Madryn House has facilities for the disabled with a ramp to the front entrance and disabled toilet.

New patients

To register with the practice you should complete an application form.

When you register with the practice you will be asked to fill in a simple medical questionnaire about yourself.

Data Protection

Yourmedical records are held in the strictest confidence. Information is not passed on without your consent unless it is within the confines of the NHS, by legal framework, or is in the public interest. Certain anonymised patient data may be shared for the purpose of public health and audit, research, teaching and training. This practice is registered under the Data Protection Act. It is a practice and legal requirement that allstaff maintain confidentiality of patients' records.

Visits

Home visit requests should be made before 10.00am if possible.

Cancellation of appointments

Please inform the surgery if your booked appointment is no longer needed so that it may be offered to another patient. Cancellation of appointments (more than 24hours in advance) can also be done on-line.

Nurse Triage

Calls must be made between 8am 11am for medical advice or an emergency appointment. Your call will be returned by our triage nurse.

Please do not turn up at surgery to be seen as you may have to wait until the end of the morning.

Out of hours

Between the hours of 6.30pm and 8am, patients in need of emergency medical care should telephone the

G.P. Out of Hours serviceon 0300 123 55 66at GlanClwydHospital.

Our doctors, in common with the great majority of GPs in the area are involved on a rota basis in staffing the G.P. Out of Hours service.

Patients in need of emergency medical care will be asked to attend the G.P. Out of Hours Centre. A small number of people will not be able to attend the centres because of a genuine clinical problem. In this case a home visit will be arranged by the centre. Please try and attend the centre if at all possible.

Repeat prescriptions

Can be obtained by request at the surgery, via your local chemist or on-line. Please use the repeat prescription slip or state in writing your needs i.e. name of medication, strength and daily dosage.

It would be helpful if you could indicate where you wish to collect the prescription.

For reasons of safety we cannot take prescription requests over the telephone.

Prescriptions will be ready within 2 working days. For prescriptions sent by post please allow 5 working days.

Results of investigations

Results may be obtained by telephoning thesurgery between the hours of 3.00pm 5.00pm only.

Change of address

Please inform staff of any change to your address or telephone number so we can amend your records. You can also do this on-line.

Complaints procedure

As part of our terms of service and commitment to improve our quality of care, the practice has an ‘in-house’ complaints procedure. If you have any criticisms regarding any aspect of the care you may have received from this practice, please contact our Practice Manager, Mrs Helen Robbins. Your complaint will be dealt with promptly and confidentially.

Health promotion clinics

These clinics are aimed at improving general health and preventing disease by examination and advice about exercise, diet, smoking and alcohol, as well as closer monitoring of chronic conditions.

The Health clinics are run by the practice nurses in consultation with the doctors.
