11. Community Services (Awareness Camps/Health camp/Social Service/ Blood Donation/ Training Programme) Three Health Centers are attached with Department of Community Medicine:

I. Primary Health Centre, Fatehpur Beri

PHC, Fatehpur Beri is located at a distance of 20 km from VMMC. It caters to a population of 73,000.

Services provided:

1. GENERAL OPD SERVICES OPD Services at PHC Fatehpur Beri are provided 6 days a week. Wide spectrums of illnesses are treated by the permanent Medical Officers of the UPHC and Community medicine residents working in collaboration.

2. DENTAL OPD Dental OPD is held six days a week.

3. EYE OPD ● Held once a week by Rajendra Prasad Center, AIIMS. ● One optometrist come to the centre providing vision testing. Patients with cataract are taken for corrective surgery. ● One Doctor also comes once a month for fundal testing and other examinations among at risk patients. ● 4. MATERNAL & CHILD HEALTH SERVICES ● We have a dedicated Maternal and Child Welfare Centre. ● Regular supervised visits with multiple basic investigations are available for antenatal women like urine and blood investigations. ● Under-5 children are also treated in the clinic. ● Various contraception facilities are available for both men and women such as condoms, OCPs, CuT insertion. ● Counseling services concerning contraception, family planning are also available. ● Well Baby Clinic is held two days in a week forNutritional status and growth assessment of under-5 children, counseling of mothers regarding feeding and child care, follow-up of malnourished children.

5. ANIMAL BITE CLINIC Providing assessment of animal bites, provision of first aid, anti-rabies vaccination and TT vaccination to the patients.

6. NON-COMMUNICABLE DISEASE (NCD) CLINIC Opportunistic screening for Diabetes and Hypertension is done for early diagnosis and management. Along with screening, the patients of Diabetes and Hypertension are also followed up in the NCD clinic. Lifestyle modification counseling is provided to all patients and emphasis given for diabetes and hypertension.

7. DOTS CENTER The DOTS centre in the UPHC provides the services of sputum microscopy, distribution of ATT and maintenance of records.

Apart from the above mentioned services, other activities conducted at the centre are as follows:- 1. HEALTH TALKS Weekly public health talks on the basis of the needs of the community are delivered by the interns posted at the centre to the patients and their families present at the centre. 3. Health assessment and screening services to students of schools in Fatehpur area. We have recently started health assessment and screening services for students of schools in Fatehpur area. In this we are doing general body check-up and taking anthropometric measurements. We also provide proper treatment and counselling to the students in case of any health problem diagnosed.

4. OUTREACH ACTIVITES • Medical camps o Multiple health camps conducted in collaboration with MCW Centre at Kharak, Chandan Hola, Bhati. • Field visits o House to house visits in Kharak and Chandan Hola villages for counseling and motivating families showing resistance to immunization of children. • Population surveys of the villages attached to the PHC.

5. TEACHING AND TRAINING • MBBS students, MD students and Interns are trained in multiple facets of health care at primary health centre. • Weekly case discussion is done with interns and PGs posted at the centre, moderated by senior resident. • Interns of every batch are oriented with various national program and visits are organized at malaria office and DOTS center for practical training.

II. Rural Health Training Centre, Najafgarh Rural Health Training Centre (RHTC), Najafgarh is located at a distance of 35 km. from VMMC and it caters to a population of 1, 60,000.

Services provided:

2. OPD services:

General OPD- O.P.D services at RHTC Najafgarh are provided 6 days a week. Wide spectrums of illness are treated by the permanent medical officers of the RHTC and community medicine residents working in collaboration.

Special OPD Clinics run: Orthopaedics OPD, Paediatrics OPD, Ophthalmology OPD, Gynaecology OPD, Dermatology OPD.

3. Maternal and child health clinic: We have a dedicated Maternal and Child welfare centre. Regular supervised visits with multiple basic investigations are available for antenatal women like urine and blood investigations. Various contraception facilities are available for both men and women such as condoms, OCPs, CuT insertion. Counselling services concerning contraception, family planning are also available.

4. Growth monitoring of under-five children: anthropometric measurements are done for all children attending the immunisation clinic, and the malnourished children are followed up, every week with help of ASHA workers.

9. DOTS Centre: The DOTS centre in the RHTC provides the services of sputum microscopy, distribution of ATT and maintenance of records.

New Services Estabilished

10. NCD Clinic (Non-Communicable Disease Clinic)

We have extended our services in the field of Non-Communicable disease by setting up a New Clinic exclusively for Screening and treatment of NCDs. It was inaugurated by additional district magistrate of that region on 23.02.2018. Regular monitoring of blood pressure and capillary blood glucose of patients with hypertension and/or diabetes mellitus is being done at the NCD clinic daily. Counselling for lifestyle modification is done for every patient. Opportunistic screening for hypertension, diabetes mellitus, oral cancer and breast cancer at the NCD clinic in the centre among all individuals 30 years of age or more attending the RHTC, in accordance with NPCDCS guidelines, is being carried out daily.

11. Weekly Yoga sessions

With help of a local yoga teacher, we started weekly yoga sessions at RHTC, Najafgarh for patients visiting OPDs, keeping in view the idea of holistic health.

Field practicing area and Outreach activities:

There are 33 villages/colonies under PHC Najafgarh. Following are the area we consider to practice outreach activities –

1. Health cum immunization camp at slum Laxmi Nagar

2. Well baby show at Mitraon to promote awareness about nutritional status of under five children in community.

3. Health camp cum awareness campaign on family planning to promote reproductive health at Roshanpura.

4. NCD awareness cum screening camp at Chhawla subcentre, Najafgarh, Delhi.

5. Adopting Village Digupura - Digupura is a small village located about 4 kms from RHTC, Najafgarh, with 705 residents. Collaborating with the ASHA worker, Shakuntala ji, we have conducted a baseline demographic survey. While the immunization coverage is close to 95%, there are still 25% home deliveries. Data also indicates that there is a hidden burden of NCDs. Based on survey findings, the deficiencies were identified and the population was sensitized about substance use disorders, importance of immunization and other relevant topics. Further activities including screening for non-communicable diseases are planned for coming near future.

6. HEALTH CAMPS FOR UNDERPRIVELEGED CHILDREN With logistic and drug support from RHTC, Najafgarh, residents of VMMC conducted health camps in The Sweet Home Children’s Village and in Wal AsrImamia Trust (WAIT) Home for orphan boys.

The Sweet Home Children’s Village is run by an NGO where orphaned girls and children from broken families are residing. We conducted monthly health check- ups for the 70 or so children living there and educated them on good health and hygiene practices. Anaemia, refractive errors, scabies and URTI are the most common illnesses we encountered there.

The WAIT Home for Orphan boys takes care of the residence, education and development of 31 boys from broken families. We conducted monthly health check up of the children and inspected their premises. We also gave health education about personal hygiene, handwashing and cough etiquettes.

7. School health component

Our team did a preventive health check-up of students of Govt. Girls Senior Secondary School of Najafgarh for all the students of class 9th to 12th.

We also conducted a sensitization programme on “Health assessment of school children by school teachers” in the same Govt. school for teachers.

III. Urban Health Centre, Aliganj The UHC at Aliganj is operational since February 2012, and is situated 2 kilometres away from Safdarjung Hospital. It caters to a population of approximately 6500, comprising mainly of migrants.

b. Rally to increase awareness regarding cleanliness under Swatch Bharat Abhiyan involving the community people and healthcare workers. c.

d. House to house survey for breeding sites inspection along with interns posted under the supervision of health inspector.

PHC, Fatehpur Beri is located at a distance of 20 km from VMMC. It caters to a population of 73,000.

Services provided:

1. General OPD Services - OPD Services at PHC Fatehpur Beri are provided 6 days a week. Wide spectrums of illnesses are treated by the permanent Medical Officers of the UPHC and Community medicine residents working in collaboration. 2. Maternal & Child Health Services • We have a dedicated Maternal and Child Welfare Centre. • Regular supervised visits with multiple basic investigations are available for antenatal women like urine and blood investigations. • Under-5 children are also treated in the clinic. • Various contraception facilities are available for both men and women such as condoms, OCPs, CuT insertion. • Counseling services concerning contraception, family planning are also available. 3. Age Friendly Clinic - On 19th July 2019, We inaugurated our new Age Friendly clinic. This is not just beginning of new health services, also opening the gate of research and training opportunities on geriatric health. The following services are provided in our clinic - 1. Screening for Depression 2. Screening for Elderly abuse 3. CVS Risk prediction 4. Anemia screening 5. Nutritional assessment 6. Hearing Assessment 7. Oral Health Screening and Management 8. 4. Non-Communicable Disease (NCD) Clinic – Opportunistic screening for Diabetes and Hypertension is done for early diagnosis and management. Along with screening, the patients of Diabetes and Hypertension are also followed up in the NCD clinic. Lifestyle modification counseling is provided to all patients and emphasis given for diabetes and hypertension.

5. Eye OPD • This year UPHC started refractory error screening on daily basis. • Additionally Eye OPD held once a week by Rajendra Prasad Center, AIIMS. • One optometrist come to the centre providing vision testing. Patients with cataract are taken for corrective surgery. • O ne Doctor also comes once a month for fundal testing and other examinations among at risk patients.

6. Well Baby Clinicis held two days in a week for Nutritional status and growth assessment of under-5 children, counselling of mothers regarding feeding and child care, follow-up of malnourished children.

7. ANIMAL BITE CLINIC - Providing assessment of animal bites, provision of first aid, anti-rabies vaccination and TT vaccination to the patients.

8. DOTS CENTER -The DOTS centre in the UPHC provides the services of sputum microscopy, distribution of ATT and maintenance of records. 9. Dental OPD - Dental OPD is held six days a week. Apart from the above-mentioned services, new activities were started this year at the centre are as follows:-

1. Teen talk: An Adolescent Health education Programme • The aim of this programme is to provide facts & accurate information to help you make healthier decision. 2. “Know your Numbers” : Community level heart health Screening Campaign • World hypertension day 2019 and beginning of our new campaign titled "Know Your Number" which is a community level heart health Screening Campaign • “Know your Numbers,” which refers to key markers of heart health like blood pressure, cholesterol, blood sugar, body mass index (BMI), and it’s important that you know your numbers and understand what they mean.

3. Healthy & Safe Elder: Our Mission Our Geriatric Screening & Preventive Health care outreach services is being organised every month in various old age homes in our field practicing area. We also encouraged them to utilize our Geriatric Health Services at Fetehpur beri, UPHC

4. Well baby Show Once in 6 Months, Department of Community Medicine organise "Well Baby Show" at Fatehpur beri UPHC involving ASHA & ANM of 11 villages. The participating children were subjected to general clinical examination and evaluated for nutritional status and immunization and hygiene. NHM Assessment Criteria is used to assess children aged between 0 to 6months, 6 to 1yr & 1yr to 3yr category. The screening of the children accompany by the 'Smart Moms' contest, wherein mothers of the participating children were evaluated for their knowledge of common childhood diseases, their prevention and home remedies. Mother's Quiz competition are designed using MCP Card information. Health talk is designed using IYCF Guidelines. Program was aligned with every year theme of Breastfeeding week like “Empower Parents, Enable Breastfeeding"

5. Annual Diabetes Health Fair On the occasion of World Diabetes Day every year we organise a Diabetes Health Fair in which we invited our Diabetes patient with their family. Family support in diabetes care has show to have a substantial effect in improving health outcomes for people with diabetes. We also hold fitness contest in order to promote physical activity as it is one of the most important things you can do to manage and live well with diabetes. We create a Wall against Diabetes where Diabetes patient with their family asked to click a selfie using WDD app (Blue Circle app) and share it in social media - We make different station to provide the following services: • Table 1- Screening (RBS, BP, Neuropathy, BMI, Hb) • Table 2 - Diabetes Education • Table 3 - Diabetes Lifestyle Modification councelling • Table 4 - Diabetes Home Care • Table 5 - Diabetes Nutrition Care • Table 6 - Diabetes Foot Care • Table 7 - CVA Risk assessment/Diabetes Risk assessment • This year we our interns presented Diet demonstration for diabetic patients & we also reward best intern team.

6. Case Discussion In addition to the outreach activities and field work, it is important to keep in constant touch with the essence of patient treatment and care in the OPD, for an all-round provision of preventive and curative services. For this purpose, every week, a suitable patient from the General OPD/NCD clinic/Geriatric clinic/Antenatal clinic/Eye OPD at UPHC, Fatehpur Beri, is identified by a PG for further discussion. The intern takes a detailed history of the patient and performs general and systemic examination of the patient and prepares the case in the standard format of clinical examination. This is an essential part of their training as physicians and clinicians of the future, as history taking and clinical examination are still the most important parts of reaching a provisional diagnosis. The intern presents the case, in the presence of the patient, beginning with the history of the patient. The presentation is moderated by a PG and discussed among junior and senior residents. This is followed by general and systemic examination and demonstration of examination methods by the intern and PGs. Differential diagnoses are thoroughly discussed and points in favour of and against each diagnosis is discussed. Appropriate treatment is prescribed and referral is initiated, if found necessary. All of this makes the exercise beneficial for the patient, as well.

7. Intern Orientation to various Health programmes As an Intern in Community Medicine, it is essential to have complete knowledge about the various National Health Programmes and their objectives, and how they are implemented at various levels of health care. The Intern must be able to carry out all the activities related to programme

implementation at the community and primary care level.

8. Health talk Weekly public health talks on the basis of the needs of the community are delivered by the interns posted at the centre to the patients and their families present at the centre. Multiple topics have been covered such as: 1. Vector borne Disease Prevention and Control 2. Breast Feeding, its importance and Technique 3. Rabies 4. Dengue and Chikungunya 5. Non-Communicable diseases 6. Family planning 7. Reuse of cooking oil 8. Nutrition

10. Health assessment and screening services to students of schools in Fatehpur area. We have recently started health assessment and screening services for students of schools in Fatehpur area. In this we are doing general body check-up and taking anthropometric measurements. We also provide proper treatment and counselling to the students in case of any health problem diagnosed.

11. OUTREACH ACTIVITES • Medical camps o Multiple health camps conducted in collaboration with MCW Centre at Kharak, Chandan Hola, Bhati. • Field visits o House to house visits in Kharak and Chandan Hola villages for counseling and motivating families showing resistance to immunization of children. • Population surveys of the villages attached to the PHC.

TEACHING AND TRAINING • MBBS students, MD students and Interns are trained in multiple facets of health care at primary health centre. • Weekly case discussion is done with interns and PGs posted at the centre, moderated by senior resident. • Interns of every batch are oriented with various national program and visits are organized at malaria office and DOTS center for practical training.

II. Rural Health Training Centre, Najafgarh Rural Health Training Centre (RHTC), Najafgarh is located at a distance of 35 km. from VMMC and it caters to a population of 1, 60,000.

14. Maternal and child health clinic: We have a dedicated Maternal and Child welfare centre. Regular supervised visits with multiple basic investigations are available for antenatal women like urine and blood investigations. Various contraception facilities are available for both men and women such as condoms, OCPs, CuT insertion. Counselling services concerning contraception, family planning are also available.

15. Growth monitoring of under-five children: anthropometric measurements are done for all children attending the immunisation clinic, and the malnourished children are followed up, every week with help of ASHA workers.

III. Urban Health Centre, Aliganj The UHC at Aliganj is operational since February 2012, and is situated 2 kilometres away from Safdarjung Hospital. It caters to a population of approximately 6500, comprising mainly of migrants.

• World rabies Day- Celebrated on 24th September 2019 in Sewa nagar MCD School and 27th September 2019 at centre. Awareness sessions were conducted highlighting importance of post exposure prophylaxis of animal bites. • World heart Day – Celebrated on 29th September 2019 in Aliganj MCD School highlighting importance of healthy dietary habits.
