INTRODUCTION

The Trust

The Hull University Teaching Hospitals NHS Trust has an annual turnover of circa £620 million, employs over 8,000 staff and serves an extended population of 1.2 million.The post will be based in the Emergency Department at Hull Royal Infirmary, part of Hull University Teaching Hospitals NHS Trust. The Trust currently operates on two sites and nearly all acute activity takes place at the Hull Royal Infirmary. The hospital is a tertiary referral hospital for Neurosurgery and Neurology, with Plastic Surgery and Cardiothoracic Surgery located nearby at Castle Hill Hospital. The hospital is a Regional Major Trauma Centre for adults providing care to patients from East and North Yorkshire and North Lincolnshire. It is also a Trauma Unit for paediatric care.

HULL YORK MEDICAL SCHOOL

Hull York Medical School is a partnership between the Universities of Hull and York and the NHS in North and East Yorkshire, Northern Lincolnshire and Teeside. The first doctors to complete the HYMS MB BS programme graduated in 2008. With a distinctive and innovative approach to training doctors for the health services of the 21st century, HYMS aims to influence positively the recruitment and retention of doctors in the region and the quality of local health services. HYMS offers a five-year programme using contemporary learning and assessment methods in an integrated structure to some 240 students each year, having launched a successful expansion bid. The programme features a modern learning environment and high quality clinical attachment opportunities. There is a strong focus on learning in community settings, including primary care, and on working collaboratively with other health professionals. In addition to the 5 year MBBS, HUTH also hosts the clinical placement element of the Physician Associate 2 year Masters programme which is now commencing its third year. The 2019 academic year will also see the launch in Hull of the gateway year programme, designed to widen participation and encourage non-traditional applicants to medicine. The Trust is an associated teaching hospital of the Hull York Medical School taking a very active part in the teaching of Medical Students undertaking the MBBS and Physician Associate MSc programmes. The school is in a period of great expansion with an annual cohort size of 240+.

Phase l of the MBBS course is substantially taught in the University with Year 1 attending half a day a week of clinical placement and Year 2 attending for 1 full day a week. These placements alternate each week between primary and secondary care.

Phase ll (Years 3 & 4) students predominantly attend the Trust for clinical teaching with 1 day a week out in general practice. They currently follow an 8 week block structure, rotating to different hospitals within the HYMS area every 16 weeks, including Hull, Scarborough, York, Grimsby and Scunthorpe.

Phase 3 (Year 5) students have 3 eight week blocks, rotating through medicine, surgery and general practice.

Postgraduate Education

MSc in Clinical Anatomy: Develop advanced knowledge and skills in clinically applied human anatomy to understand how the experience of handling cadaveric material translates to the anatomy of the living person.

MSc in Clinical Anatomy and Education: Acquire the same knowledge and skills as the MSc in Clinical Anatomy, plus a comprehensive professional education around developing teachers and researchers of anatomy.

PGCert, PGDip and MSc in Health Professions Education: Accredited by the Higher Education Academy, this programme is for health professionals who are interested in education, assessment, course design and educational research.

MSc in Human Anatomy and Evolution: This course is a unique opportunity to study human anatomy from an evolutionary perspective, focusing on anatomy and morphology and their interfaces with ecology and behaviour.

MSc in Pharmacology and Drug Development: This programme is aimed at aspiring clinical researchers, as well as those wishing to gain a deep understanding of the underlying principles of how drugs work and how to develop new drugs.

MSc in Physician Associate Studies: This two-year programme, integrated with clinical practice, will prepare you for the rapidly growing healthcare role of a Physician Associate, working alongside doctors in hospitals and GP surgeries.

Postgraduate research: MD in Medical Sciences The MD (Doctor of Medicine) is a higher research degree awarded to those individuals who have graduated from a medical school or any stream of medicine. An MD project provides the opportunity to address issues of diagnosis or management in a clinical environment.

PhD in Medical or Human Sciences: The PhD (Doctor of Philosophy) is a higher research degree, aimed at those individuals looking to contribute new theory and knowledge to their particular field.

MPhil in Medical or Human Sciences: The MPhil will give you the opportunity to conduct a substantial independent research project, leading to an original contribution to knowledge in your chosen field.

MSc in Medical or Human Sciences (by thesis): The MSc in Medical Sciences (by thesis) is ideal for you if you want to get involved in postgraduate research, and obtain the skills to progress to a PhD or MD.

PhD or MD by published work: If you have a portfolio of peer-reviewed publications research to the standard of a PhD or MD, you can submit your work for consideration for the award of PhD or MD by published work.

Hull is a postgraduate teaching centre with an enthusiasm for, and a commitment to high quality training in Emergency Medicine. In addition, the Emergency Department undertakes training of HYMS undergraduate medical students.

MEDICAL EDUCATION CENTRE

Medical Education at Hull and East Yorkshire Hospitals NHS Trust comprises of the Medical Education Centre,Hull Institute of Learning and Simulation (HILS) and the Clinical Skills Facility .The department is accessed by all health care professionals and offers a broad range of clinical training and support. These facilities are easily accessible for both internal staff and external visitors due to its ideal location on the Hull Royal Infirmary site. The Medical Education Centre provides a modern, flexible arena in which professionals can train in a supportive environment. The Centre contains a 200-seat auditorium and three large seminar rooms, which are fully equipped with up to date audiovisual presentation equipment and dual projection facilities. There is an extensive Resource Centre with a sister facility at Castle Hill Hospital. The Resource Centres have holdings of over 10,000 texts and receive 72 journals on weekly or monthly receipt, together with some Video and DVD items. Access is available to the numerous online resources available from the Resource Centres’ PCs. The Resource Centres have 24/7 access via a swipe card system.

The Clinical Skills Facility is also able to offer practical skills rooms including ward areas, operating theatre and simulation suites. Each room has been built with a high level of flexibility and all can be customised to the user’s needs. The Clinical Skills Facility has fast become an established and highly reputable centre of excellence in Clinical Skills training for all healthcare professionals locally, regionally and nationally.

The associated department at Castle Hill Hospital includes a 70-seat lecture theatre with modern audio-visual equipment.

Miss Helen Cattermole is the Director of Medical Education for the Trust and is responsible for the education of medical staff and the running of the Medical Education Centre and the Clinical Skills Facility. Dr Dave Wright is the Director of Simulation.

DEPARTMENT OF EMERGENCY MEDICINE

The Emergency Department (ED) is based in the Hull Royal Infirmary and is the major Emergency Department serving the districts of Hull and East Yorkshire, being one of the largest departments by attendance in the UK. The Department sees a high proportion of critically ill and injured patients.

The Emergency Department has been completely refurbished, and the new department opened in 2016. It has a ten bay resus, fully equipped with brand new equipment throughout. There is a 24 bay majors department, within which all bays are glass fronted, with 5 centrally monitored.

There is a separate Children’s ED with 2 resuscitation bays and a separate waiting area. The Children’s ED is open 24/7.

There is a ‘walk in’ area with 17 assessment and 5 treatment cubicles. This area includes the Primary Care Area within this Emergency Care Area and is staffed by ED Practitioners, General Practitioners and Nurse Practitioners.

The Emergency Department sits within its own Health Group and has its own identical management structure.

Facilities There are two spiral CT scanners situated adjacent to the Emergency Department, with immediate access to cross sectional imaging for critically ill patients 24/7.

The Emergency Department inks with the Acute Medical Unit which has facilities for the care of 50 patients. There is also an Ambulatory Care Unit, covered by the acute medicine team. Both are immediately adjacent to the Emergency Department.

Clinical Decision Unit The ED team have access to CDU beds in the Acute Medical Unit for a specified range of clinical conditions.

Major Trauma Centre As a Major Trauma Centre (MTC) the ED receives critically injured patients from across Hull and the East Riding, as well as receiving ill and injured personnel from offshore facilities. Our new helipad has been purpose designed to accommodate all helicopters in use for civilian operations.

THE HOSPITALS

The Trust comprises of two main hospital sites, Hull Royal Infirmary and Castle Hill Hospital and operates Outpatient Clinics from various peripheral sites.

Hull Royal Infirmary

This hospital is the major acute hospital in East Yorkshire and serves a population in excess of 600,000. It has the only Accident and Emergency department and the following specialities: -

Acute General Surgery Diabetes/Endocrinology Emergency Medicine Gastroenterology General Medicine Medicine for the Elderly Trauma & Orthopaedics Neurology Neurosurgery Obstetrics and Gynaecology Ophthalmology Oral Surgery & Orthodontics Paediatrics Renal Medicine Respiratory Medicine Rheumatology Trauma Surgery Vascular Surgery

Acute Medical Unit - The Acute Medical Unit is situated adjacent to the Emergency Department. The Unit has promoted greater integration of the General Medical teams with the Department of Medicine for the Elderly, allowing medical and nursing staff to cope better with the growing pressures on emergency admissions.

Critical Care. There are 22 beds available for ICU and HDU patients in close proximity to the 9 main operating theatre complex. There are also 8 separate Neurosurgical High Observation Bay and Respiratory Support Unit.

Women’s and Children’s Hospital - This development opened in March, 2003 and provides maternity wards and clinics, an antenatal day unit, a delivery suite, a neonatal unit with special and intensive care, theatres, an early pregnancy assessment unit, 22 gynaecology beds, a gynaecology day surgery unit, gynaecology day care and outpatients' department, a sub-fertility unit, an ultrasound department, and a children's outpatient department.

Eye Hospital – The Hull and East Yorkshire Eye Hospital was completed in October 2002, and provides three operating theatres, a pre-assessment suite, an inpatient ward,

administrative space and a seminar room. Phase two was completed in April 2011 and provides sufficient out-patient space for Paediatric and adult outpatient clinics, along with supporting facilities such as Orthoptics and optometry facilities, Lucentis treatment areas and paediatric outpatients.

Castle Hill Hospital and Queens Oncology Centre

The hospital is one of the two major hospitals in North Humberside. Clinical Services currently on site include:

Breast Surgery Cardiology Cardiothoracic Surgery Clinical and Medical Oncology Clinical Haematology ENT & Head & Neck Surgery Infectious diseases Orthopaedics Pain Medicine Plastic Surgery Rehabilitation Upper and Lower GI surgery Urology

Queens Medical Centre. This unit opened in 2008 and it is the regional centre for oncology and haematology as well as for palliative care.

THE POST

This appointment is for a Clinical Fellow in the Emergency Department, Hull University Teaching Hospitals NHS Trust. This post is offered for Full Time and Less than Full Time appointments.

The timetable will include working a 24-hour shift pattern to include weekends, night duty and cover provision. Current working patterns provide Consultant presence in the Department 08:00 to 00:00 seven days a week.

The appointee will work at Tier 2 level, and this would include grades similar to CT1-2, FY3 and the previous SHO levels. It would suit someone who has finished F2, but does not want to commit to a career post. The benefit of this role is that there is pastoral and educational support. We can also support developing your application for specialty training.

This is also open to those candidates with adequate EM experience who want to get back into a supervised, supported and substantive role.

It is intended that the appointees will work at a level suitable from their previous experience.

Additional Experience There is within the Department a designated, fully equipped seminar room, which is available to staff in the department.

There will be a range of projects that the Clinical Fellow will be offered and expected to be part of. These include education & training, IT, simulation, pathways development, management projects, audits, QIPs etc. Each CF will have a designated supervisor who will support them through these projects and will have one day a week (on average, excluding when on nights) to carry out their project.

The post holder undertakes, exceptionally, to be available for such irregular commitments outside normally rostered duties as are essential for continuity of patient care and, therefore, may be required, in exceptional circumstances, to perform additional duties

If the Fellow is interested in developing a particular skill or area of interest, this can be discussed with their supervisor and necessary support and guidance can be provided.

Appraisal/Continuous Professional Development

The Trust is committed to supporting the requirement for continuing education and professional development as laid down by the Royal Colleges.

An appraisal is carried out according to GMC guidance in preparation for revalidation. The post holder will be subject to an annual review to assess progress (including the relevant professional development activities undertaken).

A Consultant in Emergency Department will supervise the post holder and have responsibility for education and career developments. Annual and study leave will be in accordance with the terms and conditions for medical staff. There will support with the application and interview process for ACCS applications.

Conditions of Appointment

The appointment is for 6-12 months pending satisfactory review by supervising Consultants at one month. A minimum period of three months’ notice will be required on either side before termination of contract. It is required that the applicant is upfront at the interview regarding whether they would prefer a 6 or 12 months appointment. This is required for future workforce planning.

The clinical fellow is expected to adhere to the working requirements of the post which include (not exclusive) completing Trust required mandatory training modules, seeing and treating patients within ED in an expected timely manner (average of 1-2 patient per hour), ensuring to abide by the governance and safety policies of the department, doing what is best for the patient, following departmental procedures and pathways including safe handover of patient care, required interaction with the electronic patient record, good team working and abiding by the Trust’s Core Values.

A satisfactory Occupational Health assessment is a condition of employment.

This post involves substantial opportunity for access to children. It is therefore exempt from the Rehabilitation of Offenders Act 1974. The pay scale for this post will be at Nodal Points 2 and 3 based on the Nationally Agreed Payscales.

THE REGION

The Hull Royal Infirmary is within walking distance of the city centre. Hull is one of the country's main ports with the advantage of closeness to the rest of the European Community.

There are an increasing number of direct inter-city trains to London and regular services to other major centres. The M62 motorway provides fast communications within Yorkshire and links up directly with the A1 and M1 North/South motor routes. Humberside Airport provides a reliable service to several European cities. There is easy access to nearby pleasant countryside, dales, moors and first class coastal resorts. The City of York, with its Minster and many other attractions, including the Jorvik Centre, can easily be visited in a day.

East Yorkshire has forty miles of spectacular coastline from Spurn Point in the south to Filey in the north. Spurn Point has the only full-time lifeboat crew in the country and is a bird watchers' paradise. Nearby is Hornsea Mere, a huge freshwater lake and RSPB reserve. Hornsea also has a famous local pottery and an award winning Museum of Village Life.

Both the City of Hull and the nearby market town of are ancient Boroughs of considerable interest. Hull was very much rebuilt after heavy bombing in World War II and is, therefore, a modern as well as an historic city. There is an attractive Yacht Marina and an international ice rink and there is a good choice of theatre and concert programmes, at both the New Theatre and within the city centre.

Beverley has a beautiful medieval Minster, fine Georgian and other period houses and cobbled market square. The restored Dominican Friary, dating from the 14th Century and nearby racecourse are notable attractions.

The Humber Bridge links the North and South Banks of the Humber, which have a combined population of some 900,000 people.

There are good education facilities, both private and state, for school age children and, in addition to the , there is a second university, the University of Lincoln, based in the area.

Several new projects have been completed in the City, to ensure that Hull is recognised as one of the UK's Top 10 cities. These include the Deep, (a Marine Science and Visitor Centre), a £43 million Stadium and the redevelopment of the Ferensway shopping area where the St Stephen’s Shopping Mall is now located.

In November 2013, Hull was named the UK City of Culture 2017, after facing tough competition from 11 great cities. Housing locally is generally cheaper than in most other parts of the country. There are many villages with good housing/facilities surrounding Hull, within a reasonable travel time.
