We looked for it in the condom section of a large drugstore, where a small display directed us to the pharmacist, who gave us a kit from a large stack on his counter. The instructions were clear and easy to follow. The educational materials were brief but •• accurate. A finger was pricked with the . small lancet provided and three drops of blood were collected on a cardboard card. The card was then placed in a foil mailer (postage pre-paid). Another card with an identification number and a 1-800 number was retained so that the results could be obtained. We were instructed to call back in one week. Counseling is available in both English and Spanish. One week later, my patient called and punched in his ID number. Since I was concerned about the possibility of errone- ously punching in the wrong number and getting someone else's results, we tried entering several numbers that were off by one digit. We were told by computer that they were not valid. The system has safe- guards. My patient did eventually speak to a very empathetic counselor who informed him that he was HIV-positive. She did a good job of providing emotional support and of assessing his understanding of what the results meant. He was encour- aged to call back if he had further ques- tions. Since my patient claimed not to have a physician, he was referred to state and local resources for further treatment. At the completion of the counseling session, a voluntary poll was conducted which con- sisted of questions about sexual orienta- tion, drug use and the motivation that led the caller to be tested, among others. The entire phone call took about 35 minutes. 'Regardless of the method chosen, the reasons to be tested for HIV are com- pelling. Much can and should be done for those who test positive. Those who test negative should educate themselves and do everything possible to stay that way. I am optimistic that we will eventually be ~ble to completely suppress HIV and avoid Its devastating consequences.
