Appendix D: Community Focal Points Services for seniors are provided through a network of public and private agencies throughout the state. As a method of coordinating these services, the Aging and Disability Services Division (ADSD) has designated community focal points. Those entities funded by ADSD are indicated by the underlining. Aging and Disability Resource Centers are indicated by an asterisk, and provide the most comprehensive resource assistance to access services. The community focal points are predominately senior centers, which provide visible and accessible points where seniors may receive supportive services and nutrition. The community focal point is also the place where the senior may access information and assistance in locating other needed resources. Contact information is current as of December 2011.
