Westlake High School Athletics Future Warrior Packet WESTLAKE HIGH SCHOOL A California Distinguished School WARRIOR ATHLETICS December 3, 2018 Welcome to Westlake High School A Tradition of Excellence in Interscholastic Athletics Dear Future Warriors, For many students at Westlake High School, interscholastic athletics are an integral part of their high school experience. Teams from Westlake High School are known for their high standards of excellence in academics and athletics. Please review the enclosed information carefully. We encourage freshmen to take advantage of summer workouts. Participating in summer workouts prepares student athletes for the upcoming season. "The Pride's Inside" is our school motto and Westlake "prides" itself on the outstanding sportsmanship, conduct, and competitive spirit of our athletes. We welcome the support of parents and appreciate the work of our fine booster organizations. We encourage you to support our booster clubs; their fundraising efforts substantially support our sports and activities programs. Students must go through an Athletic Clearance process before trying out for any sport. The process includes creating an online clearance account (instructions available at www.westlakeathletics.com), signing all required forms electronically, uploading a current physical, and printing, signing, and submitting a signed confirmation/consent letter to the Athletics Office. We strongly encourage every student to get involved in extracurricular activities during the next four years at Westlake High School. Through hard work, perseverance, and discipline, these next four years can be all you hope for and more. Remember that final semester/trimester grades in the eighth grade year determine eligibility for the fall freshman year, so finish strong! We look forward to meeting and working with all of you, Brad Katz Kyle Kegley Assistant Principal of Athletics Athletic Coordinator Westlake High School (805) 497-6711 (Ext. 1013) 100 N. Lakeview Canyon Road Fax (805) 497-7760 Westlake Village, CA 91362 www.conejousd.org/whs WESTLAKE HIGH SCHOOL Requirements and Conditions for Athletic Participation All students are encouraged to participate in the accomplished athletic tradition of Westlake High School. As a member of the Marmonte League and California Interscholastic Federation, all Westlake High School student athletes must comply with the following requirements for participation: Residence Eligibility: Students enrolled at Westlake High School are eligible to participate in athletics. Incoming ninth grade students are eligible no matter what school attendance area they live in. However, some 10th-12th grade students exercising their school of choice option may be restricted or denied athletic eligibility. Questions regarding residence eligibility should be brought to the attention of the Assistant Principal, Brad Katz, or the Athletic Coordinator, Kyle Kegley. Scholastic Eligibility: The CIF-SS and the Conejo Valley Unified School District require students, including entering ninth graders, to earn a 2.0 grade point average during the grading period prior to their season of sport. Ninth graders must earn a minimum of a 2.0 GPA on their eighth grade report card in June. Students not earning a 2.0 GPA may file a co-curricular appeal in order to regain their eligibility. The appeal decision will be based on any extenuating circumstances that may have caused the low grades and the student’s plan to improve. Students may only be granted one academic appeal during their eight semesters of attendance at Westlake High School. Furthermore, students are required to pass at least four subjects per grading period and be making progress toward graduation in order to participate. Athletic Clearance All students wishing to try out for any sport at WHS, must complete the following steps to be cleared to participate: 1) create an online clearance account, 2) create a clearance for each sport in which they wish to participate, 3) upload a completed, valid physical* form to the online account, 4) print and sign the consent letter from the online account and submit it to the athletics office. Detailed instructions are available on our athletics website www.westlakeathletics.com. The following forms are required for athletic clearance and are part of the online clearance process: Confirmation/Consent Form Authorization for Treatment of a Minor (included on physical form) Co-Curricular Code & Contract Informed Consent & Liability Release Concussion Management Form Sudden Cardiac Arrest Form With the exception of the Confirmation/Consent and the Authorization for Treatment of a Minor, parents and students will be required to read and electronically sign these forms online. The Confirmation/Consent letter must be signed and submitted to the Athletics Department, and the Physical Form which includes the Authorization for Treatment must be uploaded to the online account. *A valid physical is a required part of the Athletic Clearance process. In order for a physical to be valid, it must be completed using the official CVUSD physical form, must be signed by a parent, and must be signed and dated by a licensed health practitioner within the last calendar year. Once a physical expires, the student-athlete may not participate in any sport until a new, valid physical is submitted. Insurance: All student athletes must have accident-hospitalization insurance. Proof of adequate private insurance is required or students may purchase Myers-Stevens & Toohey & Co., Inc. school insurance to satisfy this requirement. Insurance forms for school insurance for next year will be available in the Athletic Office. Associated Student Body (ASB) Cards: All Westlake High School students participating in athletics (sports) or activities (band, tall flags, choir, cheerleading and dance) are encouraged to purchase ASB cards. The cost for an ASB Card was $60.00 for the 2018-2019 school year. The ASB card gets you into all athletic contests for free - a substantial savings. In addition, your ASB card gets you many discounts, including reduced prices for dances and the yearbook. Transportation: In order to preserve safety and team camaraderie, CVUSD students are required to travel on busses, vans, or other transportation provided by the District to designated school sponsored activities, practices, and/or games/events. Your continued support in this effort is very much appreciated. In order to continue to deliver this valuable service to all participating students, we ask that you contribute in the following manner: $ 124.00 First Sport/Activity Individual maximum contribution: $250.00 $ 71.00 Second Sport/Activity Family maximum contribution: $500.00 $ 55.00 Third Sport/Activity No student will be excluded from participation due to a lack of sufficient funds. Westlake High School strongly supports sports participation and multi-sport athletes. However, sports may make cuts based on the optimal number of athletes on a team. Participation in summer or off-season sport physical education classes does not guarantee a spot on the final team roster. Booster Clubs: Most sports have a booster club that is managed by parent volunteers. Parents are encouraged to participate. Please strongly consider supporting our booster clubs. Booster club contributions are not required in order for a student to participate in a sport or activity. No student will be excluded from any sport or activity due to a lack of sufficient funds. Call the Athletic Department or coach if you would like more information about getting involved with a booster club. Westlake High Letter Policy: Sport Certificates are awarded at all levels of participation. However, Letter Certificates are awarded to Varsity athletes only. To receive a Letter Certificate, an athlete must participate in at least one Varsity Marmonte League contest and must remain on the team through the conclusion of the season. Specific sports may have a variation of this general policy. Letter awards are based on participation and will not be withheld for disciplinary reasons unless specified by the Assistant Principal of Athletics. Letters and jackets may be purchased in the student store upon presentation of your certificate. Summer Programs: Some athletic programs sponsor summer activities either through the booster club or through an independent club. If you sign up for a sport at Future Warrior Night, you will be contacted with information if there is a summer program for that sport. Participation in summer and off-season programs do not guarantee that a student will make the team. Please check www.westlakeathletics.com for additional information. Sport Information Baseball Westlake Baseball consists of three levels: Varsity, JV and Frosh/Soph. Practices are held Monday through Friday at the WHS Baseball field. Freshman baseball players not participating in a WHS Fall or Winter sport should enroll in PE Off-Season
