About the service

Birdston Care Home is located in area of . It is an NHS Continuing Care Facility, which operates as a partnership agreement between NHSGGC and Pacific arC e. The partnership has proven to be a long-standing success for both residents and their families.

The aim of the service, in line with the Conditions of Registration:

"To provide a care service to a maximum of 63 adults, age 30 years of above, with mental health problems, including organic mental health disease and presenting with challenging behaviour. "

Birdston Care home can accommodate 63 residents in single ensuite bedrooms. The service is in a rural area, with other care home services within the local area.

Birdston Care Home is a specialist nursing home, within a purpose-built unit. Residents have access to garden areas. The service provides care to older people including residents with a variety of mental health illness and many types of dementia.

The overall aim of the service is to provide care and support for residents within a safe environment.

There are plans to relocate residents of the care home to NHS beds in the future. This relocation will take place between 2018 and 2019.

What people told us

Prior to the inspection we issued 20 Care Inspectorate questionnaires to people using the service. We did not receive any completed questionnaires

We issued 20 Care Inspectorate questionnaires to relatives and received one completed questionnaire. The one person strongly agreed that they were happy with the quality of care their relative or friend was receiving.

Some comments included: 'There is a fantastically organised team of staff'. 'I feel that staff do an amazing job'. 'Staff take their job seriously'.

During the inspection we spoke to a number of people living in the home on a one to one basis. People told us that they were happy with the care and the staff.

Their were no relatives or visitors for us to speak to during the inspection.

What the service does well

Staff worked in partnership with a good range of healthcare professionals to ensure that people had all aspects of their health and wellbeing needs met. Regular input from the healthcare team included psychiatrists, GP, dietician, speech and language therapists. Being responsive to changes in care needs meant that people were supported in a way that continued to promote their comfort, independence and safety.

Staff monitored the needs of people and changed their plan of care if needed. We saw some good examples of staff actively managing distressed reactions in people. This showed us that staff used their skills and knowledge to promote care that was responsive to people's needs while maintaining dignity.

Nutritional needs were met through the support of the catering team who provided a good range of food and drinks. At mealtimes staff were patient, encouraging and promoted independence. When required staff provided adaptive cutlery and crockery to assist people. We were told that 'food arrives piping hot, is varied, appetising and scrumptious'.

Record keeping and documentation was in place that assured us the service complied with legislation. We saw that regular care reviews had taken place and where appropriate families were involved, conforming to good practice. Safeguarding was evident through the various registers that were kept, for example for guardianship and power of attorney. Where people needed additional support to have their voice heard local advocacy support services were called involved. All of these things help protect the rights and dignity of people.

Peoples' needs were assessed using a variety of valid tools, such as for weight management and moving and handling. This showed us staff used the most appropriate methods of assessing care to in order to support the needs of each person. Where required, specialised equipment or treatment plans were in place that meant care and support was specific to each person.

There was good access to spiritual advisers who visited often, providing important support to some people. There was a range of different activities available to suit different people's needs and tastes. A regular café on the foyer, befrienders, music in hospital visitors and a regular hairdresser were some of the activities available. These things helped provide stimulation, physical activity and enjoyment for people.

There were very good quality assurance mechanisms in place to ensure the service identified areas of good practice and any areas for ongoing development. This showed us that the team are pro active in managing the service.

What the service could do better

The service should continue to refer to Care Inspectorate notification guidance to ensure all relevant events are notified in a timely manner. We discussed this during the inspection and have confidence that the service will make notifications.

We encouraged the service to promote ongoing review of moving and handling practices to ensure that all staff conform fully to the principles of safe moving and handling in accordance with their training.

We discussed opportunities for people to make use of the garden spaces. We were aware that some people do access the garden. Staff had responded to some feedback from families about having somewhere to sit within the garden. The staff response to provide seating had been appreciated by families. However, we heard from one person who said that 'just to feel the sun on your back is a basic human right'. We discussed how opportunities for more people to access the garden could be achieved.
