The numbers 2010 - 59 31 above for 2014 are rough indicators for 2009- 44 19 now and should be considered a sampling of recent incidents only. Unfortunately, the 2008 - 35 17 numbers for 2013 were 50% higher than what we 2007 - 34 16 experienced in 2012, and 2014 is not starting out very good. ( as of Dec. 30. 2014) 12/22/94 – A Cargill employee was rescued from an open corn pile after being trapped for over 2 hours. Area emergency responders helped rescue the employee at the Cargill plant in Alberta. The employee was trapped in corn from the waist down and had his foot lodged under a pipe. ______12/19/14 – A farmer – R.V. “Rocky” Schmidt was killed by mechanical asphyxiation in his corn bin near Lynnville, IN. The farm is about 20 miles NE of Evansville. Schmidt was found with a shovel and did have a safety rope on him. However, he was working alone and likely did not have the bin-entry-line secured properly. The emergency responders cut holes into the bin and used a tractor with a vacuum system to suck the grain out of the bin. ______

11/21/14-Grain bin engulfment proves fatal for Laurel Nelson (52), of Grantsburg, WI. Laurel was a long-term employee at Burnett Dairy Cooperative in Alpha for over 30 years. ______11/19/14 – Larry Mitchell (57 year old) farmer was rescued from a bin near Waterford, Mississippi. Responders had to cut into the bin to lower the grain level. The rescue took nearly (5) hours. ______11/09/14 – A 30 year old hired man died in a Harvestore silo near a farm in Dresden, N.Y. This may have been a Harvestore converted to handle shelled corn. Need more info. In any case, the man was fatally injured when he went into silo with unloader still running, to shovel corn to center. It appears the sweep auger hit him, as he fell it caught his shirt and pulled him in. He was found by co-workers, laying at unloader opening with his clothes ripped off and right arm amputated. ______11/06/14 – Mike Miller, 58 year old farmer, died in a grain bin near Valley Falls, KS. ______9/27/14 – Tragically, Colby Williams ( just turned 5 yrs. old) died when the child “fell into a grain cart” full of wheat at a farm outside of Washington, N.C.

Colby was on top of the grain cart when farm workers began loading wheat into it. He sunk into the grain and ended up suffocating. ______9/16/14 – A man was rescued from a grain bin in Wilkin County near Brushvale, MN. The corn bin was part of the operation at Brushvale Seed. ______9/11/14 – This incident is not reflected in numbers above, since it involved a Canadian.

This 44 year old man was fatally injured on Thursday (9/11/14) and ultimately died on Sunday (9/14/14). The man was a farmhand from Keeseekoowenin First Nation. He was in the box of a grain truck that was unloading on a farm near Sandy Lake, Manitoba. ______

09/09/14 – Man escaped serious injury after being trapped in a (5,000 – 6,000 bu.) corn bin south of Newton, IA. Property owned by David L. Peters.

Crews from four area fire departments (Newton, Kellogg, Monroe, & Reasnor) were on hand for four hours. Responders used a cofferdam to stabilize the victim. Responders ended up cutting (4) holes into the bin and the victim was eventually able to exit the side entry door. ______08/24/14 –Man hospitalized after being entrapped in a corn bin in Lenoir County, N.C. The incident happened at J.C. Howard Grain Co. By the time the rescue crews arrived, the man was no longer entangled in the auger, but he was still in the grain bin. ______

08/05/14 – Tim Hansen (60), a prominent Laurel, NE. businessman & former fire chief died after becoming trapped in a grain bin in Dixon County, NE. He went into the 60,000 bu. corn bin to unplug it , while his son was waiting outside.

08/04/14 – Crews rescued a farmer trapped in a on-farm grain bin near Switz City, IN. Bill White, 81, became entrapped in grain in a bin on White Farms, south of Worthington, IN. His sons managed to free him after being entrapped for over 40 minutes in Greene County. ______

07/26/14 – Farmer (Marvin Grenz) dies in grain bin accident near Bismarck, N.D.

He was in the grain bin poling the grain to get it lose, when his son found him partially buried and the grandson called 9-1-1.

This was the 2nd. grain bin death near Napoleon, N.D. in (6) months. 07/18/14 - Ron Eisenman, 62, died in a corn bin after 6:30 PM near Britt, IA., (about 85 miles North of Ames). ______07/16/14 - Man rescued from Newark, IL. grain bin ( approx. 25 miles west of Joliet.) The man’s leg was stuck in an auger. This was largely an “Entanglement” incident. Employee entered the bin, while the auger was running and he stepped into the first intermediate discharge sump hole. He was very fortunate that the screw auger (NOT sweep) only broke ankle & leg bones. ______07/14/14 – Nine-Year-Old boy died in grain bin near Lancaster, IA. The auger inside a small 7-8,000 bushel grain bin was stuck and inoperable. The boy and several family members were trying to fix the auger when the boy became trapped inside the grain bin, which was only about half full. ______7/14/14 – BNSF employee in Kansas City, KS. died after falling into a rail car full of grain in the rail-yard, while the grain was being transferred from the rail car into a semi-truck trailer. ______07/07/14 – Crews rescue farmer stuck in ON-Farm grain bin for 4 hours in Greene County, Arkansa. The farm bin was only 14 feet high and crews cut into the bin to slowly release the grain without pulling the farmer deeper. ______06/16/14 – Farm employee killed in corn bin near Washburn, N.D. Corneilius Johannes DuPlessis, 27, from South Africa was working at the Doepke Farm about 5 miles (S.W.) of Washburn. He was sucked into the corn during the unloading process. He had only worked at the farm for 5-6 weeks. His body was sent back to South Africa to his mother. ______06/12 14 – Lewiston, MN. firefighters rescued a farmer stuck in a corn bin for 3 hours. John Daley, 80, was buried up to his shoulders. A hole was eventually cut into the bin allowing responders to free up his foot from the grating. ______05/27/14 – Man rescued from a 25 foot corn bin on a farm near Canaan, S.C. ______05 / 06 / 14 – Fire completely destroyed an ON_FARM grain bin in Beloit, IL. ______05 / 05 / 14 – Fire-fighters rescued a man trapped in an ON-FARM grain bin in Canandaigua, N.Y., which is located about 220 miles – S.E. of Rochester in the Western part of N.Y.

The man managed to make a quick cell phone call that saved his life. He made the call while trapped up to his armpits in corn.

A truck driver outside the bin managed to shut the power off to the grain auger. A short (18) minutes elapsed between the call being made and the actual rescue. ______05 / 05 / 14 – Leon Madey, 72, died in a family grain bin trying to dislodge corn by himself. The farm is located in Genoa, IL., which is about 60 miles N.W. of Chicago.

He entered the bin himself when other family members were gone. Fire crews had to cut holes into the bin in an effort to locate him around 4:15 PM that afternoon.

05 / 01 / 14 – This incident is not reflected in numbers above, since it involved a sweep auger and not a grain entrapment. Ron Zell, 59, sustained fractures to both legs, when an auger in a farm grain bin caught his clothing near Crawfordsville, IN. The bin only had about an inch of grain across the bottom of the bin at the time of the incident. ______04 / 18 / 14 - farmer survives grain bin incident. A western Iowa farmer was caught waste deep in a grain bin for over four hours before being rescued by the Harlan Fire Dept. Fire fighters involved ended up cutting (4) holes into the bin in an effort to get him out. A - - K.C. Supply cofferdam and Rescue Auger were used to make the rescue. This equipment was donated by The Andersons out of Denison, IA. The farmer entered the bin from the top. We also appreciate PRI training local fire departments in this area. ______04 / 17 / 14: This incident is not reflected in numbers above, since it is a fatality due to a fall. Man died in fall from elevator leg at Ruth, MI. Michael Rempp of Elwell, an employee with an out-of- state private contractor, had been working atop a 60’ leg tower while dismantling a feed mill leg & accidentally fell through an opening in the tower. ______03 / 27 / 14: This incident is not reflected in the numbers above, since it is a fatality due to entanglement , not engulfment. Man died in grain bin auger accident at Southwest Georgia Ethanol Plant (Mitchell County, GA.) from entanglement injuries.

Joshua Fulghum (approx. 30 yrs. old) was one of three men working in the silo. One person was sweeping milo inside, another shoveling and the third was an attendant standing outside the hole. The victim was in the silo raking the milo to unclog the auger, went to step over it, slipped and was pulled under & killed. The supervisor had to run 60 feet to hit the kill switch. ______Worker sues Adam Grain near Woodland, Calif. after engulfment accident. Hector Sanchez is suing Adams Grain Company. Sanchez was buried for 5 hours, while unloading grain. He suffered multiple injuries and could not walk for 2 months after the incident, according to the complaint. ______03/27/14: This next incident is not reflected in the numbers above, since it happened in Ontario, however, it is worth sharing. A 62 year old died in Plattsville, Ontario, Canada, after becoming trapped in a grain silo filled with around 6 PM . The employee of Einwechter Grain & Livestock entered the silo to clear an obstruction. Wilmot fire-fighters recovered the body. ______March 2014: Reports of a 72 year old farmer buried in a grain bin in S.W. corner of MI.. . in Sherwood. ______03/15/14: Tony DeKnikker died in a grain bin at an elevator near Lake Preston, S.D. In Sept. of 2014, .. .OSHA proposed fines of $120,120 to Prairie Ag Partners after the the death of this 51 year old worker, who died due to engulfment in flowing grain in a railcar load-out operation. ______03 / 11 / 14: Man dies due to engulfment in FARM bin in Edgar, NE. Victim engulfed while scooping corn out of his grain bin. ______03 /10/14: Man freed from auger in FARM bin near Bertrand, MO. Man injured when his leg got caught in an auger. The whole rescue took about 3 hours. Fortunately, the victim did not suffer any debilitating injuries. ______02 / 08 / 14: Man rescued from FARM grain bin in Centerville, IN. Workers were unloading grain when the grain “crested” (may have meant crusted) on the top of the silo. The worker then climbed in and became trapped. ______02/11/14: Man dies in On-Farm entrapment incident in Logan County, N.D. Charles Sperle, 39, died near Napoleon, N.D .when corn broke loose from a grain bin he was unloading and covered him. Sperle hired (2) area truckers to haul corn from a 30,000 bushel bin and was unloading the corn with a grain vacuum at the time of the tragedy. ______02/11/14: Man trapped in a grain bin @ Nelson Brothers bin site near Rose Creek, MN was freed by rescue crews. Crews emptied the grain out of the bottom and were able to free him. ______Feb. 6, 2014: Lawsuit settled. – Jury awards over $16 mln. to families of teens killed by entrapment at HAASBACH LLC facility in Mount Carrol, IL on July 28, 2010. Wyatt Whitebread, 14, and Alex Pacas, 19, died of “traumatic asphyxiation when engulfed in a bin of CORN. There were (4) boys in the bin initially, when this tragedy developed. ______02/04/14: Farmer dies in Granite Falls, MN.(which is about 120 miles straight West of Minneapolis). Richard Rosetter, 77, was attempting to empty a bin by himself. The farmer was reported missing by farm workers. The grain bin was emptied, which took 6 ½ hours, and Rosetter was found inside. ______02/04/14: Farmer rescued in IL., thanks to the efforts of Mark Baker and the Stateline Rescue group. A, 78 year old – Rockton farmer was successfully rescued from a grain bin. ______01/24/14: Worker died after falling into a rail car of soybeans at The Andersons facility in White Pigeon, MI. Eric Rodman, 30, died while loading rail cars at the site that morning. He lost his footing and fell into a hopper car, through the open top hatch, according to MIOSHA and likely suffocated. ______01/20/14 : Jeffrey Guebert, 44, died in a grain bin on Guebert Farms near Red Bud, ILL. It appears that he was trying to use a rod in a 20’ grain bin. He couldn’t get grain to flow and sent his father to get a longer rod. Jeffrey got sucked all the way down into the grain bin and ultimately had his foot caught in the auger, but he most likely was dead at that point. ______01/09/14: Two younger men, ages 17 & 21, were buried in 25,000 bushels of CORN on a farm in Eastern , near the Ohio line , just West of Hicksville, OH. It took over four hours to rescue the men who were shoveling corn in the bin, while their father was loading a truck with a 10” auger outside. The one young man was buried to his chest and the other up to his neck when rescuers arrived. We can credit this save to the annual industry training done at Sidney, OH. every August. The bin was cut to help level the corn inside the bin and a powered auger was used, along with GSI Res-Q-tubes to aid in the rescue.
