How to Get a Work Permit For the summer program, all youth who will be age 14 or 15 June 1, 2021 will need a work permit. Youth under 14 are not permitted to work. If you require a work permit under Ohio law, you will not be permitted to work until Y.O.U. has your work permit. • First, you (and your parent/guardian) must fill out the application on the front page. Remember, your parent/guardian must sign this. • Second, a physician’s certificate must be filled- out by your doctor. Your physician must sign and stamp the physician’s section to indicate that you had a physical exam in the last twelve (12) months and are healthy enough to work. If you had a physical to play sports for school, check with your school to see if you can use that for your work permit application. • Third, take the completed application and physician’s certificate to your school or school board (depending on where you go to school) to have them issue a work permit. All school districts are different, but your school’s office should be able to tell you where to get the permit issued. Note: The 14-year-old or 15-year-old will need to bring proof of age to get a work permit. The youth can bring a state ID or birth certificate to do this. Remember, both the 14- and 15- year-old and the parent/guardian must go together to get the work permit. The following is a list of how to get a work permit in the majority of the schools that we have had teens from in the past summer job programs. However, this list does not include every school in Cuyahoga County. Please Note: We encourage you to call your school first to confirm the above information is still current. With the pandemic things are subject to changing quickly.
