    AN INVESTIGATION OF THE MENTAL HOSPITAL BUILDING TYPE Irvin J. Kohler August 1954 Submitted to the faculty of the Department of Architecture, Massachusetts Institute of Technology, in partial fulfillment of masterts thesis study, 1954. FOREWARD This study is concerned with the application of .the disciplines of architecture to the problems of the care and treatment of the mentally ill. I feel that there is sufficient justification to demand the re-thinking of build- ings housing the patient's treatment and living facilities to make them more in keeping with our standards of contem- porary achievements in both architecture and psychiatry. The limitations of such a study were: the study, itself; the personal limitations of the investigator; and the presentation. The gap between architecture and psy- chiatry is tremendous. Each is a field unto itself, but the two must be joined together if there is to be a satisfactory solution to the problems of the mental hospital building type. A great deal of time and research was necessary be- fore the writer could even begin to bridge this gap, due to professional differences in terminology, outlook, and ap- proach. The next limitation was my personal discomfort when I saw the patients illhoused, with very little in their en- vironment to help them retain their individuality and denied the right to facilities for a complete life because of build- ing deficiencies. The last limitation was that the presen- tation of such a study must necessarily be orderly and cata- logue in an organized way what a mental hospital is and needs. However, no such order is in existence in the actual build- ing, and unfortunately this presentation tends to leave the reader with the concept of order in the mental hospital.

    Nuha El Sayed, MD Dr. Gerald J. and Dorothy R. Friedman Fellow in Diabetes & Metabolism 2007 - 2009 Education Dr. El Sayed received her M.D. / M.B.B.Ch from Alexandria University, Egypt in 2001. She completed both her Internal Medicine Residency at Boston University’s Roger Williams Medical Center and a Clinical Research Fellowship in Obesity & Diabetes at the Joslin Diabetes clinic in 2007. She then completed her clinical training in Endocrinology, Diabetes and Metabolism as a Friedman Fellow at Tufts Medical Center in August 2009. She is now pursing a Masters in Clinical Investigation at the Sackler School of Biomedical Sciences at Tufts University. Former and Current Position(s) and Academic Interests Dr El Sayed is currently the Associate Director of the obesity clinic at the Joslin Diabetes Center in Boston, MA and an instructor in Medicine at Harvard Medical School. She is also on staff at Harvard Medical Faculty physicians. At the Joslin Diabetes clinic, Dr. El Sayed focuses on the care of overweight and obese type 2 diabetes patients. She leads a team of dietitians, exercise physiologists, nurses and behavioral therapists that focus on weight and intensive treatment of diabetes through the Joslin clinic’s Obesity Clinic. She is also involved in clinical research comparing medical to surgical weight loss in a randomized controlled trial. At Harvard Medical Faculty Physicians (HMFP), she works on various international projects involving diabetes care including projects in the Middle East and Asia where she designs, helps to implement Friedman Program in Diabetes & Metabolism at Tufts Medical Center, 2011 Report, Page 20 diabetes care models, programs and diabetes prevention initiatives, consults on various clinical activities, and coordinates the efforts of different Harvard institutions to facilitate the delivery of diabetes care worldwide.

Multi- acute hospital systems were profiled based on financial performance and utilization metrics. Details for each of these metrics are included in this technical appendix. The Center for Health Information and Analysis (CHIA) relied on the following primary data sources to present financial information: the DHCFP-403 Annual Hospital Cost Report (403 Cost Report), the Hospital Discharge Database (HDD), the Hospital Standardized Financial Statement Database, and Audited Financial Statements. Unless otherwise noted, metrics included in this report are based on financial data reported by acute and non-acute hospitals from Fiscal Year (FY) 2010 to FY14. Discharge data from FY14 included in the acute hospital analysis was reported by acute hospitals in the Hospital Discharge Database, unless otherwise noted. Descriptive acute and non-acute hospital information is from FY14. Hospital 403 Cost Report: The 403 Cost Report is submitted each year by acute and non-acute hospitals and it contains data on costs, revenues, and utilization statistics. Acute hospitals are required to complete the 403 Cost Report based on a fiscal year end of September 30 regardless of their actual fiscal year end. Non-acute hospitals complete the 403 Cost Report based on their actual year end. Hospital Discharge Database (HDD): HDD data is submitted quarterly by acute hospitals and it contains patient-level data identifying charges, days, and diagnostic information for all acute inpatient discharges.
