INFORMAL CONFERENCES

January 5, 2012

TIME AND PLACE: The informal conferences held before an Agency Subordinate of the Board of Nursing were convened at 9:00 A.M., on January 5, 2012 in Board Room 1, Department of Health Professions, 9960 Mayland Drive, Suite 201, Richmond, Virginia.

AGENCY

SUBORDINATE: Jane Elliott, RN, PhD

STAFF PRESENT: Linda Kleiner, RN, Discipline Case Manager

OTHERS PRESENT: Cynthia Gaines, Adjudication Specialist

CONFERENCES

SCHEDULED: Teresa Davis, RN 0001-190433

Ms. Davis appeared, accompanied by attorney, Margaret Hardy.

Ms. Davis was informed a recommended decision will be made and mailed to her within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Davis unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Davis within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated.

CONFERENCES

SCHEDULED: Norma Crawford, LPN 0002-048341

Ms. Crawford appeared, accompanied by her daughter, Lakesia Tollier.

Ms. Crawford was informed a recommended decision will be made and mailed to her within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Crawford unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Crawford within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated.

CONFERENCES

Ms. Daniel appeared, accompanied by her mother Joyce Knick.

Ms. Daniel was informed a recommended decision will be made and mailed to her within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Daniel unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Daniel within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated.

Ms. Jones did not appear.

Based upon information provided by Ms. Gaines, Dr. Elliott ruled that adequate notice was provided to Ms. Jones.

A recommended decision will be made and mailed to Ms. Jones within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order.

CONFERENCES

SCHEDULED: Barbara Forbes, LPN 0002-080352

Ms. Forbes did not appear.

Based upon information provided by Ms. Gaines, Dr. Elliott ruled that adequate notice was provided to Ms. Forbes.

A recommended decision will be made and mailed to Ms. Forbes within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order.

Ms. Moreno appeared.

Ms. Moreno was informed a recommended decision will be made and mailed to her within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Moreno unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Moreno within such time.
