Purpose:

To promptly and safely restore circulation and ventilation to prevent irreversible anoxic damage to organ systems.

Policy:

Spooner Health employees will utilize the following Medical Alert – Cardiac Emergency guidelines in the event of a cardiopulmonary arrest involving a patient, employee, volunteer, or visitor.

Procedure:

1.  In the event of unexpected and sudden cessation of heart beat and/or respirations, personnel trained to perform cardiopulmonary resuscitation (CPR) will:

a.  Summon help by the quickest possible means-calling out, paging, etc. Overhead paging will be in clear text format: “Attention: Medical Alert – Cardiac Emergency – (location)” Stated three (3) times.

b.  Initiate and continue basic life support-CPR.

c.  Note mentally the time of discovery.

d.  Continue CPR until relieved by other personnel or until an order to cease efforts is given by a provider.

2.  If the personnel finding the above event is not trained in CPR, he or she will:

a.  Summon help by the most rapid means-calling out, paging system, etc.

b.  Assist in any way possible with other personnel in obtaining supplies or calling personnel.

3.  The following personnel should respond to the Medical Alert:

a.  Any available registered nurse (RN) or licensed practical nurse (LPN).

b.  Any available provider.

c.  Any available Nurse Anesthetist or Respiratory Therapist to assist in ventilation of the patient.

d.  Any available Lab Technician to prepare or receive specimens for blood gases or other labs.

e.  Any available personnel trained in CPR to assist in basic and/or advanced life support.

4.  All other personnel should report to any area outside the department to provide:

a.  Compressions

b.  Lifting help

c.  Traffic control

d.  Obtaining a gurney

e.  Assisting with transport

f.  Holding the elevator

5.  One person shall be in charge of resuscitation procedures. This person shall be one of those present at the scene of the arrest. If a staff provider is present, he/she shall assume charge of the resuscitation procedure, otherwise those medical and/or paramedical personnel present shall quickly and simply identify themselves and their level of life support training and experience. The individual with the highest level of training and experience shall assume the direction and coordination of the resuscitative attempt until a physician arrives. Those personnel certified in Advanced Cardiac Life Support (ACLS) may direct the resuscitation effort in accordance with the most current ACLS guidelines and protocols.

6.  One designated gurney will be used ONLY for medical alerts-Cardiac Emergencies located in hallway to surgery (before surgery no admittance doors). It is non-metallic and also a backboard (MRI safe) and transfer slip (MRI safe) attached @ all times.

GARDEN LEVEL

1. Page “Attention: Medical Alert – Cardiac Emergency – (location)” stated three times.

2. Initiate CPR.

3. All available staff report to area.

3. Second person to arrive takes charge:

·  The other staff that respond will go to rear of elevator where gurney/back board are kept and bring to location.

·  Additional staff to assist with holding elevator door open for transfer to Ed on main level of hospital.

·  Person in charge needs to send extra staff back to their departments if they are not needed.

·  Remaining ED staff to prepare to receive victim and open ED doors.

MAIN LEVEL

1. Page “Attention: Medical Alert – Cardiac Emergency – (location)” stated three times.

2. Initiate CPR.

3. All available staff report to area.

4. ED Staff or person in charge directs one person to bring gurney (non-metallic) and backboard and/or SLIPP to scene and transport to ED for advanced

Care.

5. Remaining ED staff to prepare to receive victim and open ED doors.

INPATIENT SERVICES

1. Page “Attention: Medical Alert – Cardiac Emergency – (location)” stated three times.

2. Initiate CPR.

3. All available staff report to area.

4. Crash cart brought to room from nurses station area.

IMAGING TRUCK FOR MRI SERVICES

1. The MRI Technologist will dial 0 from the MRI truck to have switchboard page the Medical Alert-Cardiac Emergency

2. The Page will be completed as- “Attention: Medical Alert – Cardiac Emergency – (location)” stated three times.

3. Imaging staff will respond directly to the MRI Truck, assisting the MRI Technologist with the patient and controlling access to the MRI truck. No staff will be allowed to enter unless directed by MRI or Imaging staff.

4. The ED staff and/or someone that was directed to bring non-metallic gurney and transfer slip and/or backboard to the MRI entrance only.

5. NO staff will enter the MRI truck entrance without direction from MRI Technologist and/or Imaging staff

6. The patient will be transferred to the non-metallic gurney by the MRI Technologist and Imaging staff using the transfer SLIPP and/or MRI safe back board and taken outside of MRI Truck.

7. When outside of MRI truck staff that responded will immediately take over transfer to ED for advance care.
