We would like to sell / distribute / promote the following items:

NOTE: NO SALE OR DISTRIBUTION OF BEVERAGES. All food items subject to Peace Committee approval. Commit to one or more of the following by initialing in the box: [ ] At our booth, we will donate some / all proceeds to support the Committee. [ ] At our booth, we will share information/increase awareness of the Parade & Festival themes of peace, compassion, and global interdependence. a) THIS FORM IS ONLY AN APPLICATION. YOU ARE NOT GRANTED OR GUARANTEED permission to operate a booth at the festival UNTIL YOU RECEIVE A LETTER OF ACCEPTANCE. b) This application is subject to acceptance or rejection at sole discretion of Committee. Preference will be given to those parties with an interesting, quality product or exhibition in accord with the Mission and Vision statements of the Committee. c) Booth fee is $40 for each 10’x10’ space. If you bring a tent larger than 10' x 10', you will be charged for an additional space and positioned accordingly. d) There will be no refunds in event of bad weather; festival booths will be re-located to the temple social hall, 45- 516 Lehua Street. Vendors shall provide all equipment, labor, materials, (banner, signage, vehicles, tables, chairs, generators, trash bins, etc.), prepare for inclement weather,display all required licenses and documents, and clean up their area before leaving. e) No cooking unless you have a permit from the fire department.

ALL APPLICANTS, FOR PROFIT OR NONPROFIT, must send the following items to: Peace Committee / Festival Booths, Honoka‘a Hongwanji Buddhist Temple, PO Box 1667, Honoka‘a, HI 96727: 1. This application form, completed and signed. 2. $40 Peace Committee Booth Fee for each space. Checks should be payable to: “Peace Committee.” 3. Copy of G.E.T. certificate and, if you are selling food, State of Hawaii, Department of Health food permit.
