POSITION TITLE: CLINICAL CARE COORDINATOR

GENERAL ACCOUNTABILITY:

Reporting to the Director Care Services, the primary purpose of the position is to assist the Director Care Services in achieving the departmental goals and objectives.

REPORTING STRUCTURE:

The incumbent reports directly to the Director Care Services for all aspects of the operation and management of the Care Services Department. Reporting to the Clinical Care Coordinator is the Manager Activation / Volunteer Services and the Coordinator Pastoral Care. The Clinical Care Coordinator will work with senior care services staff and is also responsible to maintain active liaison with residents and families.

SPECIFIC ACCOUNTABILITIES:

The specific accountabilities of the position require the Clinical Care Coordinator to:

·  Provide leadership for all nursing staff to effectively and efficiently provide care services to the residents.

·  Utilizing the Quality Management Process develops plans and implements strategies in areas requiring improvement or change.

·  Responsible for the clinical component of the Admissions process including the completion of the LTCF initial assessments, liaison with residents, families and outside agencies.

·  Effectively liaison with learning institutions to arrange clinical placements from university, community colleges, high schools, work experience programs, etc.

·  Participates in research as directed.

·  Develops implements and evaluates clinical education plans and programs, both internal and external to York Care Centre including the Back in Form training program.

·  Manages the Infection Control program and provide support to Human Resources in respect to staff health.

·  Assists the Director Care Services with clinical operations and acts in their absence.

·  All other related tasks as assigned by the Director Care Services.

DECISION MAKING:

In consultation with the Director Care Services, the incumbent is responsible to make decisions that provide for the effective planning, preparation and execution of all aspects of the Care Services department. Further, the incumbent is responsible to make decisions that result in the evaluation, development and implementation of clinical education plans for care services personnel.

POSITION QUALIFICATIONS:

·  Successful completion of Baccalaureate of Nursing Degree.

·  A minimum of 5 years of progressive nursing experience in health care services where leadership administrative ability and competency as a practitioner of nursing have been demonstrated.

·  Demonstrated exemplary character.

·  Strong organizational, communications and multi-tasking abilities.

·  Current registration with NANB.
