Case Report Familial Hyperinsulinism due to HNF4A Deficiency and Benign Premature Adrenarche: A Case Report Edward Compton,1 David H. Geller,2 Alaina P. Vidmar MD.3 Abstract Background: Familial Hyperinsulinism due to HNF4A deficiency (FHI-HNF4A) is a form of diazoxide-sensitive, diffuse hyperinsulinism, characterized by transient or persistent hyperinsulinemic hypoglycemia, and a propensity to develop Maturity-Onset Diabetes of the Young type 1 (MODY1). The association between FHI-HNF4A deficiency and benign premature adrenarche (BPA) is unknown. The Case: We report the case of a 5-year-old girl with FHI-HNF4A, controlled on diazoxide, who presented with BPA and Tanner stage 3 pubic hair associated with body odor and acne. Work-up revealed elevated dehydroepiandrosterone sulfate (DHEAS), elevated free testosterone, and advanced bone age. Insulin levels were elevated in the setting of normal fasting blood glucose. We discuss the possible hormonal underpinnings of hyperandrogenism. Conclusion: Though the underlying pathophysiology of this phenotype is unclear, a possible synergistic mechanism exists between insulin-induced hyperandrogenism and HNF4A deficiency leading to a transient decrease of SHBG and thus increased free testosterone levels. Further investigation is required to determine the association between HNF4A dysfunction and BPA. MARSHALL, M.D. University of Pretoria, Pretoria The term "erythema nodosum leprosum" has frequently appeared in the literature on leprosy since the 1930's, apparently popularized by the Japanese (9). The occurrence of "erythema nodosum" in leprosy had previously been noted at the end of the nineteenth century by Brocq (6) and by Hansen and Looft (8), but the term seems to have fallen into disuse. As will become clear from our clinical and histological descriptions of the phenomenon commonly known as erythema nodosum leprosum, we believe the title to be a misnomer. We suggest that it would be more accurate to call it panniculitis nodosa leprosa (P.N.L,). This condition is commonly confused with acute lepra reaction, both processes being referred to as "acute reactions"; but a sharp line of distinction must be drawn between them. P.N.L. occurs only in patients with lepromatous leprosy as an acute, subacute or chronic skin eruption, with or without fever. It is characterized by showers of dusky red nodules, 0.5 to 2 cm. in diameter, on the extensor surfaces of the limbs, on the face, and, less often, on the trunk. The number of lesions appearing in an attack varies from a few to several hundreds, and they sometimes coalesce to form plaques. The nodules may be painful, and they are usually tender to touch. -

Study Design: Randomized controlled trial. Place and Duration of Study: Dermatology department, Pak Emirates Military Hospital Rawalpindi, from Oct 2016 to Apr 2017. Material and Methods: Sixty patients of Pityriasis versicolor, both male and female were included in study. Diagnosis of Pityriasis versicolor was made clinically and confirmed microscopically by examining skin scrapings for fungal hyphae. Patients with concomitant systemic illnesses or those who had received anti-fungal in last three months were excluded from study. Random number tables were used to allocate patients to the two treatment groups. Group A received 2% ketoconazole shampoo twice per week for four weeks plus topical 1% clotrimazole twice daily application for 2 weeks. Group B received only topical therapy with 1% clotrimazole cream applied twice daily for 2 weeks. Assessment of treatment efficacy was done by clinical examination of patient and microscopy of skin scrapping for fungal hyphaedone at baseline and at end of study (4 weeks of treatment). A negative clinical examination and negative skin scrapping for fungal hyphae was considered effective therapeutic response. -

HAZARD IDENTIFICATION EMERGENCY OVERVIEW: Carbon Dioxide is a colorless, odorless, non-flammable gas. Over-exposure to Carbon Dioxide can increase respiration and heart rate, possibly resulting in circulatory insufficiency, which may lead to coma and death. At concentrations between 2-10%, Carbon Dioxide can cause nausea, dizziness, headache, mental confusion, increased blood pressure and respiratory rate. Exposure to Carbon Dioxide can also cause asphyxiation, through displacement of oxygen. If the gas concentration reaches 10% or more, suffocation can occur within minutes. Moisture in the air could lead to the formation of carbonic acid which can be irritating to the eyes. SYMPTOMS OF OVER-EXPOSURE BY ROUTE OF EXPOSURE: The most significant routes of over-exposure for this gas are by inhalation, and contact with the cryogenic liquid.
