Box 1346 Ann Arbor, Ml 48106-1346 ABSTRACT The work described in this thesis evaluates the role of noninvasive magnetic resonance and nuclear medicine techniques in the investigation of focal epilepsy of childhood, and more specifically in the presurgical evaluation of children who are potential candidates for surgery. Seventy five children were prospectively investigated after referral for assessment and investigation of intractable focal epilepsy. Initial clinical localisation was made on the basis of full clinical assessment with EEG. All underwent magnetic resonance imaging. Abnormalities were found in 88% of children, concordant with the seizure focus in 80%. Further magnetic resonance techniques were assessed with regard to latéralisation of temporal lobe epilepsy (TLE). T2 relaxometry of the hippocampi and proton magnetic resonance spectroscopy (^H MRS) of the mesial temporal lobes were demonstrated to be lateralising in a high proportion of these children, and also showed a high rate of bilateral abnormality. In order to relate these structural abnormalities to the epileptogenic focus, the technique of ictal single photon emission computed tomography (SPECT), using Tc'" HMPAO as a marker of cerebral blood flow, was developed for this study. Focal hyperperfusion was seen in 72% of ictal scans when compared to interictal scans, concordant with the seizure focus in all. Where no perfusion change was noted after an ictal injection, slowing of the EEG was present at the time of injection consistent with ischaemia, suggesting rCBF does not increase in all focal seizures. Correlation of interictal SPECT scans with MRS of the mesial temporal regions in children with TLE demonstrated correlation between neuronal loss or dysfunction and hypoperfusion. Where similar damage on the two sides was demonstrated by MRS, no asymmetry of interictal perfusion was seen. These findings have led to a better understanding of the underlying pathophysiology of intractable focal epilepsy of childhood and have resulted in an increase in the number of children evaluated for epilepsy surgery. The thesis concludes with a discussion of an optimised noninvasive strategy for presurgical evaluation in children. DECLARATION I, Judith Helen Cross, declare that this thesis is all my own work. All ictal EEG’s were initially read by me, with subsequent verification of the report, and review of all interictal EEG’s with the Consultant Neurophysiologist, Dr Stewart Boyd. The magnetic resonance images and spectroscopy were performed and processed by the research radiographer. Miss Cheryl Johnson, but I reviewed all the magnetic resonance images in conjunction with the independent observer. Dr Graeme Jackson. All but a small number of ictal injections were given by me; the remaining injections were given by the Epilepsy Liaison Nurse, Sister Alison Harper. I processed and read all ictal, postictal and interictal scans and subsequently analysed the results, and performed the correlative studies with MRI, EEG and *H MRS. The clinical relevance of certain subgroups of the International League Against Epilepsy (ILAE) classification scheme remains to be determined. The aim of the present work is to assess the effect of the focal cortical dysplasia type Ib and Ic histologic subtypes on surgical outcome with respect to seizure frequency. This study also provides an opportunity to compare the predictive value of the ILAE and Palmini et al classification schemes with regard to the type I focal cortical dysplasias. We retrospectively reviewed 91 focal cortical dysplasia patients (55% female; median age: 19 years (interquartile range 8–34); median seizure duration: 108 months (interquartile range 36–204)) with chronic epilepsy who underwent surgery. We compared the pathological subtypes, evaluating the patients’ post-surgical outcome with respect to seizure frequency according to the Engel’s classification and the ILAE outcome classification. Both the ILAE classification scheme and Palmini et al classification scheme were utilized to classify the histologic subtype. Using v2 and Fisher’s exact tests, we compared the post- surgical outcomes among these groups. Of the 91 patients, there were 50 patients with ILAE focal cortical dysplasia type Ib, 41 with ILAE focal cortical dysplasia type Ic, 63 with Palmini et al focal cortical dysplasia type IA, and 28 with Palmini et al focal cortical dysplasia type IB.

Nine patients experienced partial seizures, and in seven a waking electroen- cephalogram showed focal or lateralising abnormalities. Astrocytoma was the most common tumour histology. The delay in tumour diagnosis from the onset of seizures ranged from 2 weeks to 2 years with a mean of 6 months. Complete resection of the tumour was the only treatment in three patients and four underwent resection followed by radiotherapy and/or chemotherapy. Two patients died. Using a case-control study design, the authors retrospectively compared seizure outcomes after VNS therapy in patients with PTE versus those with nontraumatic epilepsy (non-PTE) who were part of a large prospec- tively collected patient registry. Results. After VNS therapy, patients with PTE demonstrated a greater reduction in seizure frequency (50% fewer seizures at the 3-month follow-up; 73% fewer seizures at 24 months) than patients with non-PTE (46% fewer seizures at 3 months; 57% fewer seizures at 24 months).

They are associated with refractory epilepsy. The typical seizure type is “Gelastic”. HH can produce different types of seizures. The epileptogenesis has been discussed focusing in “hamartoma- centrism” and “extra hamartoma epileptogenesis”. Objectives: To determine the pre-surgical aspects and surgical techniques with the best outcomes in HH associated to refractory epilepsy. Methods: We studied pediatric patients with HH who underwent epilepsy surgery between 2004 and 2017. We analyzed the type of seizures, neurological comorbidity, type of hamartoma in MRI, the electroencephalography records, type of surgery and outcome according to the Engel’s and ILAE’s classification. We also compare the neuropsychological evaluation before and after surgery. Results: 7 cases fulfilled inclusion criteria, the ages of starting symptoms vary since 4 months to 7 years old; the mean time of surgery was 5 years. 14% of cases had Gelastic semiology as the first seizure, 57% experienced gelastic seizures and 85% of the cases had at least one generalized seizure. 71.4% were classified as wide implantation and 28.6% as pedunculated.

There are few reports about EEG in children with MMD in China till now. This study is aimed to analyze the electroencephalographic features of MMD in pediatric patients in China preliminarily. Methods: Pediatric patients with MMD who were hospitalized in Peking University International Hospital and Beijing Tiantan Hospital from January 2016 to December 2018 were collected. Clinical and electroencephalography (EEG) findings were analyzed retrospectively. Results: A total of 110 pediatric patients with MMD were involved, and 17 (15.5%) cases had a history of seizure or epilepsy. Ischemic stroke was associated with a 1.62-fold relative risk of seizure. A subset of 15 patients with complete EEG data was identified. Indications for EEG in patients with MMD included limb shaking, unilateral weakness, or generalized convulsion. Abnormal EEG was seen in 14 (93.3%) cases, with the most common findings being focal slowing 12 (80.0%), followed by epileptiform discharge 10 (66.7%), and diffuse slowing 9 (60.0%). “Re- build up” phenomenon on EEG was observed in one patient. Conclusions: Seizure and abnormal background activity or epileptiform discharge on EEG were common in pediatric patients with MMD.
