Over the weekend of April 29-30, 2017, in Naya Medicity, Mathura, a group of people from various states met to discuss End of Life Care (EoLC) issues in India. Doctors were represented by intensivists, oncologists, general surgeons, family physicians, pulmonologists, nephrologists, neurologists, palliave care physicians; there was a lawyer who gave a talk on the legal aspects of end-of- life care, two experts who spoke on the spiritual aspects and a sprinkling of lay persons in the audience. At the end of the seminar, there was clarity on the differences and commonalies between EoLC and palliave care. (Euthanasia/mercy-killing is outside the purview of these topics). Being holisc and respecul of the paent’s wishes and preferences in EoLC aims to help the paent to live as well as possible unl natural death occurs. The paent’s family, care-givers and significant others are included in the process. There is an urgent need of awareness amongst the public, for steps to be taken by the government and medical professionals, so that those suffering from terrible pain and discomfort due to chronic or untreatable illnesses, especially at the end of life, are spared some of the agony. The Indian Society of Crical Care Medicine, the Indian Academy of Neurology and the Indian Associaon of Palliave Care formed the End- of-Life Care India Taskforce (ELICIT) and at its first meeng in Mumbai, in mid-August 2015, decided to move the central government to dra and enact an EoLC legislaon. Carrying that forward, at Naya Medicity, Mathura, moderated by Dr. Roop Gursahani, Member ELICIT and neurologist, Hinduja Hospital, Mumbai, the following talks were presented : • ‘Beyond Liberty: Autonomy, Privacy and the Indian Constuon’ by Ms Sarasu Thomas, Assoc. Prof. NLSIU, Bengaluru. • ‘An EoLC Law for a Changing India’, by Dipankar Gupta, Sociologist, former Prof, JNU. • ‘Public Health Care: Can It do EoLC’ by Ravi Duggal, Country Co-ordinator, Sr Program Officer, Internaonal Budget Partnership. • ‘The Costs of Dying in a Hospital’ by Raj K Mani, Chairman ELICIT, Pulmonologist and Intensivist, CEO, Naya Healthcare. • When Ethics and Economics Collide: Making Choices’ by Dr Sridevi Seetharam, Pathologist, Palliave Care Provider, Mysuru. • ‘Life, Death, Autonomy: the Karmic View’ by Dr Devdu Paanaik, mythologist. • ‘A Raonal Soluon for Spiritual Pain’ by Arul Dhas, Chaplain and Sr Lecturer, CMC, Vellore. • Offering siritual solace- the experience of Karunashraya by Dr. Nagesh Simha, Member ELICIT and Medical Director Karunashraya. • ‘Towards a Uniform Definion of Death: Why It Maers’ by Dr Sunil Shroff, Prof of Urology, Chennai, founder- Mohan Foundaon. • ‘Idenfying Fulity and Transions in Care Across Specialies: Possible Protocols’ by Dr Jeremy Johnson, Director, Research and Educaon, Karunashraya, Bengaluru. • ‘Ideas for India: Capacity Building in Palliave Care’ by Dr M R Rajagopal, Chairman, Pallium India. There were concerns that many doctors believe they must prolong life despite financial burdens and ‘worse than death’ treatments. Many a me doctors know of, but may offer no alternave to arficial venlaon and intensive care when a paent is breathless. 3 Vol. No. 27 Issue No. 02 September, 2017 Compassionate communicaon a n d a p p r o p r i a t e u s e o f morphine, for example, are not what they have studied or seen in medical or nursing schools. Corporate versus Government medical care raised other points of economic burdens due to illness. In addion to doctors, nursing aides and social workers needed to be trained in palliave care to help care-givers. The outcome of the seminar led to the ‘Cizens’ Acon Needed for Dignity in Death’ (CANDID); that, with ELICIT, brought together for the first me, individuals from varied walks of life, who signed a ‘Mathura Declaraon’ that aims to promote palliave and EoLC to the terminally ill and dying. The Mathura Declaraon states that: 1. Quality of life is a right guaranteed under the Constuon to every cizen of India and must be upheld, including at the end of life. 2. Palliave and EoLC are important means to ensure this.
