ORDINANCE NO. 002-162 AN ORDINANCE AUTHORIZING CHANGE ORDERS TO CONTRACTS APPROVED BY THE TRANSIT BOARD WHEREAS, Contracts originally approved by the Transit Board now require approval of contract change orders that are either an amount in excess of 10% of the original value of the contract or a cumulative amount in excess of $100,000; now, therefore: BE IT ORDAINED BY THE CHICAGO TRANSIT BOARD OF THE CHICAGO TRANSIT AUTHORITY: SECTION 1. The Chairman of said Board, or her designee, is authorized to approve the following contract change orders: 1.1 Contract C01FI9072 - Gonzalez Hasbrouck, Inc. Original Ordinance No. 001-152 Description: Professional Architectural & Engineering Consulting Services for design and construction phase services for Brown Line stations at Chicago, Sedgwick and Armitage. Change Order No. 1 to Phase II provides for the following: 1. Alterations to 939 W. Armitage $11,721.00 2. Alterations to 948 W. Armitage $44,842.00 3. Environmental Investigations $26,183.00 4. Alterations to 949 W. Armitage ($2,079.00) 5. Foundation Investigations ($2,079.00) 6. Escalators ($11,799.00) 7. Reallocation of Other Direct Costs $21,100.00 8. Reallocation of Labor from Phase III to Phase II $59,969.00 Total Change Order No. 1 to Phase II - $147,858.00 ORDINANCE NO. 002-162 (Continued) -2 Change Order No. 1 to Phase III provides for the following: 1. Alterations to 939 W. Armitage $11,721.00 2. Alterations to the Property at 948 W. Armitage $30,715.00 3. Environmental Investigations $5,969.00 4. Escalators ($11,721.00) 5. Reallocation of Labor from Phase III to Phase II ($59,969.00) 6. Reimbursable Costs ($21,100.00) Total Change Order No. 1 to Phase III - ($44,385.00) Revised Contract Amount: $3,885,995.00 1.2 Contract C01FI1007 - Laramore, Douglass and Popham, Inc. Original Ordinance No. 001-168 Description: Professional Architectural/Engineering Consulting Services for the design of two new traction power substations at 39th & Wentworth Avenue and at 50th & Wells Street. Change Order No. 1 to Phase I provides for the following: 1. Relocation of 39th and 50th Street Substations $97,844.00 2. Princeton Substation-63rd Street Direct Current Feed $41,196.00 Total Change Order No. 1 to Phase I - $139,040.00 Revised Contract Amount: $1,491,658.00 1.3 Contract C99FI7514 - Parsons Transportation Group, Inc. Original Ordinance No. 99-119 Description: Professional Architectural/Engineering Consulting Services for the design and construction phase services for the new Belmont and Fullerton Stations. ORDINANCE NO. 002-162 (Continued) -3 Change Order No. 4 to Phase I provides for a credit due to a miscalculation in the amount of fixed fee. Total Change Order No. 4 to Phase I - ($61,752.25) Change Order No. 1 to Phase II provides for the following: 1. Testing for Contaminated Soils $12,212.15 2. DBE Substitution ($31,408.13) 3. Fixed Fee Adjustment for Phase II ($223,782.27) Total Change Order No. 1 to Phase II - ($242,978.25) Change Order No. 1 to Phase III provides for the following: 1. DBE Substitution ($10,206.25) 2. Fixed Fee Adjustment for Phase III ($127,203.03) Total Change Order No. 1 to Phase III - ($137,409.28) Revised Contract Amount: $11,633,422.14 1.4 Contract C01FI9073 - Muller & Muller, Ltd. Original Ordinance No. 001-152 Description: Professional Architectural/Engineering Consulting Services for the design and construction phase services for the Brown Line stations at Kimball, Kedzie, Francisco, Rockwell, and Western. Change Order No. 1 to Phase II provides for the following: 1. Kimball Station Reconfiguration $8,917.20 2. Western Station Additional Stairs $14,155.48 3. Western Station Column Structure Replacement $16,560.85 Total Change Order No. 1 to Phase II - $39,633.53 ORDINANCE NO. 002-162 (Continued) -4 Change Order No. 1 to Phase III provides for the following: 1. Kimball Station Reconfiguration $7,959.45 2. Western Station Additional Stairs $14,147.79 3. Western Station Column Structure Replacement $33,664.79 Total Change Order No. 1 to Phase III - $55,772.03 Revised Contract Amount: $2,405,540.23 SECTION 2.
