Information and Instructions for Course Catalog Forms

The Course Catalog Information Form (CCI) and Course Catalog Topics Form (CCT) provide necessary information to UREG to populate the PeopleSoft Course Catalog. The College and Schools have been asked to designate an “approved sender” who submits these forms for entry into the Course Catalog. Both forms are fill-able Microsoft Word documents. Please fill out the form electronically, save the file, and e-mail it as an attachment to the next person in the approval process.

Course Catalog Information Form (CCI) The purpose of this form is to create a new course, inactivate a course no longer offered, or change an existing course. There are checkboxes on the form to indicate the type of change being requested.

 The “Effective Term” field, at the top left corner of the page, indicates the first instance the change requested will happen. For example, if the course title is changing, then this field will designate the first date this change will appear in the Schedule of Classes. In other words, PeopleSoft contains the functionality necessary to “future date” changes.

 The “Grading Basis” restricts the grading methods available for this course. The broadest, and default, choice for this field should be “Student Option.” This allows the course to be either graded or offered as Credit/No Credit. Choosing “Graded” will preclude students from taking this course as Credit /No Credit.

 If the form is being used to create a new course then “Proposed” fields should be completed, as well as any applicable “Present” fields.

 To make a change to an existing course, check the change requested and, at a minimum, provide the School/College, Department, Subject Area & Catalog Number, and Full title. In addition, the “Present” and “Proposed” fields should both be completed for any changes.

 To inactivate a course, check the change requested and, at a minimum, please provide the School/College, Department, Subject Area & Catalog Number, and Full title.

 Any forms submitted to be effective before Fall 2009 need both three-digit and future four- digit catalog numbers. If necessary, please use the comments section to indicate the four-digit catalog number.

Course Catalog Topics Form (CCT) The purpose of this form is to create a course topic associated with a specific course. Topics may only be designated for courses numbered in the _500-_599 range in each thousand series.  The “Effective Term” should specify either the single term or the first term that the topic will be offered.

 The form may be used to either add a topic as a first-time experimental course offering, or to add a topic that may be repeatedly taught.

 All applicable fields should be completed.
