URGENT- ELECTION SUPPLIES ORDER FORM
For the June 12, 2018 Primary Election

The following list contains election supplies that Municipal Election officials will require to conduct the upcoming Primary Election. The Division of Elections places orders for these items based on municipal requests. If you do not place an order, no supplies will be provided automatically.

Please indicate the number of each item you have on hand, as well as the additional quantity of each item that you will need for the June election.

Even if you do not need any supplies, please complete the form (on the reverse) and submit by the deadline. All boxes need to be filled in. Enter a zero for any of the items that you do not need.

Description of Items included on this Order Form:

Absentee Envelopes: Your envelope supply should include only the four envelopes identified as Civ-O (Civilian Outside), Civ-I (Civilian Inside), Mil-O (Military Outside), and Mil-I (Military Inside). Any envelopes with a revision date earlier than the dates provided on the supply form should be destroyed.

NOTE: The Division of Elections issues State absentee ballots to all UOCAVA (Uniformed Service and Overseas Civilian) voters. Municipal Clerks need to issue municipal absentee ballots to UOCAVA voters if a municipal election is also being held on June 12, 2018. If your municipality is not conducting a municipal election in June, you do not need to order any Mil-O or Mil-I envelopes.

DS200 Ballot Marking Pens and Thermal Tape: Please test all pens to ensure that they work. The DS200 tape is thermal paper; please do not order any more than what you will need for the June Election. Thermal paper can be affected by the environment, so it is not a good idea to keep it stored for extended periods of time. A fresh roll of thermal tape for each tabulator that you use will be enough for this election. Any tape currently on hand can be used for testing or training. Install a fresh roll of tape for Election Day.

(NEW Item) White Voter Registration Cards: These cards are now being provided prior to each election to ensure that you have the most current version. The white cards are for in-person registrations completed in the registrar’s office. Please order enough so that your supply will carry you until the General Election in November.

Tamper Proof Seals: Your supply of tamper proof seals for securing state blue boxes should be either be blue or red numbered seals. You should maintain a sufficient number to secure each blue box, plus a couple extras.

Certificate of Registration Forms (COR): This two-part form is used once you have printed your Incoming Voting List and on Election Day for changes and new registrations. Please order only what you will need for the upcoming June Election.

ExpressVote Ballot Cards: These are the blank 14-inch thermal cards used to print ballots produced by the Express Vote Accessible Voting System (AVS). Remember, you will also need ballots for pre-election testing.
