MAINTAIN the patient in a community-based setting and prevent unnecessary treatments and hospitalizations. PROVIDE an interdisciplinary approach to care coordination including physical, emotional, spiritual and social service needs. Palliative care is specialized medical care for people with advanced and serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness, whatever the diagnosis. The goal is to improve quality of life for both the patients and their families. A team of doctors, nurses and other specialists work with patient care providers to deliver an extra layer of support. Palliative care is appropriate at any age and at any stage in a serious illness, and can be provided together with curative treatment. In 2017 6,648 palliative consultations were provided in hospitals. 167 patients received palliative care at home.
