2 Norwegian American Hospital FY2017 Annual Report Letter from the President and CEO

Dear Friends,

At Norwegian American Hospital (NAH), we are committed to creating a culture that treats our patients and their family like our own. Not only do we provide compassionate care within the walls of our facility, but we also work throughout the community and host health screenings and events geared toward improving the wellbeing of those we serve.

In 2017, we were delighted to open the NAH Comprehensive Diabetes Center. Our commitment to closing the health gap around diabetes in Humboldt Park was met with reality, and now we have a center dedicated to providing care and education for patients and their families. We were honored to receive external recognition from the American Hospital Association (AHA). Our Pediatric Care-A-Van received the prestigious NOVA Award. Only five AHA NOVA Awards are given nationwide each year and honor hospital-community collaborations that improve community health.

Our organizational commitment to create and sustain clinical, service and operational excellence has elevated our Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) scores, and we also surpassed the National Resource Average several times during 2017. All NAH employees received formal training on patient service and our survey results reflect enormous progress toward our goal of exceptional patient satisfaction. This, we believe, will bring the greatest value to the patients and communities that we serve.

While the goal of an Annual Report is to look back over the past year, we must also look ahead. On the horizon for NAH is the development of a full master plan. The hospital plays a vital role in the economic stability and growth of this community. NAH recognizes that health is more than simply treating patients who come through our doors. We want to adopt an ‘anchor mission’ that can help build a healthier, and also a more prosperous community.

The hospital’s leadership team has resumed planning for a Community Medical Wellness district in Humboldt Park and the surrounding communities. We plan to hire an experienced community wellness planner to work with the community and determine the best long-term use of the hospital’s vacant land. JGMA will manage the project and provide the design services. NAH and the NAH Foundation will provide funding for the first phase of the project. In addition, we will approach philanthropic organizations to join us in the implementation phase of the project.

Finally, we wouldn’t have achieved all that is outlined in this report without the vision and determination of our Board of Trustees. In particular, thank you to Billy Ocasio, board chair, for his commitment and passion which ensures that we bequeath a future for Norwegian American Hospital. I would also like to recognize the talents and dedication of the Norwegian American Hospital physicians and staff. Each of these individuals unifies us in our never-ending pursuit of ways to support and sustain the good health of the community.

There is so much good work still to be done, and I thank you for your steadfast support of these efforts. It is my privilege to help lead this spectacular organization.

José R. Sánchez President and CEO

Norwegian American Hospital FY2017 Annual Report 1 Letter from the Chairman, Board of Trustees

Dear Friends,

One of the key factors that distinguishes a thriving community is its ability to deliver excellent medical care to every resident and visitor who walks through the doors of its hospital.

For most families, the list of basic needs is exceptionally short, with health care at the very top. If the character of a community is defined by many factors, how well it answers the health care needs of its people is surely one of the most important.

Meeting and exceeding this expectation is what guides—and drives—Norwegian American Hospital every day. Over the last several years, the landscape of health care policy has changed considerably. Federal reimbursement for Medicare, Medicaid and even private health insurance cover only a fraction of the actual cost of services. And more people than ever before now depend on their community hospital to provide basic medical care that, less than a generation ago, was obtained from doctors in private practice. The role and responsibilities of Norwegian American Hospital to the people and families of this community has increased significantly.

But despite the relentless challenges to health care, NAH has moved ahead with transformations that reflect the new dynamics of health care and that also meet the growing needs of our community. For example, in April we opened our new Comprehensive Diabetes Center to address the diabetes rates in Humboldt Park that have risen exponentially. We also started a new Family Medicine Residency program—to help attract new doctors to the area because physicians tend to remain in the communities where they complete their residency.

Less visible is our investment in recruiting new physicians so that individuals facing medical issues can turn to their own community hospital and not travel elsewhere at what can be one of the most difficult times of their lives. For the people in our neighborhood this year who will suffer a significant health care issue, having access to a physician staff composed of highly trained and experienced medical practitioners may be life changing.

The Board of Trustees of Norwegian American Hospital takes very seriously our role as fiduciary agents for the community’s health, and I am extremely grateful for their support. I would also like to thank José R. Sánchez for his expertise to guide and oversee the policies and strategic initiatives that support the ongoing success of Norwegian American Hospital.

It is with great pride that I share these efforts and thank you for your continued support in Norwegian American Hospital.

Billy Ocasio Chairman, Board of Trustees

Norwegian American Hospital FY2017 Annual Report 3 This is us.

Since 1894, Norwegian American Hospital has strived to provide excellent care for the Humboldt Park community with thoughtfulness and respect. That mission has never been more important than today, when many of our community’s residents live with economic and social insecurity, threatening the health and wellness of themselves and their families.

In the past year, we’ve made enormous progress to improve the quality of our services and how we deliver them to our neighbors—to provide increased security of accessing the care they need both within and beyond the walls of our hospital. Securing care.

Comprehensive Diabetes Center empowers patients to take charge of their health Diabetes mortality in Chicago’s Humboldt Park community is three times higher than the national average, and more than 20% of Puerto Rican residents are afflicted with pre-diabetes, type 1, type 2 or gestational diabetes. In April, NAH responded to the crisis by launching a new clinic providing expert, individualized care and support for patients with all forms of the disease. The clinic’s team of specialists is committed to a patient-centered, collaborative approach featuring consultation, medical nutrition education, medication management, expanded wound care services, podiatry, vision care, vascular care and other services. Patients learn to achieve wellness through realistic and meaningful lifestyle changes.

“The Norwegian American Hospital Comprehensive Diabetes Center takes a team approach to treatment and wellness that will empower you to take control. We’re dedicated to outpatient care, education, medication management and monitoring of complications.”

NAH Family Medicine Residency program trains future physicians With a looming shortage of an estimated 25,000 primary care physicians in the U.S. by 2025, there is a growing need for physicians who wish to practice in underserved communities. NAH proudly received accreditation for its new residency program and accepted its first class of six new doctors to help address the shortage of family medicine doctors in Humboldt Park. These residents are receiving exposure to the special needs of the community and three years of hands-on training in all aspects of ambulatory and inpatient medicine. This collaborative effort between NAH, PCC Wellness, a federally qualified health center, and St. George’s University Medical School was created to attract doctors to establish practices in the area, since many put down roots where they complete their residencies.

“The new residency program at Norwegian American Hospital is an important part of the hospital’s continuing mission to ensure that its graduates are the outstanding leaders, educators, advocates and clinicians of tomorrow to help serve both our patients and our medical profession.”

transforming our community

NAH: Shaping a wellness district for the future As a community hospital committed to providing high quality, affordable, and culturally competent health care, we know that our long-term sustainability depends on innovations to reduce health disparities and support healthy, sustainable local development. Following years of favorable financial performance, during 2017 NAH took the first ambitious steps to initiate plans for transforming the 24 blocks surrounding the hospital into a wellness district that supports physical, mental and social well-being of all those who live, work and visit in our community. The hospital currently owns 58% of this land, and has selected the award-winning Juan Gabriel Moreno Architects (JGMA) to lead pre-development activities that will identify socio-economic determinants of local health disparities and develop strategies to increase health promotion and disease prevention. Strategic planning will encompass access to affordable, healthy foods; increased access to safe and convenient transportation including walking and bicycling; recreation spaces; job training centers; improved housing security; and a TIF district to advance economic development and employment opportunities. Current collaborators include: Greater Humboldt Park Community of Wellness (CoW), Puerto Rican Cultural Center and the Diabetes Empowerment Center, Alderman Maldonado’s office, Chicago Department of Planning and Development, Association House, Chicago Park District, West Town Bikes, New Life Covenant Church, PCC Wellness, Erie Family Health Center, and more. In the coming months and years, NAH is committed to partner with many more local stakeholders to achieve this vibrant and healthy new community.
