It is designed to help students prepare for college, then advise and assist them throughout their college years.

Eyeglasses, Dentures and Hearing Aid Program (EDH) PH: 580-924-8280 or 800-522-6170 This program provides eyeglasses, dentures, and hearing aids to Choctaw Indians over the age of 55 years that possess a CDIB and live within the 10 ½ counties of Choctaw Nation. EDH services also include canes, commode boosters, crutches, commode chairs, wheelchairs, bath/shower chairs and walkers.

8 Jones Academy PH: 918-297-2518 or 888-767-2518 Jones Academy is a Native American residential learning center for elementary and secondary school age children. The facility is located in southeast Oklahoma and houses about 190 co-ed students grades 1 – 12.

The program provides academic, career, and financial counseling to its participants and encourages them to graduate from high school and continue into a post-secondary institution.

This program provides funding for supplemental programs through contracts with the public schools to meet the specialized and unique educational needs of eligible Indian students.

The courses must be BIA approved courses in accredited colleges or Kiamichi Area Technology Centers.

Clients must reside within the boundaries of the Choctaw Nation and meet other eligibility requirements by the federal regulations.

Services to prevent abuse and neglect are also offered.

Choctaw Nation Indian Child Welfare PH: 580-924-8280 x 2635 Our social workers join with state workers and the courts in working to insure safe and healthy homes for Choctaw children removed from their homes. This includes finding Choctaw Foster and adoptive homes to prevent placement in non-Indian homes.

Choctaw Nation Family Preservation PH: 580-924-8280 x 2663 The Choctaw Children and Family Services Family Preservation program promotes family strength, stability, parental functions and the protection of children. The program is designed to prevent family disruption and out of home placement by meeting the needs of a family in times of crisis.

11 Choctaw Legal Assistance for Victims Program PH: 800-522-6170 x 2550 Choctaw Nation provides legal representation to victims of domestic violence, dating violence, sexual assault or stalking, divorces, custody cases, paternity cases and others. Must be a tribal member, a victim as set forth above, and reside in the 10 ½ county service area.

Employment Assistance PH: 580-924-8280 or 800-0522-6170 Ext: 2237 or 2238 The Employment Assistance Program provides assistance to relocate and start a new, permanent type job. Must be ¼ or more Native American.

Community Based Social Work Program (CBSW) PH: 580-326-8304 or 877-285-6893 The Community Based Social Work Program is a program which serves Choctaw individuals and families. The program’s primary goal is to assist Choctaw families in locating resources to help meet immediate needs.

Choctaw Nation Transit PH: 580-326-8304 or 877-285-6893 Choctaw Nation Transit provides or assists with transportation to and from non-emergency medical appointments. The goal of the program is to develop and implement a comprehensive transportation strategy that will enable tribal members and all community members to have reliable transportation to Choctaw Nation Health Facilities, and to participate in other aspects of community life.

Injury Prevention Program PH: 580-326-8304 or 877-285-6893 The goal of the Injury Prevention Program is to focus special attention on activities to prevent or reduce the occurrence of injuries for Native Americans residing in the 10 ½ counties of the Choctaw Nation.

12 Community Health Representative (CHR) 219 North Broadway Hugo, OK 74743 PH: 877-285-6893 Ext. 6094 This program is designed to assist Choctaws and other Native Americans maintain the health of themselves and their families. The CHR Program can do many things to assist clients such as taking vitals, glucose monitoring for diabetes, hypertension and cholesterol diet education, patient education, maternal and child health, drug and alcohol abuse counseling, patient assessments, medical referral follow-up care, as well as the provision of many other health care services.

Faith Based Counseling/Services for Victims of Crime 219 North Broadway Hugo, OK 74743 PH: 877-285-6893 or 580-326-8304 This program provides faith-based counseling to restore the balance and harmony in the lives of crime victims. Through the collaboration or spiritual leaders, victim service providers, and traditional healers, the program works to overcome the violence, and abuse within our native families who reside within our 10 ½ county service area within the Choctaw Nation.

Project C.H.O.I.C.E. One Choctaw Way Talihina, OK 74571 PH: 800-349-7026 x 6295 or 918-567-7000 x 6295 Project C.H.O.I.C.E. (Committed to Helping Others (victims) in our Community and Environment) is a program committed to fighting against domestic violence. They provide services such as brief counseling, case management, safety-planning and behavioral health. They can also make referrals for other Choctaw Nation services. There are eligibility requirements.

Families in crisis situations are often assisted through this program.

Project EMPOWER 219 North Broadway Hugo, OK 74743 PH: 580-326-8304 or 877-285-6893 Project EMPOWER’s (Education, Motivating and Providing for victim’s Working to Enrich and Reclaim their lives) goal is to provide a broad range of services to individuals who are victims of domestic violence, sexual assault, dating violence and stalking. Some of the services include transitional housing and support, safety and life planning, and transportation.

OK 74743 PH: 877-285-6893 or 580-326-8304 The program assists Choctaws and other American Indians within the Choctaw Nation to cope with and heal from the victimization of crime. CNVA seeks to identify and address the needs of crime victims. Victims to be served include: victims of elder abuse, child abuse, homicide, gang violence, domestic violence, and driving while intoxicated victims, and other victims of crime. The types of services provided are emergency food, clothing and shelter, transportation, courtroom advocacy and other emergency services that will restore the victim’s sense of dignity.

Choctaw Nation Project HOUSE PH: 580-326-8304 or 877-285-6893 The goal of Project HOUSE is to serve individuals who are in need of housing assistance as a result of fleeing a situation of domestic violence by establishing a holistic, victim centered program that provides a broad range of services.

Project YOUTH (Youth Outreach and Understanding Through Healing) PH: 580-326-8304 or 877-285-6893 Project YOUTH’s goal is to serve youth and young adults ages 13 – 24 who are victims of sexual assault, domestic violence, dating violence, and stalking through a comprehensive model of care.

Choctaw Nation Chahta Inchukka Home Visiting Program PH: 580 326-8304 or 877-285-6893 Chahta Inchukka is a home based visitation program to help parents with a goal of providing early childhood development services to Native American pregnant or parenting women of children ages 1-5 who reside within Atoka, Haskell, Latimer, McCurtain and Pushmataha counties. The overall goal of Project Chahta Inchukka is to improve maternal and infant health, parenting skills, school-readiness of children and prevent child abuse and neglect.

For more than 1,700 low and very low income families in rural Southeast Oklahoma, the American Dream of Homeownership has become a reality through Little Dixie Community Action Agency’s Self Help Housing Program. The Purpose of the Section 502 Mutual Self Help Housing Loan program is to provide housing opportunities for very low and low income households. The program is targeted to those families who are unable to obtain decent, safe, and affordable housing through conventional methods.
