Dance Team Uniform Information & Order Form
· All dance orders are due Tuesday, October 18th. If you do not have your order turned in by this due day, the uniforms will not be here in time for the first game and pep rally. The order will not be placed until every dance team member has turned in their order.
· A brief parent meeting will be held on Tuesday, October 18th immediately following practice in Mrs. Hastings’ classroom (RM 332).
· Please fill out the entire order form.
· Thank you in advance for making sure to adhere to the order schedule.
· Additional required uniform items:
o Black dance shoes (These can be purchased at any dance supply store.)
o Black racer back sports bra
