EXTENDED AFTER CARE CONTRACT

This contract is entered into on this ______day of ______, 20____, by

and between Mater Dolorosa School and ______.

  1. The parent agrees to pay $10.00 a month (per month August-May) for the use of the Extended AfterCare Program. This $10 entitles the family to5 hours of After Care each month. Any additional hours will be billed to the family at the end of each month at $2.00 per hour per child. After school care hours begin promptly at 2:40pm.
  1. If the family chooses not to use any hours during a month, the $10.00 fee must still be paid in order to keep the family under contract and the students spot in the ExtendedAfterCare Program.
  1. The parent agrees to make this payment on or before the 10th of each month.
  1. The parent agrees to pay a $5.00 delinquent fee if the bill is not paid on or before the 10th of each month. Should the 10th of the month fall on a weekend, the fee is due the Friday before.
  1. The parent agrees to contact the school if some unforeseen financial circumstances should arise.
  1. The Extended After care Program is subject to all rules and regulations in the MDS Parent/Student Handbook. The parent agrees to cooperate with the Supervisor of Extended AfterCare in the event of any behavior problems that may arise. If the behavior problem is not resolved, the parent agrees to meet with the Principal to discuss further action, which may include removal of the child from the Extended AfterCare Program.
  1. The parent understands that any time they pick up their child more than 15 minutes over an hour that they will be charged for an additional hour.
  1. The parent agrees to pick up his/her child/children NO LATER THAN 5:30pm. The parent agrees to pay a LATE FEE OF $10.00 if they pick up child/children after 5:30pm. If your child is picked up late (between 5:30 & 6:30pm) more than one time a semester he/she will not be eligible for after care for the remainder of the semester. Any parent arriving after 6:30 pm will be charged $25.00 and their child will not be eligible for after care. The parent agrees that if they should arrive late without calling to notify the school, their child will not be allowed to continue in the after school care program.
  1. Due to insurance regulations, only children under contract for Extended After Care are allowed to participate. There are no exceptions to this policy.
  1. If any phone numbers change during the year, please be sure to notify Mrs. Melissa Mixon, Day Care Program Director.

WHAT IS AFTER SCHOOL CARE?

Aftercare is a service provided to parents and students who attend Mater Dolorosa Catholic School. Aftercare is available Monday through Friday from 2:40pm until 5:30pm. Children whose parents have completed and signed an Aftercare Contract will be sent to the designated Aftercare rooms at 3:00pm.

WHAT WILL MY CHILD DO IN AFTERCARE?

Students who are contracted to Aftercare will be given a snack and juice provided by our cafeteria after the bell rings at 2:40pm. Students are allowed one snack and one juice per day. Aftercare students areoffered the opportunity to complete their assignments and homework each day during aftercare hours. Students also have the opportunity to socialize with classmates and friends while playing board games such as Chess, checkers, bingo, battleship, trouble, dominos, and multiple card games. Students are encouraged to get involved in art projects and poster projects during their free time in Aftercare.

HOW MUCH DOES AFTER CARE COST?

There is a flat rate of $10.00 a month to hold the students spot in aftercare each month. The hourly rate of $2.00 per hour begins at 2:40pm.

WHERE DO I PICK MY CHILD UP IN AFTERCARE?

There are 2 rooms in the main school building that have been designated as the Aftercare rooms. One classroom is for Pre-K through 3rd where parents may pick up until 4:30pm. After 4:30pmstudents that have not been checked out and picked up are then merged to the Religion Room with the other students until aftercare closes at 5:30pm.

WHEN DO I PAY FOR AFTERCARE?

Aftercare submits your statement at the beginning of the following month of the service. Aftercare balances are due UPON RECEIPT and becomes delinquent if not paid by the 10th of that month. You can send in your payment in your child’s folder, but please use the aftercare designated gold envelope whenIt is returned to the school.
