2018 Student Scholarship Instructions Handout to Students: Scholarship Checklist Have with you to use: Sample Scholarship Application, Sample Financial Statement and Sample Special Applications Directions: Go to the BPS Website: Our Schools/Brockton High/Guidance/BHS Scholarship Program. You will see the Scholarship Book and applicable forms. 1. There are 3 Scholarship types in the Scholarship Book:

• Brockton High Scholarships #1-22 (Shaded in Blue): Apply online. Focus on eligibility requirements (i.e. Nursing, Art, etc.) Students should only apply to scholarships they are eligible for. You must fill out one Parent Financial Statement and write an essay about why you are deserving of the scholarship(s) (submit to Brockton High School Scholarship Box located at the Main Office).

• Private Scholarships #23-99 (Shaded in Red): You must print the general application and fill it out for each one of these. Some of these require a financial statement, essay and/or transcript. Please look at where the application should be mailed. Some get dropped in Main Office Scholarship Box, but for most you must mail on your own. You must provide your own postage.

• Special Scholarships #100-#121 (shaded in Green): Requires a special application. The special applications can be found on the BPS website or you can pick up the special applications from your own Guidance Office. Please look at where the application should be mailed. Note all deadlines as they vary. For example: March 30th, April 1st or April 15th. You must provide your own postage.

2. Use the Scholarship Checklist to organize yourself. Check off anything you may need. Leave the checklist with your building Guidance Office Secretary to get the paperwork ready for you to pick up. Tips to make it easier for students: • Print out general application and fill it out leaving the top line blank. Make several copies and fill in scholarship(s) name afterwards. • Parent Financial Statement is required for all scholarships. Make multiple copies. • Write one essay and make multiple copies. • You are responsible for making your copies (staples, envelopes, etc.)

Applicant must be a child or grandchild of a veteran and accepted at an accredited four year-college or university. Student needs to have a copy of their parent or grandparent’s DD2-14. (Discharge paper)

4. APPLICATION REQUIREMENTS

Complete application form obtained on line or in your house guidance office. Transcripts may be requested. Deadline for filing application is April 1st.

Chris Reagan was the youngest child of BHS Principal Robert Reagan and his wife, Sue. He had two older brothers and one older sister. In March of 1991, at the age of 20, he was killed in a tragic automobile accident. This devastated his family. Mr. Reagan had been Principal for eighteen years when he retired one year later, in 1992. In lieu of celebrating Mr. Reagan’s retirement, his colleagues and friends joined together to establish a scholarship fund in Chris’s name, commemorating his loss in a way that would honor his father and go on indefinitely.

Recipient will be a Brockton High School student planning to attend an accredited 2 or 4 year college/university. He/she will be an active participant in school activities, i.e., teams, clubs, intramural JV/Varsity sports. 110 GODDARD HEALTH FOUNDATION SCHOLARSHIP APPLICATION

1. DESCRIPTION:

The GODDARD HEALTH FOUNDATION SCHOLARSHIP is awarded to students who are planning to continue their education in the health care field. It is available to students who are enrolled in public schools, who are residents of BRIDGEWATER, BROCKTON, EAST BRIDGEWATER, EASTON, STOUGHTON or WEST BRIDGEWATER. If possible, two $2,500 scholarships will be awarded to each town’s public high school, with Brockton being eligible for four $2,500 scholarships*. East student must be enrolled or have been accepted into full-time, four year degree program. They must be a person of good character, demonstrate a financial need, and have a well-balanced interest in studies and extracurricular activities.

*Awards are all at the discretion of the Goddard Health Foundation Scholarship Committee; additional scholarships may be awarded. There will also be four $2,500 scholarships available for adult/continuing education college students, who must also reside in one of the six towns listed above. Some consideration will be given to applicants with relatives who are or have been members of the Lebanon-American National Club or its auxiliary.

To be eligible for the Scholarship, the applicant must be a BHS senior who attended a minimum of two (2) years at the Plouffe Academy. Additionally, the applicant must be furthering his/her education on a full time basis at an accredited college, university, vocational/technical school or nursing school.

On a separate sheet of paper(s), submit a letter (approx 250 words) which addresses the following points: (a) information on activities while in the Brockton Public School system; (b) special interests or work experience and (c) plans for further education and career goals. The letter will be reviewed and evaluated by a panel formed by the Scholarship Committee. The panel will judge all the letters submitted by eligible students anonymously. Therefore, it is very important that in your letter you make NO reference to your name or a family member’s identity. The panel is impartial and any indication of who the applicant is will disqualify that applicant and panel member, holding up the scholarship process. Also, an applicant will be disqualified immediately if he/she discusses his/her application and letter with panel members, or they do not follow the proper letter format.
