A listing of healthy Lifestyle programs and activities available in Baw Baw to support client referrals

Cardiovascular disease (CVD), mainly heart attack and stroke, is the leading cause of death in . In Baw Baw, between 25-30% of the adult community have CVD. The rates of Type 2 Diabetes in Baw Baw are also high and are among the highest in the state.

This document has been compiled to provide health professionals with an overview of the range of programs and activities available in Baw Baw that could assist people to adopt healthier lifestyles. It was compiled following surveys and web‐searches of service providers within Baw Baw and provides basic details and contact information for a wide range of programs– from intensive, structured interventions to casual, low‐impact exercise opportunities.

Whilst every effort has been made to capture all the healthy lifestyle programs and services on offer in Baw Baw, it is possible that there may be programs we were unaware of. Services offered through general practice have not been captured. As the document is reflective of a point in time, program information may change, new programs may be established and some may conclude.

Health professionals from a range of disciplines are well placed to assess their clients’ risk of CVD and Type 2 diabetes and to refer clients to programs and services that will help them to reduce their risk. We hope you find this document to be a useful resource of referral options. Risk assessments

We encourage GPs or health care professionals to discuss CVD and Type 2 diabetes risk with their patients/clients, and to conduct risk assessments where appropriate. There are two main risk assessment tools:

For those aged 18 years and over the risk of Type 2 diabetes can be assessed using the AUSDRISK tool and a Body Mass Index (BMI) assessment · Those aged 45 years and over, or 35 years and over of Aboriginal and/or Torres Strait Islander descent, without a history of heart disease or stroke, can have their CVD risk assessed using the Absolute CVD Risk Tool Lifestyle Modification Programs

A wide range of programs and activities are available to assist people across Baw Baw to adopt healthier lifestyles. Many organisations provide lifestyle modification programs within Baw Baw, with programs differing by type, length, focus and the method of engagement. They include programs to assist people to lead a healthier lifestyle in respect to their weight, diet, smoking status and level of physical activity.

We have tried to capture the full range of programs/activities available across Baw Baw, with an emphasis on programs most likely to reduce the risk of CVD and Type 2 diabetes.

Please note that we are not recommending one program over another. With such a diverse range of healthy lifestyle programs on offer, our hope is that health professionals will assist community members find an activity that best suits them.

For the purposes of this document, the programs have been classified into two groups:

1 Structured lifestyle modification programs

These programs are multi-faceted in nature and are evidence‐based. They are prevention’ programs that tend to involve participants setting individual goals and are conducted over several weeks by a qualified instructor. The programs include structured group programs and individual coaching and have an emphasis on physical activity, healthy eating, healthy weight management and smoking cessation.

2 General healthy lifestyle programs

These are the less structured programs and activities. They include activities such as walking groups, exercise groups, support groups and gyms. Selection criteria and evidence base of programs

At a minimum the programs listed in this document met at least one of the following criteria:

The program’s objectives are consistent with/contribute to one or more of the overarching ‘Heathy Living Programs and Strategies’ aims to:

Increase healthy eating, physical activity and/or healthy weight; and decrease smoking and/or harmful/hazardous consumption of alcohol (for adults)

by targeting groups or communities and providing one or more of the following:

information or education increasing self-efficacy and health literacy; practical skills development; participation opportunities, contributing to increased healthy eating, physical activity and/or healthy weight; programs to strengthen community action / create supportive environments All programs have been assessed against the evidence and rated as either ‘partially’ or ‘completely’ meeting the criteria. The rating is based on whether they met some (partially) or all (completely) of the evidence based criteria listed below:

Meets Healthy Food Charter OR National Nutrition OR Physical Activity guidelines There is evidence of program effectiveness Cost effectiveness of the program has been considered (where available) There is evidence of adequate participation and sustained benefit The program is inclusive of identified priority population groups The program can be adapted to local circumstances and needs Participants don’t need to purchase specific foods or supplements (i.e. shakes) Participation costs or charges (if required) are reasonable for the service provided. Low income families / communities are not disadvantaged by cost Programs and strategies designed and/or promoted for weight loss meet the weight loss criteria below

Additional Weight Loss Program Criteria

Help people assess their weight and decide on a realistic healthy target weight (people should usually aim to lose 5–10% of their original weight) Aim for a maximum weekly weight loss of 0.5–1 kg Focus on long-term lifestyle changes rather than a short-term, quick-fix approach Are multi-component, addressing both diet and activity, and offering a variety of approaches Use a balanced, healthy-eating approach Recommend regular physical activity and offering practical, safe advice about being more active Include some behaviour change techniques, such as keeping a diary and advice on how to cope with ‘lapses’ and ‘high-risk’ situations Recommend and/or provide ongoing support

The following programs are designed to support participants to make lifestyle changes that will improve their general health and wellbeing and reduce their risk of Type 2 Diabetes and Cardio Vascular Disease (CVD). Programs and/or elements of the programs are based on evidence and best practice as described in the previous section. All Healthy Lifestyle Modification programs listed in this section ‘completely’ met the evidence based selection criteria.

If referrals are made to West Gippsland Healthcare Group the client will undergo their comprehensive intake process where they will have an Initial Needs Identification completed. Through this identification and screening process if it is deemed that the client is not eligible for the referred service a more appropriate service will instead be offered to the client.

Service Providers can also be searched for on the National Health Service Directory (NHSD).

Please note that there are several secondary prevention programs in operation across Baw Baw, for people who have been diagnosed with health disease or Type 2 diabetes. They include Cardiac and Pulmonary Rehabilitation programs conducted at West Gippsland Hospital and Latrobe Community Health Service, as well as programs and support offered by diabetes educators, dietitians and a range of other health professionals.

The following programs and activities are ‘general’ in nature and include casual activities to increase physical activity and fitness and promote healthy eating. All programs listed in this section are available via ‘self-referral’. All programs listed in this section ‘partially’ meet the evidence based selection criteria, with the exception of the heart foundation walking groups which ‘completely’ meets the selection criteria.
