Introduction The South East Grey Community Health Centre is a non-prot organization committed to providing primary care services and health programs to the residents of the Municipalities of Chatsworth, Grey Highlands, Southgate and West Grey. Our purpose is to further build on the existing support of health care through the dedicated sta in the South East Grey Community. South East Grey Community Health Centre oers a range of community-based health services provided by a multi-disciplinary team including Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Registered Dietitians, Physiotherapist, Kinesiologist, Chiropodist, Health Promoter, and Community Developer. Our priority population includes the following individuals: • Individuals with no family doctor or whose doctor is out of region • Seniors • Children and youth • The economically disadvantaged • Individuals with mental illness/addictions • People with mental and physical disabilities • Pregnant women and families It has been a busy year at the South East Grey Community Health Centre. We are now operating out of 5 locations. 1. Chatsworth: We are currently oering primary care services in the Township of Chatsworth Municipal Oce (Lower Level) 316837 Highway 6. Through our continued partnership with the Township of Chatsworth, there is a chair tness class running weekly out of the re hall. We are running an afterschool program at Holland-Chatsworth Central School. We are also exploring opportunities at Pleasant View Apartments. 2. Markdale: Our main location is at 55 Victoria Avenue. We provide primary care (Nurse Practitioners, Physicians) on site and individuals can come in to pick up an enrolment form or call 519-986-2222 for more information. All other services oered at the CHC are FREE, open and available to anyone residing in South East Grey. This includes: Lab Services, Foot Care, Social Work, Dietitian, Kinesiology, and Physiotherapy. We oer a full program schedule out of “The Well” located in our lower level, including: Tai Chi, Yoga, Pilates, Chair Fitness, Zumba, LGBTQ+ group, Community Kitchen, Good Food Box Program, Pole Walking, After-School programs and P.D. Day programs. There is a free community gym with treadmills, elliptical, bosu balls, yoga balls, matts, skipping ropes, weights, bikes (upright, recumbent and NuStep®). We oer smoking cessation counselling, computer/phone and internet access, clothing exchange, Income Tax Program, free shower facilities, skate exchange, community gardens and various drop-in programs. We also partner with community organizations to oer VON Smart Fitness, Alzheimer Society Grey Bruce Minds in Motion, United Way Financial Literacy, and Healthy Smiles Ontario-a free dental program that includes regular check-ups, preventative care and treatment for children and youth 17 and under. Dundalk-Erskine: Located at 90 Artemesia Street South, the South East Grey Community Health Centre oers a variety of health and wellness programs, including: Tai Chi, Yoga, Pilates, Chair Fitness, Community Kitchen, Income Tax Program, Grab & Go Breakfast, Youth Programs, Pole Walking, Drop-In Games, Let’s Get Craftin’, After School Programs and English as Second Language (ESL). We also partner on-site with community organizations such as Community Mental Health & Addictions (CMHA), United Way Financial Literacy, South East Grey Support Services-Life Directions, Upper Grand District School Board-Skills Upgrading, Grey Bruce Community Legal Clinic. We provide primary care (Nurse Practitioners, Physicians), foot care and lab services on site. Individuals can come in to pick up an enrolment form or call 519-986- 2222 for more information. 5. Flesherton-Grey Highlands Secondary School: “The Wellness Zone” is an onsite clinic for the youth attending Grey Highlands Secondary School. Each day, a member of the South East Grey Community Health Centre is on-site daily to support health and wellness needs: Social Work, Nurse Practitioner, Registered Dietitian. In addition to these main locations, we are providing programs and outreach to numerous schools and communities from Holstein and Eugenia to Maxwell and Flesherton.
