Welcome to the Annual General Meeting 2011. The Twenty-second Council has completed its term of office of two years. The report of the work done is presented in the pages of this Annual Report. This being the election year, a new Council is to be elected.

WORK OF THE TWENTY-SECOND COUNCIL (2009 – 2011) Membership The membership of the College as of 31 March 2011 stood at 1407 compared to 1368 two years ago. There has been an addition of 23 Fellows and Honorary Fellows and 4 Collegiate members to the College membership list. In the two years, the number of MMed(FM) holders has increased from 297 in 2009 to 321 as of March 2011, an addition of 24 holders. The number of GDFM holders has increased from 434 in 2009 to 492 as of March 2011, an addition of 58 holders.

Assets As of 31 March 2011, the College’s cash and cash equivalents stand at $3,965,183 as compared to $3,262,732 a year ago.

Courses conducted In the two years under review, the College conducted 12 skills courses, and 8 modules of the modular courses. Thanks are due to the many resource persons, speakers, chairpersons, tutors, and trainers in the various educational activities. The College would not be able to do without these people.

College publications A total of 11 issues of the Singapore Family Physician have been produced during the two years under review. Since 2009, the Singapore Family Physician is in the Western Pacific Region Index Medicus (WPRIM). A total of 8 issues of the College Mirror, and 3 issues of the College Digest were produced during the two years under review. Thanks are due to the many contributors to the College publications.

Research related activities In 2010, the College hosted the 2nd Asia Pacific Primary Care Research Conference with 116 paid participants. The Fellowship by Assessment Classes hosted two workshops on research skills, and each candidate produced 3 clinical reviews, and a research project each as part of the course work.

RELATIONSHIPS The College enjoyed good relationship and collegiality with the Ministry of Health, Ministry of Community Development, Youth and Sports, Health Promotion Board, Singapore Medical Council, Singapore Medical Association and the Academy of Medicine. It also enjoyed good fraternity with colleagues in the restructured hospitals, the Polyclinic clusters and GP clinics. The synergy was particularly strong between the College and these stakeholders during the H1N1 outbreak which occurred in April to August of 2009.

THE COLLEGE COUNCIL AND SECRETARIAT Much of the achievement of the two years was made possible by a synergistic College Council and hardworking College Secretariat. The membership has also been supportive of the efforts of the College in vocational training and continuing professional development. Let me take this opportunity to thank the Executive Committee, members of the retiring Council, and members of the College Secretariat for the contributions they have made to the College in the last two years.

ELECTIONS This Annual General Meeting will also be electing its next College Council for 2011 – 2013.

Dr Cheng Heng Lee

The Albert and Mary Lim Award first presented in 1974 in memory of the late Dr Albert Lim Liat Juay and his wife Mary Lim was re-instituted in 1999.

This award is the highest accolade bestowed by the College for contribution and services rendered to the College and the discipline of family medicine.

The recipients of this award for 2010 were A/Prof Cheong Pak Yean and Dr Cheng Heng Lee.

College of Family Physicians Singapore ANNUAL REPORT 2010-2011 CONTINUING PROFESSIONAL DEVELOPMENT / MEDICAL EDUCATION PROGRAMMES AND COURSES

The College promotes Continuing Professional Development (CPD) and Continuing Medical Education (CME) through the Family Medicine Modular Course, Family Practice Skills Course, MMed (Family Medicine) Programme B, Family Medicine Membership and Fellowship Programme.

The Institute of Family Medicine (IFM), an organ within the College, plays a significant role in developing the contents and syllabi of these programmes and courses.

During the year the IFM reviewed, updated and edited the notes as well as their e-learning components for Modules 8, 1, 2 and 3 of the Family Medicine Modular Course to make them more relevant and user-friendly to the course participants

The Family Medicine Modular Course consists of 8 modules of knowledge acquisition and is a requirement for doctors who are preparing for the Graduate Diploma in Family Medicine (GDFM) Programme and the Master of Medicine (Family Medicine) Programme A. Each year, four modules are covered.

The notes for the modules are on the College’s e-learning website and can be downloaded by the Family Medicine trainees. By having them online, the trainees can refer to these notes twenty-four hours a day

For the year April 2010 - March 2011, there were 102 trainees registered in the GDFM course, 8 trainees in the MMed (FM) Programme B.

Family Practice Skills Course (FPSC)

During this financial year, the College conducted seven Family Practice Skills Courses.

The components of a skills course may include one or more of the following: 1. Distance Learning with verification (6 CME points) 2. Seminars (max. of 2 CME points per seminar) 3. Assessments which can be written, oral, demonstration and/or clinical (for some courses)

A typical FPSC template is a course with six units of distance learning with verification of 30 MCQs, two seminars and one workshop conducted over two afternoon sessions e.g., one Saturday afternoon and Sunday afternoon or two Saturday afternoons.

The E-Learning programme launched in 2003 has been incorporated into the GDFM training programme. An E-Learning component for each module was introduced, consisting of therapeutic notes, case scenarios, assessment by MCQs and a case review to be put up by a tutorial group.

Accreditation of core CME points

The College is represented by A/Prof Lee Kheng Hock and Dr Pang Sze Kang Jonathan in the Singapore Medical Council (SMC). Maintenance of Competency Committee and Dr Tan Tze Lee and Dr Leong Choon Kit in the SMC CME Co-ordinating Committee. The College works closely with the Singapore Medical Council to ensure that the quality of the CME events is acceptable. It continues to advise the SMC on the accreditation of core family medicine CME events through its panel of Family Medicine(FM) CME advisors appointed by the College.

In the year under review, the College Mirror continued in its mission of putting together timely news to update the family doctor fraternity, under the leadership of Dr Eu Tieng Juoh Wilson and Dr Wong Tien Hua and their editorial teams.

The College Digest as an e-newsletter is into its second year of production. It continues to serve the objectives of covering the time space between the quarterly College Mirror issues. More importantly, it has a practice management focus in mind -- practice news around the College, practice notes, practice information and a call from the editorial team. College thanks the work of the three editors in this publication: Dr Eu Tieng Juoh Wilson, Dr Leong Choon Kit and Dr Wong Tien Hua.

SIGNIFICANT COLLEGE EVENTS

Commencement of Academic Year 2010 The Commencement Ceremony was held on 26 June 2010 at College of Medicine Building.

There were 102 trainees in the GDFM course, 19 trainees in the MMed (FM) Programme A, and 8 trainees in the MMed (FM) Programme B.

Convocation 2010 The Family Medicine Convocation Ceremony was held on 28 November 2010 and the Guest-of-Honour was Prof K Satku. The special invited guests to the Convocation were Professor Claire Jackson (President, RACGP), Dr Jennifer Kendrick (Censor-in-Chief, RACGP) and Professor Jan Radford (Immediate Past Censor-in-Chief, RACGP) to witness the adoption of the Memorandum on reciprocal recognition of the FRACGP and FCFP(S) between The Royal Australian College of General Practitioners and the College of Family Physicians, Singapore.

13 doctors were conferred the FCFP(S), 8 doctors were conferred the MCFP(S).

58 GDFM graduates and 24 MMed (FM) graduates were recognised.

Annual General Meeting 2010 The College AGM was held on 26 June 2010. The minutes have been circulated.

CFPS Holdings Pte Ltd The CFPS Holdings Pte Ltd was formed as a company to purchase, hold and manage the property or properties for the College and this was incorporated on 23 March 2009.

A property at 7 Cantonment Road Singapore 089734 was subsequently purchased in 2009. This completed the terms of reference of the Building Committee and CFPS Holdings Pte Ltd was set up on 23 March 2009 to manage this property. A Board of Directors was appointed under the terms of reference of CFPS Holdings Pte Ltd.

During its term of office from 2009 to 2011, the Council had the following tasks in mind: strengthening the recognition and equitable worth of the family doctors; implementing the accredited modular course programme for senior family doctors; and making ‘right-siting’ (Shared Care) work for family doctors.

On a larger platform, it has also promoted the level of recognition of scholarly activity by having the Singapore Family Physician joining WPRIM; as well as promoting primary care research. Council has also worked on the reciprocal recognition of the FRACGP and the FCFPS.

Council has also promoted the standard of practice through the initiatives of the practice management committee and the family practice skills courses.

INTRODUCTION 2010 has been a fruitful year for the Censors Board as we spearheaded the interactions with The Royal Australian College of General Practitioners (RACGP) over the Reciprocity issue. The Censor-in-Chief of the 2 colleges exchanged notes on the respective College’s training programs as well as flew over to observe and view the conduct of examinations and the training facilities of the individual colleges. The exchanges culminated in the signing of the reciprocity on 6 October 2010 in Cairns at the RACGP convocation ceremony. This feat would not have been possible but for the high standards and rigor of the educational programs that College has upheld over the years. This was made possible by the foresight and tenacity of our leaders (both past and present); the hours of yeoman service by the supervisor and trainers; as well as the commitment and hard work of the trainees. The College Censors Board will strive to improve the training and recognition of our fellow colleagues in the field of family medicine with the ultimate goal of achieving better care for our patients.

College of Family Physicians Singapore ANNUAL REPORT 2010-2011 GRADUATE DIPLOMA OF FAMILY MEDICINE (GDFM) The Graduate Diploma in Family Medicine (GDFM) is a vocational training certification for primary care doctors. The aim of this 2-year part-time trainee programme is to train primary care doctors to practise Family Medicine at an enhanced level to meet the needs of the child, the adolescent, the adult and the elderly. The course emphasises on basic clinical, diagnostic and management skills essential to general practice.

The programme is planned to accommodate the busy doctor’s schedule as almost all courses that require in-person attendance are conducted outside regular office hours. The course consists of the following: • 8 modules of Family Medicine Modular Course (FMMC) with each comprising of 4 workshops (2 1/2 hours each), 1 tutorial (1 hour each), online case studies and multiple choice assessments • 3 Practice Management Courses (16 hours in all) and 1 elective Family Practice Skills Course (6 hours)

The examination is conducted by Division of Graduate Medical Studies (DGMS), National University of Singapore (NUS). The standard set is a pass in all three segments: • Written paper I - Applied Knowledge Test: 100 MCQ questions (2 hours) • Written Paper II - Key Features Problems (KFP): 10 questions (1 hour) • Skills Assessment in the Objective Structured Clinical Examination (OSCE) - 10 stations of 11 minutes each, comprising 3 minutes for reading and 8 minutes for assessment (2 hours)

The GDFM(NUS) is a title or qualification approved by the Singapore Medical Council for display by Registered Medical Practitioners.

The 9th GDFM Examination was held on 25 June 2010 for the written papers and 3 - 4 July 2010 for the OSCE. A total of 58 of the 80 candidates passed, giving a passing rate of 72.5% for the GDFM Examination 2010. The Censors Board, which was the Examination Board, vetted and finalised all the questions.

MASTER OF MEDICINE (FAMILY MEDICINE) [MMed(FM)] There are currently two programmes to prepare doctors for the MMed(FM) examinations.

Programme A Programme A is for applicants who are fully or conditionally registered with Singapore Medical Council (SMC) and are awarded sponsorship by the health clusters after choosing Family Medicine as the specialty.

The New syllabus from May 2007 features more posting for trainees to gain wider exposure to various specialties to help the family physician.

Note: With effect from 2011, Programme A is organised into a 3-year Residency Program. The first intake nationally is 26 [10 in Singhealth, 10 in National Healthcare Group (NHG) and 6 in National University Health System (NUHS)].

Programme B is a structured training programme designed for mature practicing family physicians in Singapore. Programme B is jointly organised by the College of Family Physicians Singapore and the DGMS, NUS. This 1-year part-time course provides a comprehensive and structured training programme for doctors working in primary care who have completed the 8 modules of the Family Medicine Modular Course (FMMC) to prepare them to sit for the MMed(FM) examinations.

The course participants from both the Programmes sit for the same examination – namely, the MMed(FM) examination conducted by the DGMS, NUS.

This programme was started in 1988. Potential candidates for this award must successfully complete a demanding two year assessment programme and an exit viva voce examination. To be in this programme the applicant must: • Have Collegiate Membership of the College of Family Physicians Singapore • Possess the MMed(FM) or the MCGP (Singapore) or equivalent qualifications; the acceptance of equivalent qualifications has to be approved by the Censors’ Board • Have embarked on personal professional development and training related to family medicine • Be actively involved in postgraduate training and undergraduate education in family medicine • Be in active clinical practice • Be of sound professional and personal character

College of Family Physicians Singapore ANNUAL REPORT 2010-2011 Family Practice Skills Course (FPSC)

The Family Practice Skills Courses serve the continuing professional development needs of the General Practitioner / Family Physician. We are able to run at least one course a quarter. See Table 2.

The components of a skills course may include one or more of the following: 1. Distance Learning with verification (6 CME points) 2. Seminars (max. of 2 CME points per seminar) 3. Workshops (max. of 1 CME point) 4. Small group activities including clinical attachments, tutorials, ward-rounds 5. Assessments which can be written, oral, demonstration and/or clinical (for some courses)

A typical FPSC template is a course with six units of distance learning with verification of 30 MCQs, two seminars, and one workshop conducted over two afternoon sessions e.g., one Saturday afternoon and Sunday afternoon or two Saturday afternoons.

During the year the College conducted seven Family Practice Skills Courses.

E-Learning

The E-Learning programme is a growing medium of CME activity delivery. Launched in 2003, it has been incorporated into the GDFM training programme. An E-Learning component for each module was introduced, consisting of therapeutic notes, case scenarios, assessment by MCQs and a case review to be put up by a tutorial group.

The secretariat headcount remains stable although we had to replace 2 staff. One of our former staff left to join SMU while the other decided to start a family. We wish them all the best. We welcomed a new Administrative Executive (Programmes & Training) and an Administrative Assistant.

They have since integrated well and form part of our very hardworking and diligent secretariat workforce. The rest of the staff has been able to help the College co-ordinate the functions and duties in the delivery of the various training programmes and Family Practice Skills Courses.

We endeavour to provide a high quality service to all our members and strive to assist all our members in their work and requirements for CME training or other post-graduate training requirements.

The College continues to work closely with the various statutory boards and Ministries in the various programs. We wish to thank everyone who has persevered and continue to help us in our daily work. We wish each and everyone who are our members and those who have or will come into contact with us the best of health always!

One of the biggest challenges faced by the committee is the need to bring fellow members together, to hear them out, to support them and to implement any of the suggestions. We are pleased to report that we were able to start a few small steps.

Town Hall Meetings Our committee has decided to revive the town hall meetings held in the College previously. So far, we have conducted 3 of them. The number of members attending was small but encouraging. There were some issues raised by the attendees and the younger ones who attended have been ministered and appropriately advised. We hope to continue these meetings in the future but in a more relaxed environment.

Lunch Meetings These lunch gatherings were organised for the purpose of helping fellow GPs working in the same vicinity to know each other. The atmosphere was cordial and relaxed. And these situations were conducive for building bridges between GPs and breaking the ice between them.

Email Discussion Groups Presently, we have set up 5 active email discussion groups of different maturity. Each group comprises 8 members of which 3 will play the role of moderators. This avenue allowed for more personal sharing and discussion of more sensitive topics.

One on One Visit We have visited two colleagues at their newly established clinics as well as meeting up with a few younger colleagues who are keen to start their own practice. We find this avenue the most useful as we are close to the ground and listen to their concerns first-hand. We hope that we can do more of these visits.

Other Collaborations With the purpose of supporting better practice management among ourselves, we have started the College Digest with our College Mirror editors. We tried to bring up issues or ideas in short snippets for the busy FPs/GPs.

In September last year, together with the College Secretariat, we tried to bring some tangible benefits to our members by helping to secure the newly launched iPads. The response was much better than expected and we were overwhelmed with orders.

Future We would like to continue serving our members and help bring out the best of our members. We look forward to more opportunities to work together for our common good with all of you.

With the implementation of the online Editorial Manager in February 2010, the efficiency of the management of submitted articles has been greatly enhanced. Since February 2010, the total number of articles submitted to the Editorial Manager was 54. 31 articles were accepted, there were 13 incomplete submissions, 6 were withdrawn, 3 were sent back for revision and 1 remains under review. The time taken to process each article will still need to be improved and we will be aiming to streamline this in the coming year. The number of original articles accepted for publication under ORIGINAL PAPER section in SFP was 9. The quality of articles has improved perceptibly with each submission. It is the hope of the Editorial Board that more of our colleagues will contribute to writing and submitting original articles to the SFP.

The SFP’s role in providing CME is of great importance to our members and readers and this it continues to fulfill very well indeed. The Journal is also a platform for our colleagues to publish articles and this role is slowly but surely becoming more important. As more original articles are submitted, we would also like to encourage submissions of case reports, letters to the editor and book to our journal.

Though writing may appear a daunting task, it is actually not that difficult to make observations, put pen to paper and then submit through the Editorial Manager. If any fledgling authors out there have concerns about writing and submitting an article, please feel free to approach any of our Editorial Board who will be most happy to lend an ear and give advice.

College Mirror and Digest During the term of the present Council, the College Digest was launched in July 2010. The aims of this electronic newsletter were to: n Establish an on-line communication path between College and its members n Maintain a current email database n Add value to being a College member n Provide timely and useful electronic tools and references

The College Digest is published four times a year, in the month immediately following the College Mirror, i.e. January, April, July and October. It is emailed to College members only. About 1400 are on the email database. The College Mirror continues to be the main College bulletin. It reaches an audience of 5400 including members, other doctors in the community and in hospitals and polyclinics. Sadly, we bid adieu to our long serving staff Ms Linda Marelie who took care of the editorial and publishing needs of the SFP and the College for the past 5 years. She had overseen the College’s publications, was instrumental in getting them to print and for this, we offer our heartfelt thanks. We welcome Ms Stella Teh who has taken over Ms Linda Marelie’s portfolio. She will henceforth answer all your editorial and publication queries. TERMS OF REFERENCE This Committee was set up in July 2001 with the following terms of reference: 1. To advise the College Council on developments in Family Medicine in the international scene; 2. To implement activities decided by Wonca World & Wonca Asia Pacific Region; 3. To implement bilateral activities in Family Medicine teaching, research and practice.

FAMILY MEDICINE DEVELOPMENTS IN THE INTERNATIONAL SCENE The 19th Wonca World Conference was held from 19 – 23 May 2010 in Cancun, Mexico. One of the important events was the taking office of the new team for the triennium 2010 to 2013. Dr Alfred Loh remains at the helm as CEO of Wonca. Professor Richard Roberts is the World President. The President Elect is Professor Michael Kidd. The Regional President for Wonca Asia Pacific Region is Dr Donald Li. The new Wonca News Editor is Dr Karen Flagg. The other office bearers are listed in the Wonca News August 2010 issue page 21.

In his Presidential Address, Professor Richard Roberts paid tribute to the work done by the outgoing President, Professor Chris van Weel. Chris travelled extensively to meet the family doctors around the world during his term of Wonca President and is well-known to many. For his contributions to Wonca as President and his many contributions to Wonca over the years, he was awarded Honorary Direct Life Membership of Wonca. His landmark contributions were: being the WHO Liaison person for Wonca, ICPC classification of medical conditions, the WHO Mental Health Report, the Young Doctors Movement and Primary Care Research. He has visited Singapore on several occasions during his term of office and before that.

The next World Conference will be held in Prague, Czech Republic in June 2013. The dates are 26 – 29 June.

ACTIVITIES IN ASIA PACIFIC REGION There were two significant activities in the Asia Pacific Region in the year under review.

(1) Second Asia Pacific Primary Care Research Conference The Second Asia Pacific Primary Care Research Conference was held from 4 - 5 December 2010 in Singapore at the Gallery Hotel. It was well attended with 116 paid delegates from Singapore, Malaysia, Japan, Australia, Bangladesh, Thailand, India, Hong Kong and Myanmar.

The Second Asia Pacific Primary Care Research Conference achieved what it set out to do, namely, attracted attendance to a Primary Care Research Conference by making it a positive experience, just like the predecessor Conference in Melaka, Malaysia in 2009. There was research learning and teaching: the four tracks of research learning and teaching in this Conference were successful. The presentations of the output from the Research Championship bore the effects of the polishing by the coaches in each of the four presentations made. We look forward to the research questions being answered by the next Research Conference in 2011 which will be hosted by Malaysia.

Congratulations go to the winners of the Second Asia Pacific Primary Care Research Competitions. The presentations had much to tell about what has been found about Primary Care practice. (2) Wonca Cebu 2011 The 2011 Wonca Asia Pacific Regional Conference was held on 20 - 24 February 2011 in Cebu, Philippines. The theme was “Paradigms in Family Medicine: Bridging Old Traditions with New Concepts”. Wonca Cebu was unique in that it was a “four-in-one” activity: 18th Wonca Asia Pacific regional conference, 10th Wonca Rural Health world conference, the Golden Anniversary of the Philippine Academy of Family Physicians (PAFP) and the Silver Anniversary of the Foundation for Family Educators. Of special note at this Conference was the paper submitted by Dr Tan Ngiap Chuan entitled “Treat to target approach in managing modifiable risk factors of patients with coronary heart disease in primary care setting in Singapore. What are the issues” won second prize in the Wonca Asia pacific and Chinese Taipei Association Research Contest, Oral category. Congratulations go to Dr Tan Ngiap Chuan.

BILATERAL ACTIVITIES IN FAMILY MEDICINE TEACHING, RESEARCH AND PRACTICE

(1) Reciprocal recognition of the FRACGP and the FCFP(S) The memorandum on the reciprocal recognition of the FRACGP and the FCFP(S) was signed on Wednesday, 6 October 2010 at the Cairns Convention Centre between The Royal Australian College of General Practitioners and the College of Family Physicians Singapore. The signing ceremony celebrates the decision of reciprocal recognition of the fellowships of the two Colleges, and leads to the award of the FRACGP by Fellowship ad eundum gradum to those with FCFP(S) who wish to apply and are working in Australia. The signing ceremony in Cairns was witnessed by the Council members of the RACGP and heads of delegations from sister Colleges attending the Annual Scientific Meeting and Academic session of the RACGP. As President I paid tribute to the contributions of the leaders in the RACGP who were instrumental in helping the Singapore College to be formed and also in the development of the academic programme in general/ family practice in Singapore. The College of Family Physicians Singapore was also represented by Associate Professor Tan Boon Yeow.

Professor Wesley Fabb, former CEO of Wonca and his wife Marian were also present. Professor Fabb gave a brief history of the long relationship between the Australian and Singapore Colleges.

The four signatories to the memorandum were: Dr Chris Mitchell, President of RACGP, Associate Professor Jan Radford, Censor-in-Chief of RACGP, Associate Professor Goh Lee Gan, President of College of Family Physicians Singapore and Associate Professor Tan Boon Yeow, Censor-in-Chief of College of Family Physicians Singapore.

Through this memorandum, holders of the FCFP(S) who wish to apply for the FRACGP and are working in Australia can do so. Holders of the FRACGP as we understand will be recognised to be eligible to be in the Family Physicians’ Register when this is set up by MOH later this year.

The new Undergraduate programme in the Yong Loo Lin School has made a quantum leap with its new MIII Programme. There were 5 postings in MIII namely, Family Medicine, Medicine, Paediatrics, Orthopaedics, and General Surgery. Each posting is 8 weeks in length. The students are divided into 5 batches (Batches A, B, C, D, E) and the batches went through the postings as a round robin. We will be completing Batch E of cohort 2010/2011 MIII, as of 7 April 2011. The components in the 8 weeks Family Medicine for MIII are: FP clinic – 2 weeks • Polyclinic – 2 weeks • Community Hospital & Rehab Unit in acute hospitals – 1 week • Palliative Care – 1 week • Community Medicine – 1 week • End of Posting Test Family Medicine is one of the 5 disciplines in the MIII end of year examination. This examination consists of 12 MEQs, 120 MCQs, and a clinical examination of 1 clinical reasoning station, 4 clinical skills stations and 1 clinical skills investigations station. Note that family medicine is now a posting of 8 weeks which of the same length as the other rotating disciplines in MIII. Thanks are due to the FM Tutors in the FP Clinics, in the Polyclinics, as well as trainers in the community hospital, palliative care, as well as rehab medicine.

We are pleased to report that the College is on a firm financial footing during the period 1 April 2010 to 31 March 2011.

Revenue from our courses and seminars contributed to 46.7% of our total revenue. Sponsorships contributed 23.2% to our total revenue while subscriptions contributed another 15.5%.These 3 items together account for 85.4% of our revenue.

College recorded an accumulated surplus of $ 401,504 brought about by a gross revenue of $1,403,379 and an expenditure of $ 1,001,875.
