Commonly referred to as ‘mature onset’, it is usually diagnosed in people over 45 years of age, but is increasingly being noted in younger people. As the obesity epidemic hits the Western world, the cases of teenagers developing Type 2 diabetes are becoming alarmingly frequent. At least three quarters of the estimated 366 million people with diabetes world-wide are Type 2 and the World Health Organisation expects numbers to double to over 500 million by 2030. In 2003, the countries with most people living with diabetes are: India (35.5 million), China (23.8 million), United States (16 million), Russia (9.7 million) and Japan (6.8 million). There are approximately 1.7 million people with diabetes in Australia, with atleast half undiagnosed. What are the risk factors? Type 2 diabetes develops gradually, so symptoms often go unnoticed. These may include frequent urination, thirst, blurred vision, skin infections, slow healing, tingling and numbness in the feet. Sometimes no symptoms are noticed at all or the individual assumes that they are part of the normal ageing process. Diagnosis is made by either a Fasting Blood Glucose (FBG) or Oral Glucose Tolerance Test (OGTT), which are ordered by a doctor. People with diabetes are significantly more likely to develop serious health problems like heart disease, stroke, high blood pressure, circulation problems, nerve damage, and damage to the kidneys and eyes. The appearance and severity of these complications is dependent on the duration of Type 2 diabetes and can be delayed or even prevented by early detection and treatment. “ Be part of What research is Garvan doing in this area? This essentially represents the challenge that we, as metabolic researchers, face. In a sense, we are trying to unravel millions of years of evolution to determine how we can resolve this potentially catastrophic situation. We know that in many cases diabetes can either be avoided or sent into remission with diet and exercise. However, the desire to eat is hard wired in each of us and as long as food is available, we are, in a sense, programmed to eat it. In other words, the default state is to eat, not the converse. At Garvan we believe that the solution to this problem will not be simple because metabolism has had millions of years of trial and error to get to where it is today. We are now undertaking the challenge to embrace this complexity and we are ideally placed to do this. Garvan’s Diabetes and Obesity program comprises nearly 100 research scientists and clinicians working together to untangle the complexity of metabolic disease. Our team comprises expertise in a host of areas. This includes functional analysis of individual molecules, either on their own or in the context of the living cell, as they work together to coordinate complex actions like burning energy to coordinate muscle contraction. We have people who specialise in the function of different metabolic organs including the liver, pancreas, muscle and adipose tissue. In particular, we are interested in understanding how these organs work together to control the fate of food and how it is either stored or utilised. We have considerable expertise in working with whole animals and how they respond to genetic manipulations or environmental perturbations. Finally, we have a very advanced clinical group who study humans with diabetes or those who are at risk of acquiring diabetes. More and more of our studies involve the acquisition of huge amounts of data that describe the behavior of the organism. To analyse this we rely on people with very sophisticated expertise in branches of mathematics or physics to help us. It is our philosophy that through the coordinated efforts of all these individuals and their expertise, we will be able to establish how people in our community are likely to respond to certain environmental cues at the individual level. We believe that, in the end, there will not be just one solution to this problem, so we need to devise ways of predicting at an individualistic level how best to treat metabolic disease.     Review Article Expanding the definition of noncommunicable disease Ranabir Salam Department of General Medicine, Regional Institute of Medical Sciences, Imphal, Manipur, India ABSTRACT Noncommunicable diseases (NCDs) are responsible for 68% of all deaths in 2012. Eighty‑two percent of these “premature” deaths occurred in low‑ and middle‑income countries. Most of the NCD deaths are caused by cardiovascular diseases, cancer, diabetes, chronic respiratory diseases, mental health, road traffic accidents, and violence. The World Health Organization, several governments, and nongovernmental organizations have taken up numerous programs to curb the menace of NCDs. However, the present programs do not include some common chronic medical conditions which also lead to considerable morbidity and mortality. The present review highlights three important chronic disorders: chronic kidney disease (CKD), liver disease (cirrhosis and nonalcoholic fatty liver), and thyroid diseases. CKD is an internationally recognized public health problem affecting 5–10% of the world population. CKD resulted in 956,000 deaths in 2013 and proposes them to be included in the world wide accepted definition of NCD. Cirrhosis and chronic liver disease were the tenth leading cause of death for men and the twelfth for women in the United States in 2001. Moreover, 4–10% of the global population have thyroid dysfunction. This mini‑review proposes to expand the definition of NCD to include these three major illnesses. By 2030, it is estimated that non-communicable diseases (NCDs) will account for 52 million annual deaths worldwide. One of the priorities of The United Nations' high-level meeting of the General Assembly on NCDs in 2011 is to reduce "the level of exposure of individuals and populations to the common modifiable risk factors for NCDs, namely tobacco use, unhealthful diet, physical inactivity, and the harmful use of alcohol, and their determinants, while at the same time strengthening the capacity of individuals and populations to make healthier choices and follow lifestyle patterns that foster good health." The biggest challenge in dealing with NCDs is to help patients change their health behavior. Physicians have the potential to influence their patients' health behavior by presenting them with a set of skills in managing their lifestyle choices and by being a role model by practicing a healthy lifestyle. Both approaches have been proven to be very effective in supporting behavioral change. The practice of lifestyle medicine incorporates many public health approaches, but it is primarily a clinical discipline. We are faced with a challenge: on one side, preventive approaches (lifestyle modifications) are generally not recommended or adopted due to multiple etiologies: lack of time, knowledge and skills by primarily medical giver, lack of economic benefits to commercial companies and the biggest challenge of mankind to make a change in oneself behavior, even an unhealthy one.

Healthy lifestyle factors include good nutrition, regular exercise, non-smoking and body mass index of less than 25 kg/m2, etc. Because the oral cavity is generally considered the window of systemic health and disease, the lifestyle behaviors that promote oral health also decrease risks for developing lifestyle diseases. Both periodontitis and all of the lifestyle diseases mentioned above are associated with chronic low-grade inflammation. When bacteria in the oral cavity are dysregulated, periodontal diseases will develop, particularly obvious in those suffering systemic diseases like diabetes and other metabolic disorders.

However, the impacts of different health states have rarely been compared amongst each other, whilst quality of life data from Asia are relatively sparse. We examined and compared the effects of different health states on quality of life in a Thai population. Methods: In 2008–2009, 5,915 corporate employees were invited to participate in a survey where HRQoL was measured by the Short Form 36 (SF-36) questionnaire. The adjusted mean SF-36 scores were calculated for each self-reported illness, number of chronic conditions, lifestyle factors and awareness of diabetes and hypertension. The effect sizes (ES) were compared using Cohen’s d. Results: The response rate was 82% and 4,683 (79.1%) had complete data available for analysis.
