curative services to remote areas.

3. TRAINING ACTIVITIES AT FWTRC 3.1DIPLOMA IN HEALTH PROMOTION EDUCATION Family Welfare Training & Research Centre, Mumbai was approved for conducting one year Diploma course in Health Education in December 1986, by Rural Health Division of Ministry of Health and Family Welfare, Govt. of India under scheme of 'Development of National Trained Health Manpower' for the training of specialists and para-medical workers. The Diploma in Health Education Course is affiliated to the Deemed University of International Institute for Population Sciences (I.I.P.S.), Mumbai - 400088 The training programme is action-oriented with an in-built concurrent field-work spread over the whole academic year. Thus, the participants learn to identify their functions, tasks and roles and they are equipped with basic knowledge and skills to perform their jobs effectively by doing these jobs in the community placement in the field. The training programme is converted to two semister based prgramme divided into eight specific teaching units or areas since 1998-99. 3.1.1 OBJECTIVES : The objectives of the training are : General Objective : To develop Health Education Specialists needed to provide Effective Health Promotion and Education activities. Specific Objectives : At the end of the training course, the student are able to -

1. Relate the basic health sciences and apply this knowledge in Health Promotion & Education. 2. Apply the knowledge of Behavioural Sciences with Community Organization and Community Development in bringing about desirable changes in the health behaviour of the community and to make Health Promotion and Education Programmes in the Community Programmes. 3. Apply simple statistical methods and demographic techniques in his/her present Job situation for programme planning and for conducting surveys and research studies related to health. 4. Perform the role of Health Education Specialist in the community conducting Health Promotion and Education Programmes effectively by proper application of concepts, principles, objectives and approaches of Health Promotion and Education. 5. Explain Health Policy, Health Care Delivery System, Development skills in Public Health Administration for effective implementation of IEC component of National Health Programmes, along with Health Promotion and Education activities. 6. Become an effective Communicator, for advocacy in Health Promotion and Education in the Community. 7. Plan, implement and evaluate Health Promotion and Education Programmes in community by positive

and constructive attitudes on the identified health problems.

Keeping in mind, the curriculum was revised by this Centre and was approved by the Academic Council of I.I.P.S. Deemed University, Mumbai.

During the year 2004-2005 a final selection of 22 sponsored candidates was made. Five candidates from state of , fifteen candidates from state of Andhra Pradesh, one candidate from state of Orissa and one candidate from Maldives joined and continued the DHPE course. 3.1.2 TRAINING METHODOLOGY :

A. Audio-visual workshop (D.H.P.E.) Training is imparted through class room sessions, laboratory session and observational visits. Three workshops were organised to develop special skills in preparation of A.V.aids. Senior Artist conducted these workshops. Trainees prepared useful audio-visual aids, which were utilised in the field areas for the various health education activities.

B. CONCURRENT FIELD-WORK :

The trainees worked in the field area through-out the course. During the concurrent field work, they could simultaneously apply the knowledge gained in the class rooms to actual field situation and could learn by doing.

Trainees were divided into three groups and placed in a slum community of 5000 population each, close to the Centre. In this community, they identified the health problems and planned, implemented and evaluated a health education programme in reference to a selected problem over a period of time.

The urban field areas were chosen before the course began and three Group Guides were made responsible to guide the students in the field and also in their day-to-day work, with a ratio of 5 to 6 students per group guide. Faculty members who are experienced in Public Health guided the students in the field . This was carried out in a phased manner over a period of time. The health problems selected for health promotion education activities in the community were- • ARI • Oral Health • Malaria

Extra stress was laid on the above health problems identified in the community for health promotion education programme during concurrent field training. The group organised various health promotion education programmes including exhibition on health problems, their prevention and treatment in the field area. Health check-up camps were arranged.

Group talks, demonstrations and return demonstrations were organised on the health problems, its prevention and treatment in order to make it more effective and easy to understand.

C. FIELD-TRIP :

A study tour for the Diploma in Health Promotion Education trainees was organised from 18/12/2004 to 24/12/2004 to observe some Voluntary Health Projects and some Government Organisations in Maharashtra. The following Projects were visited 1. Health & Family Welfare Training Centre, Pune 2. IEC Bureau, Pune 3. State Family Welfare Bureau, Pune 4. Vadu Health Project, Pune 5. Jyotsna Arogya Prabhodhan, Dehu

An observational visit was planned for the students to visit the following institutions involved in the field of Health Education, in the month of February 2004 1. National Institute of Health & Family Welfare, New Delhi 2. Central Health Education Bureau, New Delhi 3. Indian Institute of Mass Communication, New Delhi 4. World Health Organization (SEARO), New Delhi 5. Voluntary Health Association of India, New Delhi 6. Head-quarters of Union Ministry of Health & F.W. at Nirman Bhavan, New Delhi.

D. SUPERVISED FIELD TRAINING (SFT) The annual examination was held in mid-February and the trainees were placed in the field for Supervised Field Training for two months to apply their knowledge and skills gained during the academic session. Their postings were given as below :-

During SFT, DHPE trainees work in a community, either rural or urban. Based on the community diagnosis, the trainees plan, implement and evaluate a health promotion education programme under the supervision of an experienced guide from respective HFWTC/DTC of various states where the trainees are placed. After completion of work, each trainee submits a report of his / her work to F.W.T.&R.C., Mumbai. D. Follow-up Visits And State Visits

The Faculty members visited the Health & Family Welfare Training Centres of Pune, Aurangabad, Hyderabad, Vishakhapatnam, Kurnool, Warangal, & State Institute of Health & Family Welfare, Orissa during this year in supervised field training of DHPE students, Faculty members assessed training need assessment of the various categories of health personal. 3.1.3. URBAN FIELD PRACTICE AREA

The Centre has selected some of the urban slums of Mumbai city in and around Khetwadi as field practice areas. These are utilized for providing practical training to different categories of health professionals at F.W.T.&R. Centre, Mumbai.

The faculty members regularly visit the area as a part of the field development. During the year 2004-2005, work was done BIT Chawl (Agripada), BIT Chawl (Chandanwadi – Marine Lines), Ambedkar Nagar (Tardeo).

These slums are utilised for concurrent field work of D.H.P.E. trainees. A sample survey was conducted. The health problems identified were ARI, Oral Health, Malaria. As per the need of the community, health exhibition, slide show, group discussion and health camps were organised on prioritized health problems. The resources from Bombay Municipal Corporation were utilised for service camps. Community leaders were involved in organising such programmes. Same Field practice demonstration areas are utilised for the other training programmes.

3.2 TRAINING PROGRAMMES :

Family Welfare Training & Research Centre, Mumbai was the first Family Planning Training Centre under the Union Ministry of Health & Family Welfare, New Delhi conducting the various training courses for the different categories of Health Personnel in the key health area to deliver the better health care services.

3.2.1 TRAINING COURSES SPONSORED BY MINISTRY OF HEALTH & FAMILY WELFARE & NIHFW, NEW DELHI. Ministry of Health & Family Welfare, New Delhi & National Institute of Health & Family Welfare, New Delhi identified FWTRC as the collaborating Training Institute for conducting Training of Mid-level managers under Immunization Strengthening Project (R.C.H.).

This project is funded by World Health Organisation. Mid Level Managers included CMO, CHMO, DHO, ADHO, RMO, Faculty of HFWTCs and medical colleges from the states of Maharashtra, Madhya Pradesh, Chattisgarh, Gujarat, Goa, Daman & Diu, Dadra Nagar Haveli. Under this project, 349 mid level managers were trained during 2003-2004. There was only 44% (60/135) target achievement from Madhya Pradesh. Hence it was decided to organise two extramural training programmes for mid level managers from Madhya Pradesh only at State Institute of Health & Family Welfare, Gwalior to facilitate the participation of mid level managers so that the training target could be achieved. During 2004-05, a total of two Extramural Training Programmes were conducted for the states of Madhya Pradesh on 14/6/04 to 19/6/04 and 21/6/04 to 26/6/04. It facilitated to train twenty two more mid level managers. Thus, a total of 371 mid level managers were trained against the target of 399 under this project. Dr. S. D. Khaparde, Director, Dr. Nishikant Rahate, Dy. Director and Mrs. Rosy Joseph, PHN were facilitators from FWTRC.

3.2.2 WORKSHOPS ORGANISED BY F.W.T&R.C., MUMBAI

Grading system for evaluation of Diploma in Health Promotion Education course

One day workshop was conducted on “Direct Grading System of Evaluation” of Diploma in Health Promotion Education (DHPE) a one year course affiliated to International Institute for Population Sciences, Deemed University, Deonar, Mumbai.

Objective of the workshop were to adopt Direct Grading of Evaluation of DHPE as per the guidelines laid down by University Grants Commission (UGC), New Delhi and to orient the faculty members from FWTRC and also the visiting faculties of other institutes and medical colleges who are regularly involved in the teaching and the evaluation of DHPE course.

The workshop was conducted on 5th July 2004 at FWTRC, Mumbai.

Oration programme on T.B.

Oration programme on “Changes to Global Tuberculosis control – Role and efforts of FIDELIS” organised by F.W.T&R.C, Mumbai at H. N. Hospital Mumbai on 7th July 2004. The oration was delivered by Dr. Khatri and attended by the fifty five participants from FWTRC & various Medical colleges in Mumbai.

The objective of the programme :

To orient the participants towards new trends in the control of Tuberculosis.

The main objective of the Urban Health programme is to provide an integrated and sustainable system for primary health care service delivery, with emphasis on improved Family Planning and Child Health Services in the urban area of the country for urban poor living in slums.

With this objective F.W.T&R.C., Mumbai organised a Seminar to provide a platform where public and private stakeholders viz. State Government, BrihanMumbai Municipal Corporation (B.M.C.), NGOs, Corporate houses and International Agencies could share their experiences and views towards effective and sustainable urban health care needs.

A one day seminar on urban RCH with focus on public-private partnerships was conducted by FWTRC on 11th August 2004 at Hotel Heritage Comfort Inn Byculla, Mumbai. There was an even representation from both private and public sectors with participants coming from various organisations like Municipal Medical Colleges, NGOs, Mubai Port Trust, Railways, Central and state Government. The resource persons discussed the different aspects as follows:

Dr. S. D. Khaparde, Director, FWTRC overview of RCH with emphasis on proposed strategies under urban RCH-II for primary health care, two-tier service delivery model, stakeholders for urban RCH, Dr. Shanta Sankarnarayan, Professor, T. N. Medical college gave a concise listing of Health delivery system and the issues. Dr. Radha Aras, Professor and Head of Dept. of PSM, T. N. Medical College highlighted the role and contributions of medical college in effective delivery of service in Urban RCH.

A panel discusssion on role of various agencies in Urban RCH was organised. The panelists were Dr. A shok Ladha, Dy. Director, Health Servvices, Dr. Prakash Deo, State programme officer for the state of Maharashtra and Kerala UNFPA, Dr. Mahinder Watsa, President FPAI, Dr. Santosh Datar, Senior Medical Officer, Larsen & Toubro. The session was chaired by Dr. G. B. Daver, ex-dean J. J. Hospital and present Medical Director, Professional Services, P. Hinduja Hospital, Mumbai. The discussion brought forth role of various stakeholders, and also discussed in detail the current perspective and issues which needs to be addressed to develop an effective service delivery model for urban RCH.

3.2.3 AD-HOC TRAINING COURSES

4. RESEARCH: RESEARCH & CONFERENCE 4.1. FWTRC, Mumbai was Collaborative Institute with Department of PSM Government Medical College, Nagpur for Mid term evaluation. Action Research Project for “Enhancing eligibility Couple Protection Rate in Tribal blocks of UNFPA assisted IPD Project district in Maharashtra State”. Dr. Nishikant Rahate, Investigator, Mr. Sanjay Bhonge, Research Associate participated for Data collection from Nandurbar district during 16-21 November 2004 and in Thane district during 29th November to 2nd December 2004.

4.2. In 32nd Annual National Conference of IAPSM and 12th Annual conference of MCIAPSM organized by Dept. of Community Medicine, Rural Medical College, Pravara Institute of Medical Sciences, Loni, Ahmednagar, the following papers were presented during 17-19 February 2005.

4.3. Dr. Nishikant Rahate attended the Protocol Development workshop for midterm evaluation from 24/10/04 to 25/10/04 at Health & Family Welfare Training Centre, Nagpur.

4.4. Dr. Nishikant Rahate, Dy. Director, F.W.T&R.C.Mumbai was involved as External Monitor for NPSP for PPI and IPPI in Chandrapur district during 17/10/04 to 23/10/04.

4.5. Annual Sentinel Surveillance of HIV Infection and STD for Maharashtra. National Institute of Health & Family Welfare, New Delhi, the Nodal centre to undertake Annual Sentinel Surveillance of HIV Infection and STD in India, identified FWTRC as Regional co-ordinting Centre for the Sentinel Surveillance activities in Maharashtra. The surveillance of HIV infection and STD is under taken established at Sentinel surveillance an sero-surveillance every year in the month of August-November to assess and to disease found in India, to project the estimates and to decide the strategies for prevention and control of HIV infection and STDs. Dr. S.D. Khaparde, Director, is the Regional Co-ordinator to supervise these activities during Annual Sentinel Surveillance. He has visited the different sentinel and sero-surveillance centres of Maharashtra and submitted the report to N.I.H.F.W., New Delhi & NACO, New Delhi for their action.

LIBRARY

The Library of F.W.T.&R.C., Mumbai was established in 1957. The primary mission of the library is to support the educational and research programmes of the institute by providing physical and intellectual access to information, consistent with the present and the anticipated educational and research functions of the institute. Library is having collection of 5003 books, 500 bound periodicals, 1540 reprints & DHPE Project reports. The library received 76 periodicals annually by the way of subscription as well as gift.

The books are mainly on the following subjects : Epidemiology, Nutrition, Public Health, Occupational Health, Communicable diseases, Sociology, Psychology, Community Organisation, Behavioural Sciences, Statistics, Demography, Population education, Non Communicable diseases, Sex Education, National Health Policies, National Health programmes, Family Welfare, Health Education, Communication, Hindi Books, Admin Books, Manuals, NFHS books, WHO books.
