Why do churches not have possessive names? Example: St. Mary and not St. Mary’s Church? The answer is because it is Christ's Church of or dedicated to St. Mary or St. Nicholas or St. George, etc., rather than St. Mary's Church

When a couple has been married out side of the faith and is married in the Orthodox Church do you fill out the marriage form differently? ( often write the marriage is blessed) No, it is still the crowning service, not a "blessed marriage", they are actually being married in the Orthodox Church.

May I send a funeral form by email? Yes but only send once either by email or mail, all other forms must be sent by mail because they have to be signed.

When someone adopts a new, Orthodox Christian name at their chrismation should this name be listed on the chrismation certificate, and if so, in what position: replacing the given first name, or before or after the given first name? You may list the new Christian name after their legal first, or middle name in quotation marks. Example: Courtney Rose “Mary” Smith

How and where should that "Orthodox Name" be listed on subsequent records (marriage, of children, funeral)? It should not be listed on subsequent records.

Do adults who are chrismated need sponsors? No they do not but children who are chrismated do need a sponsor.

When someone requests a certificate and there is not a copy of a form/record or certificate what do you do? Call the Archdiocese to see if they have a record. If they do not, have the clergy verify it with a witness. Fill out the form/record. Have the clergy’s full name that administered the then have the signature of the clergy filling out the form/record with his name also printed stating this information is true.

What do I do to reissue a certificate that has wrong information? Please send back original certificate with correction clearly marked on the certificate. If you send in a new form/record please clearly have written that it is a reissue but still send in the original certificate. A new form/record with correction are not necessary. If the mistake is yours please send in $15 for the reissue.
